PURPOSE: Inflammation of retinal vessels occures in variety of ophthalmic and systemic conditions. In this paper we attempt to reconstruct the events that lead to retinal vessels inflammation.
MATERIAL AND METHODS: We have studied 30 patients (14 F, 16 M)(age 27-54 years) with retinal vessels inflammation. 25 have additionally uveitis during 2 weeks to 21 years. In 7 cases etiology was established (sarcoidosis, tuberculosis, toxoplasmosis, systemic lupus erythematosus, hepatitis C). All patients were evaluated with ophthalmoscopic fundus examination. In some cases fluoresceine angiography and…

A 29-year-old man developed acute visual impairment, cough, and headache. Both eyes showed serous retinal detachment in the posterior fundus. Fluorescein angiography showed subretinal pooling of fluorescein in the late phase. A diagnosis of Vogt-Koyanagi-Harada (VKH) syndrome was made based on clinical features. Treatment with systemic corticosteroids resulted in improvement of uveitis and both eyes showed "sunset glow" fundus 11 months later. Insulin-dependent diabetes mellitus (IDDM) developed 13 months later (3 months after systemic corticosteroid therapy). Despite treatment with insulin, g…

PURPOSE: To report the response of acute Behçet retinitis to high-dose corticosteroids.
METHOD: Case report. A 58-year-old man with Behçet disease and severe bilateral glaucoma experienced a sudden decrease of visual acuity to counting fingers in his (better-seeing) left eye. Examination disclosed hypopyon uveitis and an infiltrative retinitis threatening the fovea. He received intravenous methylprednisolone hemisuccinate, 1 gram per hour on each of 3 successive days, followed by oral prednisone and cyclosporine.
RESULTS: The retinal infiltrate disappeared within 24 hours. Visual acuity impr…

Using a modified enzyme-linked immunosorbent assay that included dissociation of antigen antibody complexes with 6M urea solution, we analyzed the avidity of Toxoplasma-specific IgG in aqueous humor and serum samples from 24 patients with toxoplasmic chorioretinitis. As a control, we studied aqueous humor and serum samples from 14 cataract patients without history of uveitis and serum samples from 10 patients with recent primary systemic toxoplasmic infection without ocular lesions. IgG avidity was markedly lower in aqueous humor samples from patients with toxoplasmic chorioretinitis than in s…

Over a 15 year period 18 patients (eight men, 10 women), 16-50 years old, were diagnosed as having adult onset Still's disease. Fever and arthralgia were always present but prominent lymphadenopathy was uncommon and the serosa were rarely affected. The typical rash of this disease was observed in nine patients. Several complications, including deforming arthritis, amyloidosis, granulomatous hepatitis, uveitis, scleritis, cutaneous vasculitis, and cardiomyopathy, were observed during follow up. Two patients were affected by a nosocomial infection during immunosuppressive treatment for uncontrol…

Serological studies are one of the important diagnostic tools used in the investigation of uveitis. This study was based on anti-toxoplasma serotitres obtained from 80 normal healthy adults and 122 patients investigated for ocular toxoplasmosis over a 42 month period. Fifteen per cent of normal healthy sera were found to have low titres (1:64 to 1:256), while 3.8% had high titres (1:1024 or higher). Thirty five eyes of 29 patients had presumed ocular toxoplasmosis based on clinical features and a typical fundal appearance. 89.6% of clinically positive ocular toxoplasmosis were found to have ti…

A total of 104 eyes and 43 resected corneal discs from patients with failed intraocular lens implants (IOL) received over a period of 35 years were subjected to histopathological analysis. Eyes collected at necropsy from 18 people with clinically successful implants were also examined. Corneal decompensation leading to bullous keratopathy was the most frequent reason for failure, followed by glaucoma and intraocular inflammation. Of 18 cases in which inflammation was the principal clinical cause of failure 12 presented as infectious endophthalmitis, but minor degrees of sterile uveitis were fa…

The prevalence of sacroiliitis was assessed clinically and radiologically in 261/323 1st degree relatives of 47 B27 positive probands who had ankylosing spondylitis (AS). Of the 151 relatives who were B27 positive, 23 had bilateral sacroiliitis (asymptomatic in 4), 2 had had Reiter's syndrome (RS) and 3 acute anterior uveitis (AAU). Similar assessment of 148/182 relatives of 34 B27 positive patients without demonstrable sacroiliitis (including 15 who had had RS and 11 AAU) showed 84 to be B27 positive. Only 1 had sacroiliitis, 2 had RS, 2 AAU and 2 chronic polyarthritis. Features of this study…

Immunoglobulin A nephropathy (IgAN) is one of the most common causes of primary glomerulonephritis and follows a 'four hit' model of pathogenesis. Novel treatments show promise in treating either the early insults described by this model or the subsequent inflammatory reaction. This case report describes a man in his 50s who received bimekizumab for his HLA B27-positive spondylarthritis in the setting of IgAN. His background history includes bilateral enthesitis of the ankles, bilateral Achilles tendonitis, peripheral arteritis and anterior uveitis. 6 months after starting on bimekizumab, his …

Neuro-Behçet's Disease (NBD) is a rare neurological complication of a chronic, multisystem inflammatory condition. It typically manifests several years after the onset of initial non-neurological symptoms, such as oral and genital ulcers, uveitis, and dermatological or gastrointestinal issues. Neurological involvement occurs infrequently and can be challenging to diagnose in its early stages. This case describes a young adult who presented with progressive neurological symptoms, following a long-standing history of systemic inflammatory features. The diagnostic process included neuroimaging th…

We report a case of Blau syndrome in a 22-year-old Chinese female. The patient initially presented with joint swelling and pain at the age of one, subsequently developing a generalized rash and uveitis. Initially diagnosed with juvenile idiopathic arthritis, she was treated based on this diagnosis. However, genetic testing conducted in 2020 revealed a heterozygous mutation, C. 1538(exon 4)T > C, in the NOD2 gene (NM_022162), resulting in the substitution of methionine with threonine at position 513 of the encoded protein (p. M513T). This finding led to the re-diagnosis of Blau syndrome. The pa…

BACKGROUND: To describe a 14-year-old male with probable ocular sarcoidosis and bilateral peripapillary choroidal neovascularization (PCNV).
CASE PRESENTATION: We report a case of a 14-year-old boy with a 2-month history of floaters and gradual vision loss in both eyes. Examination revealed bilateral granulomatous uveitis with peripapillary subretinal lesions. The serum angiotensin-converting enzyme was elevated. Positron emission tomography demonstrated increased metabolic activity in the nasopharynx and small intestine, consistent with diagnosis of sarcoidosis. Despite resolution of vitreou…

This case report highlights a rare presentation of a hidradenitis suppurativa (HS) flare occurring concurrently with peripheral ulcerative keratitis (PUK) in a patient with a longstanding history of HS. HS is a chronic inflammatory skin condition known to be associated with various ocular manifestations, including inflammatory eye diseases (IEDs) such as anterior uveitis. Although the link between HS and IED is well documented, the association between HS and PUK is less frequently reported. Previous cases have demonstrated improvement in keratitis among HS patients using treatments ranging fro…
