Corkscrew esophagus in an elderly patient: A case of diffuse esophageal spasm with literature review.

Diffuse esophageal spasm (DES) is a rare motility disorder characterized by uncoordinated esophageal contractions, often presenting with dysphagia and chest pain. This case describes a 70-year-old male who presented with atypical symptoms of DES, including episodic dysphagia to solids, postprandial vomiting, and chest pain triggered by cold liquids. Initially misdiagnosed as indigestion, further evaluation with barium swallow imaging revealed a "corkscrew" esophagus, leading to the diagnosis of DES. The patient's symptoms improved with isosorbide dinitrate. This case underscores the importance

The Use of Direct Oral Anticoagulants (DOACs) in the Geriatric Population-How to Overcome the Challenges of Geriatric Syndromes.

Because the number of elderly people is increasing worldwide, and the prevalence of cardiovascular risk factors and cardiovascular diseases increase with age, in current clinical practice we are faced with a large number of geriatric patients requiring oral anticoagulant treatment. Our review highlights some of the particularities of using direct oral anticoagulants (DOACs) in the geriatric population. We focused on the difficulties of managing DOAC treatment in the presence of geriatric syndromes. We highlighted the practical steps needed to overcome the challenges related to the risk of fall

Mediastinitis secondary to esophageal rupture; A case of Boerhaave syndrome.

Boerhaave syndrome is caused by spontaneous, vertical tears, which are most commonly seen in the distal esophagus. Boerhaave syndrome may develop after vomiting, severe cough, blunt trauma, and difficulty swallowing. In our study, we aimed to present a 66-year-old male who developed mediastinitis secondary to Boerhaave syndrome and was admitted to the emergency department of a secondary-level public hospital. In our case, despite early exploration, esophageal repair, mediastinal debridement, and subsequent esophageal stent application to the perforation, the condition was mortal. In this rare

Inclusion Body Myositis with Isolated Dysphagia: An Earlier-stage Diagnosis Using a Surgical Specimen from Cricopharyngeal Myotomy.

A 76-year-old man was admitted to our hospital with progressive dysphagia. His physical and neurological examination results were normal, except for dysphagia. Videofluoroscopy of swallowing demonstrated a cricopharyngeal bar, a highly specific finding for inclusion body myositis (IBM). A cricopharyngeal muscle specimen was obtained during cricopharyngeal myotomy to improve his symptoms. Muscle histopathology successfully led to the diagnosis of IBM. Dysphagia can occasionally precede limb weakness in IBM, and an early histopathological diagnosis using limb muscles is challenging. In such case

Imaging approach to ingested foreign bodies in the neck.

Foreign body ingestion is a common clinical occurrence worldwide, with high morbidity in the pediatric population and in adult patients with intentional attempts. Coins and button battery ingestions are more common among children. Bone impaction and swallowed dentures are usually seen in older adults. While most ingested foreign bodies pass through the gastrointestinal tract spontaneously with no complications, some require endoscopic and/or surgical intervention. Complications such as pharyngoesophageal ulceration, perforation, stricture, and deep neck infection can develop without timely dia

Uncommon cause of chest pain: Case report of esophageal foreign body diagnosed by chest computed tomography scan.

A 44-year-old man with a history of epilepsy presented with mild, persistent chest pain lasting 3 days, despite normal cardiac evaluations. A chest computed tomography scan revealed 3 artificial dental prostheses lodged in his esophagus, which the patient had inadvertently swallowed during a recent seizure. Endoscopic removal of the foreign bodies resolved his chest pain without complications. This case emphasizes the importance of considering esophageal foreign bodies as a differential diagnosis for persistent chest pain, particularly in patients with neurological conditions, and highlights t

Brunner Gland Hamartomas-Uncommon Presentations and Endoscopic Management.

Brunner gland hamartoma (BGH) is a rare condition that requires a high clinical suspicion to diagnose. Large hamartomas may initially present with iron deficiency anemia (IDA) or symptoms suggesting intestinal obstruction. Barium swallow may demonstrate the lesion, but endoscopic evaluation is the acceptable first line management unless a concern for underlying malignancy. The present case report and literature review highlight the uncommon presentations and endoscopic role in large BGHs management. Internists should consider BGH in their differential, especially in patient with occult bleedin

Serratus Anterior-Rib Composite Flap as a Novel Approach for Tracheal Reconstruction.

OBJECTIVE: To report a novel case of tracheal reconstruction using a serratus anterior-rib composite flap. METHODS: Case report and literature review. RESULTS: A 46-year-old male with a 4 cm anterior tracheal wall defect underwent reconstruction with a serratus anterior-rib composite flap. The patient experienced excellent results regarding phonation, swallowing, and cosmesis. CONCLUSION: The serratus anterior-rib composite flap appears to be a suitable candidate for tracheal reconstruction and merits further analysis in this context. The flap's intrinsic incorporation of a perfused rib seg

Speech-induced Atrial Tachycardia with Presyncope.

Speech-induced atrial tachycardia (AT) with presyncope is extremely rare. A 52-year-old woman employed at a supermarket reported recurrent presyncope while speaking out loud at her job. Holter electrocardiography revealed AT while swallowing without presyncope. The patient's blood pressure decreased during AT, and she experienced presyncope while saying "IRASSHAIMASE" loudly during a tilt table test. Accordingly, bisoprolol 1.25 mg was prescribed, and the patient did not experience episodes of presyncope with recurrence of AT for 2 years. This case suggests that provocation of arrhythmia in th

Pneumomediastinum and subcutaneous emphysema post cocaine and amphetamine insufflation.

A 20-year-old gentleman presented with blood-streaked vomitus after insufflation of an unknown amount of powder cocaine and amphetamine. This was taken with an unspecified amount of alcohol. Other notable symptoms were dysphagia, chest pain, palpitations and the patient reported a 'crunchy' sensation in his chest. A chest x-ray revealed pneumomediastinum and a computerised tomography (CT) trauma confirmed these findings with associated subcutaneous emphysema without an identifiable cause. Follow up investigations included a barium swallow and gastroscopy which showed no obvious perforation but

Generation of DMBi002-A human induced pluripotent stem cell line from patient with Spinal muscular atrophy type 3.

Spinal Muscular Atrophy (SMA) is a genetic neuromuscular disease caused by mutations inSMN1 gene encoding survival motor neuron (SMN) protein. Lack of this protein leads to progressive loss of motor neurons and therefore to gradual loss of signal transmission between motor neurons and skeletal muscle cells. As a consequence, patients develop muscle atrophy and lose the ability to move independently, what is also related to problems with breathing and swallowing. Here, we describe the generation of human induced pluripotent stem cells (hiPSC) from peripheral blood mononuclear cells (PBMC) of ad

Physical therapy of temporomandibular disorder in a child with arthrogryposis multiplex congenita: A case report and literature review.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a rare congenital disorder characterized by the occurrence of polyarticular contractures. Temporomandibular disorders (TMD) affect 25% of patients with arthrogryposis. CLINICAL PRESENTATION: In this case report, the authors describe rehabilitation of a child between 8 and 24 months of age with AMC accompanied by Pierre-Robin Sequence. Physiotherapy directed to the TMJ included manual therapy, food intake training, and sensory training. CONCLUSION: Increase in the mobility of the TMJ and improvement in food intake, swallowing, speech, an

A case series of intestinal obstruction secondary to shiitake mushroom intake during Chinese New Year.

Shiitake mushroom is a common ingredient in East Asian cuisines. Food processing/preparation can cause the mushroom to be soft and slimy, leading to accidental swallowing. Due to its high insoluble fibre content, it remains the same size and shape in the intestinal tract. We present two cases of small bowel obstruction caused by shiitake mushroom requiring surgical intervention. Preoperative imaging showed dilated small bowel with a suspicious mass in the ileum. However, the exact cause was unclear. For both cases, exploratory laparotomy and enterotomy were then performed and undigested shiita

Cervical Hyperextension Treated by Posterior Spinal Correction and Fusion in A Patient with Ullrich Congenital Muscular Dystrophy: A Case Report.

CASE: An 18-year-old man with Ullrich congenital muscular dystrophy (UCMD) noted difficulty of looking forward and discomfort swallowing and breathing because of his hyperextended neck. We treated his cervical deformity with posterior spinal correction and fusion alone. He underwent a tracheotomy because of lung function deterioration 2 years after cervical surgery. The tracheotomy was performed safely because the anterior cervical spine anatomy was normalized and soft tissues around trachea were preserved by the posterior cervical correction. CONCLUSION: Cervical hyperextension can be a prob

[Antibiotic therapy in dental practice].

Dentists and dental surgeons very frequently prescribe antibiotics to their patients. In a small percentage of cases, that is appropriate; however, patients can often also heal without antibiotic therapy. Microbiological analysis is only carried out in a very limited number of cases, and is complex and time-consuming. A small assortment of oral antibiotics is usually sufficient. Antibiotics are indicated when dental infection is accompanied by fever or indications of infection of a more systemic nature, such as trismus or lymphadenopathy. A patient with cellulitis of the head and neck area, wi

A potential role of toll-like receptors, IFN-γ and the phosphatidylinositol 3-kinase pathway in the pathogenesis of acquired mediastinal lymphatic malformation.

Sarcoidosis is a multisystem disorder with non-caseating granulomas in various organs. The etiology of sarcoid granuloma formation is not clear and likely an antigen-induced process. We came across a previously treated sarcoidosis patient who presented with worsening dyspnea on exertion for several months and several days of difficulty swallowing. On Chest CT imaging, large posterior mediastinal mass was found that subsequently diagnosed as macrocystic lymphatic malformation after surgical resection. Pathophysiology of development of acquired lymphatic malformations in a sarcoidosis patient is

Rheumatoid arthritis in temporo-mandibular joint: A review.

This article summarizes the temporomandibular joint (TMJ) rheumatoid arthritis (RA). In particular, TMJ-RAs are collected in a short summary by examining every aspect of RA; the treatment of TMJ-RA is also briefly mentioned. TMJ-RA is usually characterized by bilateral pain, tenderness and swelling, and limitation of jaw movements. Due to these symptoms, patients may experience limitations in their daily activities, such as eating, speaking, and swallowing. MEDLINE and Scopus databases were searched electronically using the terms "temporomandibular joint" and "rheumatoid arthritis." The electr

Electrophysiology of Cranial Nerve Testing: Cranial Nerves IX and X.

The cranial nerves IX and X emerge from medulla oblongata and have motor, sensory, and parasympathetic functions. Some of these are amenable to neurophysiological assessment. It is often hard to separate the individual contribution of each nerve; in fact, some of the techniques are indeed a composite functional measure of both nerves. The main methods are the evaluation of the swallowing function (combined IX and X), laryngeal electromyogram (predominant motor vagal function), and heart rate variability (predominant parasympathetic vagal function). This review describes, therefore, the techniq
