[Advantages of biplane transesophageal echocardiography].

Biplane transesophageal echocardiography (BTEE) was intraoperatively performed on 27 patients; ten patients with coronary artery bypass graft surgery, ten with aortic valve replacement, five with mitral valve replacement, one with reconstruction of complete AV-canal, and one with surgical repair of dissecting aortic aneurysm. Compared with the transverse views of the monoplane TEE, BTEE permits the following additional images of the heart: 1) Longitudinal "two-chamber-view" for assessment of left ventricular (LV) anterior, apical, and posterior wall motion, and for assessment of mitral valve a

Mechanism of myocardial contractile depression by clinical concentrations of ethanol. A study in ferret papillary muscles.

Moderate alcohol intoxication in man, a ubiqitious social event, causes acute but reversible myocardial depression, the mechanism of which is unknown. We investigated whether this depression could be due to a direct effect of ethanol on the process of electromechanical coupling by simultaneously measuring the transmembrane action potential and contraction, or the cytosolic calcium transient (via aequorin photoluminescence) and contraction in isolated ferret right ventricular papillary muscle. Ethanol, in concentrations that are similar to plasma levels in man during intoxication (0.15 vol %),

Post-induction residual time: a novel sensing phenomenon during induced ventricular fibrillation in subcutaneous ICD recipients.

BACKGROUND: Subcutaneous implantable cardioverter defibrillators (S-ICD) are extensively used in the primary and secondary prevention of sudden cardiac death. Ventricular fibrillation (VF) induction during implantation remains standard to confirm device functionality. OBJECTIVE: To compare electrocardiographic (ECG) and technical variables between induced and spontaneous ventricular arrhythmias in S-ICD recipients. METHODS: A retrospective observational study was performed on 10 consecutive patients with S-ICD. Variables included episode duration, sensing time, charge time, and shock impedan

Premature foramen ovale restriction/closure in pregnancy: echocardiographic features and overlap with aortic coarctation.

OBJECTIVES: Evaluation of the foramen ovale (FO) is essential for understanding right heart predominance in the fetal circulation during the third trimester. Rather than considering the FO as a simple orifice, it should be assessed as a complex anatomical and functional system. Premature FO restriction or closure represents a clinically relevant but diagnostically challenging condition in fetal echocardiography. METHODS: We conducted a monocentric retrospective study including 600 uncomplicated pregnancies between 22 and 38 weeks of gestation that underwent fetal echocardiography at the Cardi

The Spectrum of Congenital Heart Disease with Transposition of the Great Arteries from the Cardiac Registry of the University of Padua.

Transposition of the great arteries (TGA) is a cardiac condition in which the arterial trunks arise from the inappropriate ventricle: the aorta from the right ventricle and the pulmonary trunk from the left ventricle [discordant ventriculo-arterial (VA) connection]. In complete TGA, the discordant VA connection is associated with situs solitus or inversus and concordant atrioventricular (AV) connection. The hemodynamic consequence of these combined connections is that systemic and pulmonary circulations function in "parallel" rather than in "series". The presence of situs solitus or inversus a

Three-Dimensional Transthoracic Echocardiography in the Comprehensive Evaluation of Right and Left Heart Chamber Remodeling Following Percutaneous Mitral Valve Repair.

BACKGROUND: Percutaneous mitral valve repair (PMVR) is an alternative treatment in patients with significant mitral regurgitation (MR) who are denied surgery. Although in surgical patients, outcomes have been related both to acute hemodynamic favorable results and to positive cardiac remodeling in the midterm, in the case of PMVR the effect on cardiac chamber remodeling has never been extensively studied. The aims of this study were (1) to evaluate the short- and mid-term remodeling induced by PMVR on cardiac chamber volume using two- and three-dimensional (3D) transthoracic echocardiographic

[Reliability and limitation of various diagnostic methods including nuclear medicine in myocardial disease (author's transl)].

Electrocardiography (ECG), echocardiography, nuclear method, cardiac catheterization, left ventriculography (LVG) and endomyocardial biopsy (biopsy) were performed in 40 cases of cardiomyopathy (CM), 9 of endocardial fibroelastosis (EFE) and 19 of specific heart muscle disease (SHMD), and the usefulness and limitation of each method was comparatively estimated. In CM, various methods including biopsy were performed. The 40 patients were classified into 3 groups, i.e., 1) hypertrophic (17), 2) dilated (20) and 3) non-hypertrophic . non-dilated (3) on the basis of left ventricular ejection fract

Increased epicardial tissue and reduced TAPSE and MAPSE scores in borderline personality disorders. Early indicators for cardiovascular risk?

INTRODUCTION: Life expectancy of patients with borderline personality disorder (BPD) is reduced compared to the general population, which has been in part attributed to a heightened risk for cardiometabolic disorders. One prior study reported increased values of epicardial adipose tissue (EAT), which has been shown to be a sensitive marker for cardiovascular disease risk. Against this background, studies assessing cardiac function in patients with BPD have been missing to date. METHODS: The present study included 28 female patients with a diagnosis of BPD and 28 age, sex, and BMI-matched cont

Outcomes after the Ross procedure with pulmonary autograft reinforcement by reimplantation.

BACKGROUND: Pulmonary autograft reinforcement to prevent dilatation and subsequent neo-aortic valve regurgitation has been reported; however, data on long-term function of the neo-aortic valve after this modified Ross procedure are lacking. Our objective here was to assess long-term outcomes of the modified Ross procedure with autograft reinforcement using the reimplantation technique. PATIENTS: The outcomes of 61 consecutive patients managed using the Dacron-conduit reinforced Ross procedure between 2009 and 2021 were reviewed. Most patients had a unicuspid or bicuspid aortic valve (n = 52;

Echocardiographic assessment of single-ventricle diastolic function and its correlation to short-term outcomes after the Fontan operation.

BACKGROUND/HYPOTHESIS/OBJECTIVES: Postoperative complications after the Fontan operation for single ventricle heart disease are common and include persistent pleural drainage and prolonged length of hospital stay (LOS). Diastolic ventricular dysfunction may increase risk for postoperative complications by raising central venous pressures. We sought to determine the relationship between preoperative echocardiographic measurements of diastolic function, including myocardial deformation imaging, on (a) preoperative invasive catheterization measurements and (b) postoperative outcomes after the Fon

Changes in Loop Diuretic Dose and Outcome After Cardiac Resynchronization Therapy in Patients With Heart Failure and Reduced Left Ventricular Ejection Fractions.

Cardiac resynchronization therapy (CRT) improves cardiac hemodynamics. Therefore, the maintenance dose of loop diuretic therapy might be reduced. Consecutive patients who underwent CRT (n = 648) were retrospectively evaluated. Loop diuretic dose was recorded at baseline before implantation and 6 months later with patients classified into 4 groups: (1) no loop diuretic, (2) down-titration, (3) unchanged dose, and (4) up-titration. Afterward total loop diuretic exposure was calculated. Renal function trajectories were evaluated as the difference between implantation and censoring serum creatinin

Current concept of transcatheter closure of atrial septal defect in adults.

After the introduction of catheter intervention for atrial septal defect (ASD) in the pediatric population, therapeutic advantages of this less invasive procedure were focused on adult through geriatric populations. The most valuable clinical benefits of this procedure are the significant improvement of symptoms and daily activities, which result from the closure of left to right shunt without thoracotomy and cardiopulmonary bypass surgery. These benefits contribute to increase the number of adult patients of this condition who have hesitated over surgical closure. In terms of technical point

Does the degree of cyanosis affect myocardial adenosine triphosphate levels and function in children undergoing surgical procedures for congenital heart disease?

OBJECTIVE: The outcome of children with cyanosis after cardiac surgical procedures is inferior to that of children who are acyanotic. Animal studies indicated detrimental effects of chronic hypoxia on myocardial metabolism and function. We studied whether the presence or the degree of cyanosis adversely affected myocardial adenosine triphosphate, ventricular function, and clinical outcome in children. METHODS: Forty-eight children who underwent repair of tetralogy of Fallot were divided according to their preoperative saturation: group I, 90% to 100% (n = 14 patients); group II, 80% to 89% (n

The value of transthoracic and transesophageal echocardiography for the diagnosis of the native aortic infective endocarditis valve complications: a case report and literature review.

Despite its current limitations transthoracic echocardiography is still widely used for the anatomical and functional evaluation of patients with infective endocarditis. However, all these limitations can be overcome by using transesophageal echocardiography. We present the case of a 42-year-old male patient, diagnosed with aortic valve infective endocarditis, whose transthoracic echocardiography showed only a cusp vegetation and aortic regurgitation, but raised the suspicion of periannular complications. The transesophageal echocardiography revealed a circular aortic root abscess and a ventri

Surgical management of chronic pulmonary regurgitation after relief of right ventricular outflow tract obstruction.

Relief of right ventricular (RV) outflow tract obstruction in tetralogy of Fallot or similar physiology often results in pulmonary regurgitation (PR). The resultant chronic volume overload can lead to RV dilatation, biventricular dysfunction, heart failure symptoms, arrhythmias and sudden death. Although pulmonary valve replacement (PVR) can lead to improvement in the functional class and a substantial decrease or normalization of RV volumes, the optimal timing of PVR is not well defined. Benefits of PVR have to be weighed against the risks of this procedure including subsequent reoperation. T

The hypoxaemia in chronic obstructive pulmonary disease (COPD) and its management.

Chronic hypoxaemia < or = 55 mmHg, induces various clinical and physiological consequences in patients with stable COPD: dyspnoea, decrease in exercise performances, erythrocytosis impairment of neuropsychological functions, pulmonary hypertension, right ventricular heart failure, all of these troubles impairing quality of life and vital prognosis. To restore a PaO2 > 60 mmHg (or SaO2 > 90%), long term oxygen therapy (LTOT) is the best method in the management of COPD with severe chronic respiratory failure. For patients with PaO2 55-70 mmHg.
