An evaluation of anti-ulcer effects of mono- and dibasic carbamate local anaesthetics was performed using two experimental models of gastric ulcers, i.e. those induced by stress, or by ligation of the pylorus. Somatostatin in gastric juice was determined in normal subjects and patients with duodenal ulcer. Gel exclusion chromatography of gastric juice revealed that the main immunoreactivity existed at the position of somatostatin-14. We report a clinical case of a woman, 56 years old, with a history of duodenal ulcer and esophagitis, under medication with H2 receptors blockers during 3 years. Plasma basal gastrin was near normal levels. A stimulation test with secretin was compatible with gastrinoma. CT scan and selective angiography didn't localize the tumor. At laparotomy we found a pyloric nodule that prove to be the gastrinoma. Eighteen months after surgery the patient is asymptomatic and with a normal endoscopic examination and secretin stimulation test.

Peptic ulcer among Japanese and Koreans in Japan has rarely been studied. In this 10-year study of hospital-based endoscopy, we focused on the epidemiology of peptic ulcer among these ethnic groups in Japan. Between 1980 and 1990, 81.2% of all patients examined via endoscopy at Saikyo Hospital in Kyoto completed a life-style questionnaire: 1,264 Japanese (70.5%), 503 Koreans (28.1%), and 25 persons of unknown ethnicity (1.4%). The curative effects of combined traditional Chinese and Western medicine in 508 patients of intractable peptic ulcer with gastroscopy confirmation were evaluated. The patients were divided into Chuanjia Weidan group (Group I, 260 patients) and cimetidine group (Group II, 248 patients). Debridement, injection and drug covering of ulcer surfaces were followed by oral Chinese medicine Chuanjia Weidan (50 g daily) or cimetidine (800 mg every night) for 4 weeks. The gastrin response to a low and a high dose of gastrin-releasing peptide infusion was studied in healthy volunteers and in patients with duodenal ulcer disease. Nine hundred and sixty-six patients' compliance with drug therapy of peptic ulcerations and reflux oesophagitis in four clinical trials were studied. The mean compliance rates varied from 90.5% (95% C.I. 89.0-92.1%) to 99.2% (95% C.I. 98.2-100.1%). Review of the literature on the role of Helicobacter pylori (HP) in the patho- and morphogenesis of chronic gastritis (CG) type B, gastric ulcer (GU) and duodenal ulcer (DU) is presented. Various hypotheses of pathogenetic effect of HP, histologic and ultrastructural characteristics of changes in the gastric and duodenal mucosa in HP infection are presented. The majority of authors consider HP as a possible pathogenic factor in CG type B, GU and DU. However, there are works in which HP is regarded as a saprophyte or a secondary infection. This indicates a necessity of further studies.
