Plantar Injuries in Runners: Is There an Association With Weekly Running Volume?

Running is an athletic activity that is increasingly gaining popularity. Despite its benefits, there are many suspected risk factors for running-related overuse injuries. The objective of this study is to describe injuries and clinical symptoms observed on the sole of the foot in runners, giving special attention to the weekly running volume. The literature presented in this narrative review is based on a non-systematic search of the Medline, Google Scholar, and ResearchGate databases and focuses on foot injuries (the full spectrum of the foot pathology from bones to tendons and plantar fascia

Myotendinous Rupture of the Posterior Tibial Tendon with Associated Fibular Fracture: A Case Report.

CASE: A 20-year-old man presented after a fall from an 8-foot ladder. Radiographs revealed pronation external rotation injury. Closed reduction and splinting was performed in the emergency department. Surgery was planned 6 days later to include open reduction and internal fixation of the fibula and syndesmosis, and open repair of deltoid ligament. Intraoperatively, a myotendinous rupture of the posterior tibial tendon (PTT) was identified and repaired. CONCLUSION: PTT ruptures are rarely reported but should be suspected in high-energy mechanism ankle fractures in all patients. Direct repair c

Dermal Allograft and Biceps Superior Capsule Reconstruction for Massive Irreparable Rotator Cuff Tears.

Since the use of autologous fascia lata for superior capsule reconstruction of massive irreparable rotator cuff tears (MIRCTs), the technique has evolved into various modifications, including dermal allografts, long head of the biceps tendon (LHBT), and combinations of both, which will be discussed in this article. After making sure the remnant cuff cannot be restored to the anatomical footprint of the supraspinatus, a double-loaded or triple-loaded, suture-based anchor is inserted 5-8 mm posterior to the bicipital groove to secure the long head of the biceps (LHBT) initially. A bone trough is

Applications for the Biceps Tendon in Rotator Cuff Repair.

The long head of the biceps has traditionally been ignored, tenodesed, or tenotomized during shoulder arthroscopy. However, more recently, it has been recognized as robust autograft tissue that may be mobilized or harvested to aid in a range of augmentation and reconstructive procedures. Several applications are included in this infographic. An intertendinous augmentation (biologic superior capsular reconstruction) technique is indicated in massive tears that are either partially or completely reparable. The intact biceps tendon is mobilized from the groove, transposed and secured to the great

Surgery to remove the plate from the distal radius with simultaneous reconstruction of the lacerated flexor pollicis longus tendon performed under WALANT: A case report.

The tendon rupture was caused by the protruded distal edge of the palmar plate inserted for fixation of the distal radial fracture 2 years earlier. The patient was a 28-year-old patient who had stopped bending the distal phalanx of his right thumb 2 months prior to the presentation. Performance of anaesthesia and operation is presented in details. The operation went smoothly, bleeding was minimal despite the relatively extensive wound, and the patient felt only little pain at the end of the operation.

Editorial Commentary: Subacromial Balloon Spacer Is an Alternative to Rotator Cuff Repair in Older, Lower-Demand Patients Who Prioritize Pain Relief and Faster Recovery Over Strength Restoration.

The subacromial balloon spacer was originally designed to treat massive irreparable rotator cuff tears in patients with pain but preserved function, and favorable results have been reported. However, surgeons have expanded its application to include use as an adjunct for salvage procedures such as superior capsular reconstruction and tendon transfer, as a delivery device for steroids or biologics, and as an augmentation in the setting of primary or revision rotator cuff tear. When adapting technology to new techniques, one must ask, What is the basis by which we can expect success? In rotator

Editorial Commentary: Augmentation With Allograft Patch Results in Similar Outcomes to Anterior Cable Reconstruction With Autograft Biceps Tendon for Chronic, Retracted, Anterior, L-Shaped Supraspinatus Tears.

Chronic retracted rotator cuff tears are difficult entities to treat. L-shaped tears are a particular subset of such rotator cuff tears that pose challenges for surgeons attempting to reduce the supraspinatus tendon back to the greater tuberosity. Lack of full coverage of the tuberosity, need for medialization of the tendon, undue tension, and incomplete reconstitution of the rotator cable are some of the reasons L-shaped retracted tears of the supraspinatus can be challenging. Anterior cable reconstruction (ACR) is a technique that has gained increasing recent popularity, as is the use of pat

Rip-Stop Fixation Constructs for Rotator Cuff Repair.

In arthroscopic rotator cuff repair, poor tendon quality, medially based tears, lateral tendon loss, or limited tendon mobility can all preclude the use of double-row suture constructs, presenting a challenge in achieving secure fixation and tendon-to-bone healing. Rip-stop suture configurations can be used in these settings to improve resistance to tissue cutout and provide enhanced biomechanical characteristics compared with standard single-row repairs. The load-sharing rip-stop technique uses 2 double-loaded medial suture anchors, which are placed adjacent to the articular margin, and 1 rip

Tenodesis yields better functional results than tenotomy in long head of the biceps tendon operations-a systematic review and meta-analysis.

BACKGROUND: Pathology of the long head of the biceps tendon (LHBT) is a common disorder affecting muscle function and causing considerable pain for the patient. The literature on the two surgical treatment methods (tenotomy and tenodesis) is controversial; therefore, our aim was to compare the results of these interventions. METHODS: We performed a meta-analysis using the following strategy: (P) patients with LHBT pathology, (I) tenodesis, (C) tenotomy, (O) elbow flexion and forearm supination strength, pain assessed on the ten-point Visual Analog Scale (VAS), bicipital cramping pain, Constan

No Difference in Clinical Outcomes for Arthroscopic Suprapectoral Versus Open Subpectoral Biceps Tenodesis at Midterm Follow-up: A Randomized Prospective Analysis.

BACKGROUND: We have previously reported the 1-year outcomes of arthroscopic suprapectoral biceps tenodesis (ASPBT) versus open subpectoral biceps tenodesis (OSPBT) for the management of long head of the biceps tendon (LHBT) pathology. While patients had similar 1-year biceps muscle strength and pain, longer-term functional outcomes are unknown. PURPOSE: To directly compare clinical outcomes of ASPBT versus OSPBT with interference screw fixation, distal to the bony bicipital groove, at a minimum of 2 years' follow-up. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS:

Are there differences in arthroscopic and histological features between traumatic and degenerative rotator cuff tears in elderly patients? A prospective dual-center analysis.

BACKGROUND: Discriminating traumatic rotator cuff tears (RCTs) from degenerative RCTs is sometimes difficult in elderly patients because the prevalence of asymptomatic RCTs increases with age. Little intraoperative information is available on the characteristics of traumatic and degenerative RCTs in elderly patients. The purpose of this study was to compare the arthroscopic findings and histological changes of the coracoacromial ligament (CAL) between traumatic and degenerative RCTs in elderly patients. METHODS: Forty-two shoulders of 42 patients aged ≥ 65 years underwent arthroscopic rotator

The effect of sterilization and storage on the viscoelastic properties of human tendon allografts.

Allografts have become increasingly preferred for anterior cruciate ligament replacement purposes. The risk of infections necessitates thorough sterilization procedures, and the allografts usually need to be stored prior to surgery. Classical mechanical tests have been performed with various types of tendons, however, tibialis anterior and peroneus longus tend to suffer the least biomechanical changes after irradiation. Only few results are available of the strain and creep behaviour of tendons, even though this information is necessary to provide suitable allografts. The aim of the present st

Adipose Stem Cell-Derived Exosomes Recover Impaired Matrix Metabolism of Torn Human Rotator Cuff Tendons by Maintaining Tissue Homeostasis.

BACKGROUND: Adipose stem cell-derived exosomes (ASC-Exos) are reported to effectively prevent muscle atrophy and degeneration of torn rat rotator cuff, but their influence on human samples and their potential mechanism are still unclear. PURPOSE: We aimed to investigate the effects of ASC-Exos on the metabolic activities of torn human rotator cuff tendons and explore the potential mechanism behind it. STUDY DESIGN: Controlled laboratory study. METHODS: Diseased supraspinatus tendons were harvested from 15 patients with a mean ± SD age of 65.8 ± 3.2 years who underwent reverse shoulder arthr

Result of delayed repair of quadriceps muscle following a sharp cut injury.

We herewith report a case of a 22-year-old man who suffered from a transverse laceration of the quadriceps muscle. The patient presented to us after 3 months of the injury with an inability to extend the knee. We undertook a surgical repair of the muscle tear using the modified Mason-Allen technique and a polypropylene mesh augmentation. To the best of our knowledge, the use of polypropylene mesh for repair augmentation of mid-substance tear of quadriceps muscle has never been described in the literature. The patient had achieved a full active knee extension at 9 months following the surgery.

Editorial Commentary: Shoulder Rotator Cuff Tendon Repair Using Bone Marrow Aspirate: Stem Cell Quantity Does Not Equal Quality.

The use of biological agents in orthopaedic surgery is rapidly evolving. The potential to augment the healing environment at a surgical repair site is an especially exciting possibility. There are a few popular biological agents, including platelet-rich plasma, concentrated bone marrow aspirate (BMA), and adipose-derived connective tissue progenitor cells. BMA is an especially appealing biological agent because it can be harvested from a variety of sources, including the iliac crest, distal femur, and proximal humerus. As a result, BMA is readily accessible with minimal added surgical time and

Use of Gracile and semi-tendinosus tendons (GRAST) for the reconstruction of irreparable rotator cuff tears.

BACKGROUND: Irreparable rotator cuff tears are common and difficult to treat. Techniques for "filling the loss of substance" require fixation to the rotator cuff stump (tendon augmentation) or to the glenoid (superior capsular reconstruction), which are complicated by the narrow working zone of the subacromial space. The main objective of this study was to determine whether a braided graft of gracilis (GR) and semitendinosus (ST) could fill a loss of tendon substance from an irreparable rupture of the supra- and infraspinatus, by fixing the graft to the greater tuberosity and the spine of the

Patient-related outcome measurements after operative and conservative management of traumatic proximal adductor longus avulsion injuries.

PURPOSE: In the rare case of complete proximal adductor longus avulsion injuries, scientific recommendations of surgical or conservative injury management are limited to small case series and reviews of small cohorts about treatment examples. A clear consensus regarding optimal management so far has not been made. Thus, the aim of our study was to investigate patient-related outcome measurements (PROM) after either operatively or conservatively treated patients in order to evaluate and display own decision criteria for one of the two treatment options. METHODS: In this retrospective case cont

Are Implant Choice and Surgical Approach Associated With Biceps Tenodesis Construct Strength? A Systematic Review and Meta-regression.

BACKGROUND: Despite the increasing use of biceps tenodesis, there is a lack of consensus regarding optimal implant choice (suture anchor vs interference screw) and implant placement (suprapectoral vs subpectoral). PURPOSE/HYPOTHESIS: The purpose was to determine the associations of procedural parameters with the biomechanical performance of biceps tenodesis constructs. The authors hypothesized that ultimate failure load (UFL) would not differ between sub- and suprapectoral repairs or between interference screw and suture anchor constructs and that the number of implants and number of sutures

Mini thigthrope® fixation of unstable bony avulsion of the extensor hallucis longus tendon.

Reports of isolated avulsion fracture of the distal phalanx of the hallux that comprise the functionality of the extensor hallucis longus tendon (EHL) are scarce and treatment for such injury has only been described in isolated single case reports. Two patients with an unstable avulsion fracture of the distal phalanx treated with reinsertion of the EHL with a Mini ThigthRope® system are presented in this paper. Two patients whom suffered an extreme plantarflexion mechanism sought attention in our clinic. Plain x-rays depicted a displaced and angulated bony avulsion fracture of the base of the
