In vivo near-infrared autofluorescence imaging of pigmented skin lesions: methods, technical improvements and preliminary clinical results.

BACKGROUND/PURPOSES: Fluorescence emission from in vivo cutaneous melanin was recently detected under near-infrared (NIR) excitation by our group. We then built a prototype NIR autofluorescence imaging system to observe and characterize the melanin distribution in human skin. In this article, we reported a new setup of NIR fluorescence imaging system and calibration methods to optimize the system for better clinical feasibility and clearer image. METHODS: The imaging system was designed to perform both fluorescence and reflectance imaging with a 785-nm fiber-coupled laser source. The illumina

Genome-wide association study of skin complex diseases.

Complex diseases are caused by both genetic and environmental factors. Over decades, scientists endeavored to uncover the genetic myth of complex diseases by linkage and association studies. Since 2005, the genome-wide association study (GWAS) has been proved to be the most powerful and efficient study design thus far in identifying genetic variants that are associated with complex diseases. More than 230 complex diseases and traits have been investigated by this approach. In dermatology, 10 skin complex diseases have been investigated, a wealth of common susceptibility variants conferring ris

Different clinical and laboratory evolutions in ataxia-telangiectasia syndrome: report of four cases.

We report four patients with ataxia-telangiectasia syndrome that presented varied neurologic evolution. Three patients initially presented neurologic alterations of slow progression, evolving to late immunocompromised conditions. The fourth patient presented, from symptom onset, immune and neurologic debilitation, that were both severe and of fast progression. The chronological sequence of the most commonly observed immunocompromised conditions were in our patients, in ascending order, IgA deficiency, IgG2 deficiency and the neutrophil phagocytosis stage and common variable immunodeficiency. Catecholamine phenotyping: clues to the diagnosis, treatment, and pathophysiology of neurogenetic disorders.

One purpose of clinical neurochemistry has been to indicate "activities" of catecholamine systems, by assaying levels of the effector compounds or their metabolites in body fluids such as plasma, cerebrospinal fluid, urine, or microdialysate. This review discusses a new purpose: relating specific catecholaminergic phenotypes to neurogenetic disorders. Distinctive catecholamine patterns in several neurogenetic conditions reflect enzyme deficiencies as direct or indirect effects of gene mutations. These neurochemical patterns can provide potentially important clues to the diagnosis, treatment, a

AIDS and HIV infections: uncommon presentations.

Infection with HIV or AIDS has a great impact on skin diseases, not only by affecting the immune system and thereby host defense against bacterial, viral, or mycotic infection, but also by changing tumor immune response and autoimmune reactivity. In the present review, emphasis will be made on infectious diseases, including sexually transmitted disease, inflammatory skin disease, and neoplasias. Knowledge of changing disease pattern with HIV/AIDS may help the clinical dermatologist and venerologist to identify dermatoses and act in the most appropriate manner to support the patient.

[Tacrolimus in diseases other than atopic dermatitis].

Topical tacrolimus is an immunosuppressant that acts through the inhibition of calcineurin and thus of the T cells. This causes a decrease in the production of interleukins, the granulocyte colony stimulating factor, alpha interferon and tumor necrosis factor. Although the use of topical tacrolimus is only indicated for the treatment of moderate or severe atopic dermatitis, its immunosuppressant effect and fewer side effects regarding topical corticosteroids have lead to the increase of its use in other types of inflammatory skin diseases. The purpose of this article is to review the use of ta

LLM-Assisted Reanalysis of Unsolved Rare Disease Genomes Increases Diagnostic Yield.

BACKGROUND: Rare and undiagnosed genetic disorders affect millions of patients globally, and many patients endure years of inconclusive test … , including a candidate association between the sphingosine-1-phosphate receptor 1 gene (S1PR1) and vitiligo. CONCLUSIONS: In retrospective reanalysis, an explanation-first LLM applied to routine HPO terms and variant tables produced clinically relevant gains in diagnostic yield, surfaced overlooked pathogenic findings, and generated biologically grounded …

Frequency of autoimmune diseases in children and adolescents with type 1 diabetes mellitus.

BACKGROUND: Children and adolescents with type 1 diabetes mellitus (T1D) are at increased risk of developing additional autoimmune diseases. … t comorbidities were autoimmune thyroiditis (AIT, 12.7%) and celiac disease (CD, 5.9%), followed by vitiligo (1.3%), autoimmune gastritis/pernicious anemia (0.15%), and autoimmune hepatitis (0.15%). Female sex was independently associated with the presence of additional autoimmune diseases (OR = 1.61, 95% CI 1.07-2.41, p = 0.022). Most autoimmune diseases …

Conclusions LSEA may be confused with other various dermatoses, such as morphea, vitiligo, and lichen planus.
