Analysis from the EMPA-REG OUTCOME® trial indicates empagliflozin may assist in preventing the progression of chronic kidney disease in patients with type 2 diabetes irrespective of medications that alter intrarenal hemodynamics.

In patients with type 2 diabetes mellitus (T2DM) and cardiovascular (CV) disease, empagliflozin (EMPA) decreased progression of chronic kidney disease (CKD), likely via a reduction in intraglomerular pressure. Due to prevalent comorbidities, such as hypertension and albuminuria, patients often receive other agents that alter intrarenal hemodynamics, including angiotensin converting enzyme inhibitors/angiotensin receptor blockers (ACEi/ARBs), calcium channel blockers (CCBs) and diuretics. Nonsteroidal anti-inflammatory drugs (NSAIDs) may also be used by some individuals. In this exploratory, no

Artificial Discs in Cervical Disc Replacement: A Meta-Analysis for Comparison of Long-Term Outcomes.

BACKGROUND: Cervical disc replacement (CDR) has emerged as an alternative to anterior cervical discectomy and fusion for the surgical treatment of degenerative cervical disc disease. Although comparison of the 2 techniques has been studied in the literature, a thorough assessment of all artificial discs between each has not been performed. The objective of the present study was to examine the long-term outcomes of 5 artificial discs. METHODS: An electronic literature search was conducted for studies of CDR devices for all years available. Only articles in English were included. Heterotopic os

OCCUPATIONAL HAZARDS AS A RISK FACTOR OF ONSET AND UNFAVORABLE OUTCOME OF ISCHEMIC HEART DISEASE.

Objective -to identify the role of occupational hazards (OH) as a risk factor of onset and unfavorable outcome of coronary artery disease (CAD). Retrospective study included analysis of 307 case reports of CAD patients, including 59 patients with long term history of OH exposure. Prospective study included observation of 244 STEMI patients during their in-patient treatment, including 113 patients with prior exposure to OH. OH were demonstrated to contribute to the risk of CAD, and to be directly involved in atherosclerotic coronary artery (CA) injury, its role being equal to that of the classi

Evaluation of the DSM-5 Severity Specifier for Bulimia Nervosa in Treatment-Seeking Youth.

A new severity specifier for bulimia nervosa (BN), based on the frequency of inappropriate weight compensatory behaviours (e.g., laxative misuse, self-induced vomiting, fasting, diuretic misuse, and excessive exercise), has been added to the most recent (fifth) edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) as a means of addressing variability and heterogeneity in the severity of the disorder. While existing research provides support for the DSM-5 severity specifier for BN in adult patients, evidence for its validity and clinical utility in youth is currently lack

Efficacy and safety of etanercept in the treatment of sciatica: A systematic review and meta-analysis.

Etanercept might be promising to alleviate sciatica caused by lumbar disc herniation and spinal stenosis. However, the results remained controversial. We conducted a systematic review and meta-analysis to evaluate the efficacy of etanercept in patients with sciatica. PubMed, EMbase, Web of science, EBSCO, and Cochrane library databases were systematically searched. Randomized controlled trials (RCTs) and Controlled clinical trials (CCT) assessing the efficacy of etanercept on sciatica caused by lumbar disc herniation and spinal stenosis were included. Two investigators independently searched a

Motor control exercise for symptomatic lumbar disc herniation: protocol for a systematic review and meta-analysis.

IntroductionLumbar disc herniation (LDH) is a common condition in adults and can impose a heavy burden on both the individual and society. It is defined as displacement of disc components beyond the intervertebral disc space. Various conservative treatments have been recommended for the treatment of LDH and physical therapy plays a major role in the management of patients. Therapeutic exercise is effective for relieving pain and improving function in individuals with symptomatic LDH. The aim of this systematic review is to evaluate the effectiveness of motor control exercise (MCE) for symptoma

Erythropoietin production by PDGFR-β(+) cells.

PDGFR-β-expressing cells of the kidneys are considered as a relevant site of erythropoietin (EPO) production. The origin of these cells, their contribution to renal EPO production, and if PDGFR-β-positive cells in other organs are also capable to express EPO are less clear. We addressed these questions in mice, in which hypoxia-inducible transcription factors were stabilized in PDGFR-β(+) cells by inducible deletion of the von Hippel-Lindau (Vhl) protein. Vhl deletion led to a 600-fold increase of plasma EPO concentration, 170-fold increase of renal EPO messenger RNA (mRNA) levels, and an incr

Reduced Fas ligand-expressing splenic CD5+ B lymphocytes in severe collagen-induced arthritis.

INTRODUCTION: The objective was to study immune regulation in a mouse model of rheumatoid arthritis that exhibits considerable heterogeneity of disease activity. METHODS: T-cell receptor transgenic mice, in which nearly all CD4+ T cells recognize a single peptide of type II collagen, were immunized with collagen and observed for development of arthritis for 4 weeks. At 28 days post-immunization, splenocytes were analyzed by flow cytometry and in vitro assays for markers of immune activation and regulation. RESULTS: Disease severities ranging from 0 to 12 (on a 12-point scale) were observed.

Characterization of binding sites for the omega3 receptor ligands [3H]PK11195 and [3H]RO5-4864 in human brain.

The kinetics and pharmacology of the isoquinoline and benzodiazepine binding sites of the omega3 or peripheral-type benzodiazepine receptors were studied using the specific ligands [3H] 7-chloro-5-(4-chlorophenyl)-1,3-dihydro-1-methyl-2H-1,4-benzodiazepin -2-one ([3H]PK11195) and [3H]1-(2-Chlorophenyl)-N-methyl-N-(1-methylpropyl)-3-isoquinolinecarb oxamide ([3H]RO5-4864), respectively. Binding of both ligands was saturable, reversible, displayed nanomolar affinity, and best fit to a single site model. Occipital cortex and cerebellum displayed highest and lowest densities of binding sites respe

Lactobacillus-Based Microbiome Therapy for Acne Vulgaris: A GRADE Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Acne vulgaris is one of the most prevalent disorders affecting 9%-10% of the global population, representing as papules, pustules, and comedones, with a pathogenesis involving increased sebum production, C. acnes colonization, and inflammation. Conventional treatments like retinoids and antibiotics often cause side effects, thus diverting attention toward probiotics as an alternative therapy. Lactobacillus probiotics, having their immunomodulatory, anti-inflammatory, and antimicrobial properties, are useful in managing acne by reducing inflammation and oxidative stress with proved

Spatiotemporal heterogeneity of immune cells in knee osteoarthritis: Cytokine actions and targeted therapies.

Knee osteoarthritis (KOA) is a whole-joint disease involving the synovium, articular cartilage, subchondral bone, and infrapatellar fat pad. Immune responses contribute to its onset and progression. As immune cell composition and functional states change across disease stages and tissue environments, cytokine sources, activity, and effects also vary over time and among tissues. Most studies have examined individual immune cell types, cytokines, or joint tissues in isolation, limiting their ability to explain the diversity of KOA clinical phenotypes and the variable outcomes of cytokine-targete

Who benefits most from couple relationship education: A machine learning approach.

OBJECTIVE: Couple relationship education (CRE) seeks to enhance relationship functioning and prevent deterioration of relationship quality over time. However, impacts of CRE are mixed and often appear to be influenced by the characteristics of the couples receiving the intervention. To provide effective interventions, a better understanding of the couples who are most likely to benefit from CRE is needed. Unfortunately, the existing literature has failed to account for the complex and interdependent nature of pretreatment risk factors, leading to inconsistent and inconclusive results. METHOD:

Liposome-mediated delivery of a ruthenium-based metallodrug to overcome cisplatin resistance in osteosarcoma.

The treatment of osteosarcoma has remained largely unchanged over the past decades. Its low incidence, predominantly affecting children and young adults, combined with marked biological heterogeneity, poses major challenges to the development of effective therapies, and currently approved regimens often yield variable responses. Additionally, the frequent emergence of drug-resistant phenotypes in these mesenchymal tumors further compromises treatment success. As a result, despite substantial research efforts, patients with osteosarcoma still face a poor prognosis, underscoring the urgent need

The impact of MRI on real-world staging pathways and management timelines in pancreatic cancer: A systematic review.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) has a poor prognosis, and accurate staging is crucial to avoid unnecessary surgery. MRI may improve the detection of occult liver metastases when compared with CT. The impact of adding MRI on treatment timelines and cost-effectiveness remains uncertain. This systematic review synthesises the available evidence of incorporating MRI alongside CT for pre-operative staging of pancreatic cancer. METHODS: We systematically searched MEDLINE, Embase, Web of Science, and the Cochrane Library (2005-2025) for studies comparing staging pathways using CT
