Optimum left heart filling pressure during fluid resuscitation of patients with hypovolemic and septic shock.

The effect of increasing filling pressures on cardiac performance was examined in 15 patients undergoing fluid resuscitation for hypovolemic and septic shock. The initial significant increase in pulmonary artery wedge pressure (WP) from 7.0 +/- 2.0 to 11.9 +/- 1.6 mm Hg was associated with an increase in stroke volume index (SVI) from 24.2 +/- 9.8 to 34.7 +/- 12.4 ml/M2 (p less than 0.01), left ventricular stroke work index (LVSWI) from 16.9 +/- 7.8 to 28.5 +/- 11.6 g x m/M2 (p less than 0.01) and cardiac index (CI) from 2.25 +/- 0.68 to 3.06 +/- 0.85 L/min x M2 (p less than 0.01). Sodium-Glucose Cotransporter 2 Inhibitors: An Emerging Therapeutic Approach for Ischemic Stroke Management.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i), a cornerstone therapy for type 2 diabetes mellitus (T2DM) and heart failure, are emerging as a promising therapeutic class for the management of ischemic stroke (IS). Given that T2DM is a significant risk factor for IS, understanding the potential neuroprotective role of SGLT2i is of paramount clinical importance. This review provides a systematic and logically structured synthesis of the current evidence, beginning with foundational preclinical studies in animal models that consistently demonstrate a reduction in infarct volume and improved

Axonal degeneration in hemorrhagic stroke: A systematic review.

Hemorrhagic stroke occurs due to a rupture of a blood vessel in the brain. This leads to initial mechanical damage at the site of injury and secondary injuries including axonal degeneration (AxD). Since axons are critical for all brain functions, we systematically reviewed studies that focused on AxD in two major types of hemorrhagic stroke, intracerebral hemorrhage and subarachnoid hemorrhage, to understand how and to what extent AxD develops and to interrogate underlying mechanisms and potential therapeutic targets. After screening 817 publications published until September 18, 2024, we iden

Pretreatment parameters associated with hemorrhagic transformation among successfully recanalized medium vessel occlusions.

Although pretreatment radiographic biomarkers are well established for hemorrhagic transformation (HT) following successful mechanical thrombectomy (MT) in large vessel occlusion (LVO) strokes, they are yet to be explored for medium vessel occlusion (MeVO) acute ischemic strokes. We aim to investigate pretreatment imaging biomarkers representative of collateral status, namely the hypoperfusion intensity ratio (HIR) and cerebral blood volume (CBV) index, and their association with HT in successfully recanalized MeVOs. A prospectively collected registry of acute ischemic stroke patients with MeV

Ligands of the Neuropeptide Y Y2 Receptors as a Potential Multitarget Therapeutic Approach for the Protection of the Neurovascular Unit Against Acute Ischemia/Reperfusion: View from the Perspective of the Laboratory Bench.

Ischemic stroke is the third leading cause of death and disability worldwide, with no available satisfactory prevention or treatment approach. The current treatment is limited to the use of "reperfusion methods," i.e., an intravenous or intra-arterial infusion of a fibrinolytic agent, mechanical removal of the clot by thrombectomy, or a combination of both methods. It should be stressed, however, that only approximately 5% of all acute strokes are eligible for fibrinolytic treatment and fewer than 10% for thrombectomy. Despite the tremendous progress in understanding of the pathomechanisms of

Predictors of hemorrhagic transformation after acute ischemic stroke based on the experts' opinion.

BACKGROUND: Hemorrhagic transformation (HT) is a common complication after ischemic stroke. It may be associated to poor outcomes. Some predictors of HT have been previously identified, but there remain controversies. OBJECTIVE: To describe the risk factors for HT more frequently reported by a panel of experts surveyed for this project. METHODS: We sent a standard questionnaire by e-mail to specialists in Vascular Neurology from 2014 to 2018. Forty-five specialists were contacted and 20 of them responded to the invitation. Predictors cited by three or more specialists were included in a tabl

Intracerebral hemorrhage for the palliative care provider: what you need to know.

Intracerebral hemorrhage (ICH) makes up 10%-30% of all strokes. Palliative care providers are often asked to get involved with ICH cases to aid with development of short-term and long-term goals. Prognosis can be calculated using the ICH score (based on Glasgow Coma Score score, ICH volume, presence of intraventricular hemorrhage, age, and location of origin) or the Essen score (based on age, NIH Stroke Scale [NIHSS], and level of consciousness). Do-not-resuscitate (DNR) status is important to discuss with families. Expert consensus states DNR is appropriate if the patient has two of the follo

Subcortical contributions to drawing.

Constructional impairment is often considered a sign of cortical damage. However, aphasia, agraphia, and apraxia, disorders traditionally deemed cortical, have been well described following subcortical lesions, suggesting an important role for subcortical structures in cognition. Drawing impairment following subcortical lesions has not been systematically explored or compared with that following cortical damage. We examined relative incidence and severity of drawing impairment after cortical and subcortical strokes and whether there were qualitative differences between these two groups' drawin

Clinical and artificial intelligence assessed echocardiographic predictors of outcomes after acute myocardial infarction.

BACKGROUND: Early risk stratification in patients after acute myocardial infarction (AMI) is critical for guiding therapy and resource allocation. While left ventricular ejection fraction (LVEF) is routinely assessed by echocardiography, novel markers offer additional prognostic utility but are not widely assessed due to time constraints or limited expertise. Artificial intelligence (AI) enables rapid, fully automated analysis of echocardiograms, producing standardized, comprehensive measurements. We evaluated the utility of AI-derived echocardiographic parameters on top of clinical variables

Right Heart Cardiac Magnetic Resonance Profiles and Cardiopulmonary Reserve in Older Adults Undergoing Long-Term Multicomponent Exercise: A 2-Year Longitudinal Study.

Right heart geometry and function are essential for preserving physical performance in healthy aging, yet evidence regarding how long-term supervised multicomponent exercise influences right heart characteristics and cardiopulmonary reserve over time remains limited. To address this gap, this interim analysis of a prospective, non-randomized, two-armed cohort study included 155 adults aged ≥ 65 years assigned to supervised multicomponent training (n = 75) or education control group (n = 80). Cardiac magnetic resonance imaging and cardiopulmonary exercise testing were performed at baseline, 1-y

Hemoglobin Glycation Index and Prognosis in Patients With Acute Myocardial Infarction Without Reduced Left Ventricular Ejection Fraction: A Multicenter Retrospective Cohort Study.

BACKGROUND: The prognostic value of the hemoglobin glycation index (HGI) in patients with acute myocardial infarction (AMI) without reduced left ventricular ejection fraction (LVEF) remains unclear. METHODS: This multicenter retrospective cohort study included 1706 patients with AMI without reduced LVEF. HGI was calculated as measured HbA1c minus predicted HbA1c. Restricted cubic spline (RCS) analysis was used to evaluate nonlinear associations between HGI and outcomes and identify the cutoff value. Piecewise Cox regression and subgroup analyses were also performed. The primary endpoint was m

Risk prediction in patients with heart failure with preserved ejection fraction: the LIFE-Preserved model.

BACKGROUND AND AIMS: Heart failure (HF) with preserved ejection fraction (HFpEF) constitutes a heterogeneous disease with varying prognosis. Given the rising incidence of HFpEF, accurate risk prediction for these patients is needed to identify high-risk individuals, who may benefit the most from preventive treatments. The LIFE-Preserved model was developed and validated for the prediction of individual short-term and lifetime risk for HF hospitalization or cardiovascular (CV) death in patients with HFpEF. METHODS: LIFE-Preserved was derived in 20 332 patients aged 40-90 years with a left vent

Interchangeability of the KCCQ-12 and KCCQ-23 Across >18,000 Participants Enrolled in 4 Large-Scale Trials of Heart Failure.

BACKGROUND: The 23-item Kansas City Cardiomyopathy Questionnaire (KCCQ-23) is formally qualified by the Food and Drug Administration as a valid patient-reported outcome measure for use in heart failure (HF) clinical trials. However, its length may increase respondent burden. The 12-item version (KCCQ-12) provides a more efficient alternative but was originally developed and validated in HF with reduced left ventricular ejection fraction (LVEF), and its performance relative to the KCCQ-23 has not been evaluated in HF with mildly reduced or preserved LVEF (HFmrEF/HFpEF). OBJECTIVE: The purpose

Left ventricular hypertrophy and myocardial fibrosis in heart failure with preserved ejection fraction: mechanisms and treatment.

Heart failure with preserved ejection fraction (HFpEF) accounts for nearly half of all heart failure cases and is characterized by phenotypical heterogeneity with a high prevalence of multiple, often overlapping cardiometabolic disorders. Comorbidities such as hypertension, obesity, or diabetes are present in many HFpEF patients and are hypothesized to contribute to adverse cardiac remodelling and myocardial fibrosis through a variety of haemodynamic and metabolic impairments, with nearly half of all HFpEF patients exhibiting left ventricular (LV) hypertrophy or concentric remodelling. Myocard

Prognostic impact of residual symptoms under low and normal physical performance in older patients hospitalized for worsening heart failure.

BACKGROUND: The New York Heart Association (NYHA) classification is well-established prognostic indicator and treatment goal in heart failure (HF). However, older patients may underreport subjective symptoms due to age-related physical decline. The prognostic significance of residual symptoms in older patients hospitalized for worsening HF, particularly across physical performance levels, remains unclear. We aimed to evaluate the significance of residual symptoms and physical performance at discharge on outcomes. METHODS: This FRAGILE-HF sub-analysis included 1272 consecutive hospitalized pat

Comparison of the efficacy of non-pharmacological interventions for patients with heart failure with preserved ejection fraction: protocol for a systematic review and network meta-analysis of randomised controlled trials.

INTRODUCTION: Heart failure with preserved ejection fraction (HFpEF) represents a common subtype of heart failure, accounting for approximately 50% of all heart failure cases. It is characterised by a left ventricular ejection fraction of ≥50%, alongside persistent symptoms and signs of heart failure. The pathophysiology of HFpEF is complex and multifactorial, involving mechanisms such as hypertension, coronary artery disease, left ventricular dysfunction, right ventricular impairment, systemic inflammation and metabolic abnormalities. Although pharmacological therapies play a central role in

Distinct Exercise Response Patterns in Patients With Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous disease characterized by exercise intolerance. Defining pathophysiologically distinct subgroups allows more personalized therapy, but efforts mainly relied on resting examinations. This study aimed to define HFpEF phenotypes based on exercise limitations using combined cardiopulmonary exercise testing with stress echocardiography. METHODS: A total of 913 patients with HFpEF were recruited from 4 third-line centers and divided into derivation (n=623) and validation cohorts (n=290). Unsupervised graph-based cl

Echocardiographic Markers of Cardiac Response to Therapy in Patients with Light Chain Amyloidosis.

PURPOSE: The aim of this study was to identify echocardiographic parameters which correlate with cardiac response to treatment in patients with light chain cardiac amyloidosis (AL-CA). METHODS: We identified 39 patients with AL-CA treated at City of Hope and divided them into two cohorts; cardiac responders as defined by improvement in global longitudinal strain (GLS) over time and cardiac non-responders as defined by no change or worsening in GLS. We then compared baseline demographics and echocardiographic parameters between the two groups. RESULTS: There were 16 cardiac responders and 23

Subclinical Myocardial Impairment Tracking the Comorbidity Burden: A Layer-Specific Strain and Myocardial Work Analysis in Patients With Preserved Ejection Fraction.

BACKGROUND: The Charlson Comorbidity Index (CCI) is a validated mortality predictor in heart failure, but its utility in tracking subclinical left ventricular (LV) mechanics remains unclear. We evaluated the relationship between cumulative comorbidity burden, assessed by a modified CCI (mCCI), and LV mechanics using layer-specific speckle tracking echocardiography and non-invasive myocardial work (MW) analysis. METHODS: Seventy-one stable outpatients with preserved LVEF (≥50%) and no overt heart failure or significant coronary artery disease were stratified by mCCI scores (adapted by excludin
