The sympathetic nerve activity in heart during cardiopulmonary bypass was studied in 20 patients. The rate of release of dopamine-beta-hydroxylase from cardiac nerves was used as an indicator. It was found that the thoracotomy alone does not represent such a stimulus which would lead unambiguously to sympathetic overactivity in heart. Cardiopulmonary bypass represents, on the other hand, an intensive sympathomimetic stimulus, as it was demonstrated by significnat increase of coronary A-V difference negativity of dopamine-beta-hydroxylase as early in 15th minute of its duration. The washout of

Nineteen seriously disabled patients were operated upon a second time (using closed-heart technique) for recurrence of symptoms of mitral stenosis. The hospital mortality rate was 10.5%; there was one late death. Ten patients (52%) were well and working six months to five years after the second operation. In considering the cause of restenosis it was noted that nine patients had inadequate opening of the mitral valve at first operation. In five cases (26%) rheumatic activity appeared to be solely responsible for restenosis. In a further four cases (21%) it was considered that rheumatic activit

A 76-year-old patient, admitted to the department of cardiac surgery, anticoagulated, started with acute dysphagia. An upper endoscopy revealed a blue mucosa from the upper to distal esophagus, with narrowing of the lumen caused by an extrinsic compression. A computed to¬mography (CT) of the thorax was performed, showing a 85x55 mm mediastinal hematoma causing extrinsic compression of the esophagus. Subsequently, a contrast-enhanced CT scan was performed, confirming a contained rupture of descending thoracic aortic dissection as the cause of the hematoma.

Aortic root dilation and aortic insufficiency are prominent causes of morbidity in Marfan syndrome. These pathologies necessitate surgical repair, including aortic root and aortic valve replacement procedures, to improve prognosis. Coronary artery aneurysms, particularly giant coronary ostial aneurysms, are rare complications of these surgeries in the Marfan population. Due to the significant life-threatening sequelae of coronary artery aneurysms, it is imperative to bring attention regarding this complication to the radiologist assessing thoracic imaging in this patient population.
