The instep flap for anterior ankle coverage with bone and hardware exposure.

PURPOSE: The instep medial plantar flap is a well-known flap based on the medial plantar artery of the foot and usually used for coverage of soft tissue defects of the heel area. It has seldom been reported for coverage of anterior ankle area with exposure of the bone and metallic hardware after open reduction and internal fixation of distal tibial fractures. The primary purpose of this study is to evaluate the feasibility and viability of this flap as well as its reliability saving the internal fixation devices and efficiency protecting bone healing; the secondary purpose is to assess the con

Clinical outcomes of isolated acute instability of the syndesmosis treated with arthroscopy and percutaneous suture-button fixation.

INTRODUCTION: To present the short- to midterm clinical outcomes of a series of patients with isolated acute syndesmosis instability treated with arthroscopy and percutaneous suture-button fixation. MATERIALS AND METHODS: A review of 17 patients treated through the arthroscopic approach. The mean age of the patients was 27.8 years and the mean follow-up was 31.5 months. Clinical and functional evaluations were based on the VAS, AOFAS ankle-hindfoot score and time of return to preinjury level of activities. Imaging analysis was performed with stress radiographs, MRI and CT scan with stress. R

Osteochondral Lesions of the Talus: An Individualized Treatment Paradigm from the Amsterdam Perspective.

Osteochondral lesions of the talus (OLTs) are characterized by damage to the articular cartilage of the talus and its underlying subchondral bone. Up to 75% of OLTs are caused by trauma, such as an ankle sprain or fracture. Physical examination and imaging are crucial for diagnosis and characterization of an OLT. No superior treatment for OLTs exists. It is paramount that an evidence-based personalized treatment approach is applied to patients with OLTs because lesion and patient characteristics guide treatment. This current concepts review covers clinical and preclinical evidence on OLT etiol

Distinguishing between homicide and suicide knots and ligatures: A comparative analysis of case and survey data.

Distinguishing between suicide and homicide can be challenging owing to ambiguous and confusing case details. In particular, there is a paucity of useful information comparing homicide and suicide knots and ligatures in the literature. Multiple knot and ligature characteristics have not been recognized previously because complete and accurate information has been lacking. Ninety external tying cases (mainly homicide) and 56 cases involving self-tying (mainly suicide) were reviewed to compare multiple knot and ligature characteristics. Additionally, 189 survey volunteers performed four standard

Center of pressure during stance and gait in subjects with or without persistent complaints after a lateral ankle sprain.

STUDY AIM: To investigate differences in the center of pressure (COP) during gait and single leg stance between subjects with persistent complaints (PC) and without persistent complaints (NPC) after a lateral ankle sprain. METHODS: 44 patients who consulted the general practitioner, 6-12 months prior to inclusion, with a lateral ankle sprain were included for the current study purpose. Using a 7-point Likert scale patients were divided into the PC or NPC group. All subjects filled out an online questionnaire, walked along a walkway and performed a single leg stance, both on the RSscan. Primar

Reconstruction of a large osteochondral lesion of the distal tibia with an iliac crest graft and autologous matrix-induced chondrogenesis (AMIC): a case report.

Isolated osteochondral lesions (OCL) of the distal tibia are rare and lack clear treatment guidelines. With the case we present here, we suggest a novel surgical approach and report the successful use of autologous matrix-induced chondrogenesis-aided reconstruction for OCL of the distal tibia. A 29-year-old male patient complained about persisting pain of the left ankle joint and a restricted activity level 12 months after an ankle sprain. Imaging revealed edema of the subchondral bone and thinning of the cartilage above the osseous defect at the lateral distal tibia. The OCL was debrided foll

In-shoe plantar pressures within ankle-foot orthoses: implications for the management of achilles tendon ruptures.

BACKGROUND: Advances in the management of Achilles tendon rupture have led to the development of immediate weightbearing protocols. These vary regarding which ankle-foot orthoses (AFOs) are used and the number of inserted heel wedges used within them. PURPOSE: This study was conducted to evaluate plantar pressure measurements and temporal gait parameters within different AFOs, using different numbers of heel wedges. STUDY DESIGN: Controlled laboratory study. METHODS: Fifteen healthy participants were evaluated using 3 different AFOs, with 4 different levels of inserted heel wedges. Therefor

The role of arthroscopy in the management of fractures about the ankle.

Anatomic restoration of the joint is the goal of management in fractures about the ankle. Open reduction and internal fixation (ORIF) is the standard of care for unstable ankle fractures; however, arthroscopic management has been proposed. The use of arthroscopic reduction and internal fixation (ARIF) is surgeon-dependent. Reported indications for ARIF include transchondral talar dome fracture, talar fracture, low-grade fracture of the distal tibia, syndesmotic disruption, malleolar fracture, and chronic pain following definitive management of fracture about the ankle. Among the potential bene

Modelling bounded health scores with censored skew-normal distributions.

Health care interventions that use quality of life or health scores often provide data which are skewed and bounded. The scores are typically formed by adding up numerical responses to a number of questions. Different questions might have different weights, but the scores will be bounded, and are often scaled to the range 0-100. If improvement in health over time is measured, scores will tend to cluster near the 'Работает' or 'good' boundary as time progresses, leading to a skew distribution. Further, some patients will drop-out as time progresses, hence the scores reflect a selected population

Incidence and pattern types of fractures of the lateral process of the talus.

Fractures of the lateral process of the talus have historically been considered unusual. This study sought to more precisely define the typical lateral talar process fracture pattern and its incidence in general trauma patients. Such information may have implications for the clinical treatment of these seemingly increasingly recognized injuries. A retrospective review was performed at our level I trauma center of the x-rays and available computed tomography scans of all talus fractures treated between 2000 and 2005 to identify the respective incidence and variation in fracture configuration of

[Complete anterolateral dislocation of the talus].

A 22-year-old woman suffered an anterolateral dislocation of the talus in a horse-riding accident. Immediately after admission, closed reduction was performed, and then a low knee cast was used for 12 weeks of immobilisation with partial weight bearing (10 kg). Nine months later, the patient had achieved nearly free range of motion of the ankle without pain (dorsal-plantar 10-0-30 degrees ). There were no radiological signs of osteoarthritis or talus bone necrosis. Immediate closed reduction led to an excellent outcome.

Selectively attenuating soft tissues close to sites of inflammation in the peritalar region of patients with rheumatoid arthritis leads to development of pes planovalgus.

OBJECTIVE: To compare the 3-dimensional (3D) orientation of the tibiotalar, tibiocalcaneal, and intertarsal joints in cadaveric specimens following structural weakening to predetermined ligaments in the peritalar region and medial ankle tendons under axial loads and simulated calcaneal valgus deformity. METHODS: Eight fresh-frozen, unembalmed human lower leg and foot specimens were placed in a materials testing machine. The mid-stance period of gait was simulated and the 3D orientation of the tibiotalar, tibiocalcaneal, and intertarsal joints was measured using an electromagnetic motion analy

Fracture of the posterior process of talus.

Fracture of posterior process of talus is quite rare, but is associated with significant morbidity as it involves two articular surfaces. We report two such cases in which open reduction and internal fixation were done with good functional results. One of the cases had a concomitant medial malleolus fracture which to the best of our knowledge has not been reported so far.
