BACKGROUND: Management of unstable pertrochanteric fractures remains a challenge with various implant choices. Intramedullary devices are usually preferred for the management of the unstable fractures. When nailing is unsuitable for the configuration of the fracture extra medullary procedures are preferred. PFLCP is a contact limited implant that allows multiple angularly stable fixations with preserving more bone stock after implantation as an extramedullary implant. There are only a few reports in the literature about the osteosynthesis of unstable trochanteric fractures with proximal femora

OBJECTIVE: The aim of this surgical technique is fracture healing with anatomical alignment and less implant irritation due to smaller, low-profile plates. Equal to superior stability is provided compared to single superior- or anterior-based plates.
INDICATIONS: The same general indications for surgical stabilization of clavicle fractures apply for low-profile double plating and include fracture displacement of one or more shaft width, shortening of more than 1 cm in length, and patients with high physical activity levels. Double plating is especially suitable for fractures in the midportion

PURPOSE: Conventional mandibular reconstruction frequently results in stress shielding, compromised osteotomy site healing, or bone resorption. This study introduces an Embedded Polygonal Bone Reconstruction Structure (EPBRS) that eliminates titanium plate dependency, enhanced through fillet-based topological optimization.
MATERIALS AND METHODS: A computational model of the Embedded Polygonal Bone Reconstruction Structure (EPBRS) was developed from CT scans. Four variants with fillet radii (0.2, 0.4, 0.6, and 0.8 mm) were designed. Physiological masticatory loading was simulated under identic

INTRODUCTION: Concomitant acromioclavicular joint (ACJ) disruptions with coracoid base fractures are rare high energy injuries. The management of these injuries can be challenging. The aim of this study is to assess the functional and radiographic outcomes of a retrospective case series of patients presenting with concomitant ACJ and coracoid base injuries managed with a clavicle hook plate with subsequent hardware removal at a later stage.
METHODS: Six patients were identified for inclusion in the study. Radiographic and clinical data were available which allowed for collection of demographi

BACKGROUND: Occipital plate fixation has been shown to improve outcomes in cervical spine fusion. There is a paucity of literature describing occipital plate fixation, especially in the pediatric population. The authors reviewed a case series of 34 patients at a pediatric hospital who underwent cervical spine fusion with occipital plate fixation between 2003 and 2016. This study describes how occipital plates aid the cervical spine union in a case series of diverse, complex pediatric patients.
METHODS: Our orthopaedic database at our institution was queried for patients undergoing an instrume

Posttraumatic bone defects (BDs) remain a difficult complication for orthopaedic surgeons. Surgical goals in these reconstructive cases are to create stable limb fixation, maintain limb length, and provide adequate soft-tissue coverage. Historically, surgical approaches in these cases have required the use of an external fixator, which is associated with several postoperative complications. A plate-assisted bone segment transport (PABST) technique using a magnetic limb lengthening system eliminates the need for an external fixator and is effective for these reconstructive cases. A 51-year-old

PURPOSE: Appropriate fixation method for the posterior malleolar fractures (PMF) according to the fracture size is still not clear. Aim of this study was to evaluate the outcomes of the different fixation methods used for fixation of PMF by finite element analysis (FEA) and to compare the effect of fixation constructs on the size of the fracture computationally.
MATERIALS AND METHODS: Three dimensional model of the tibia was reconstructed from computed tomography (CT) images. PMF of 30%, 40% and 50% fragment sizes were simulated through computational processing. Two antero-posterior (AP) lag

PURPOSE: To evaluate the treatment outcome for enchondromas of the proximal phalanx with pathological fracture.
METHODS: Between June 2008 and October 2012, we treated 9 patients with solitary proximal phalanx enchondromas and pathologic fractures by curetting the tumor, filling the void with a block of autogenous bone chip, and applying a low-profile miniplate. Postoperative follow-up included clinical assessment, pain evaluation, and radiographs.
RESULTS: Follow-up time ranged from 13 to 42 months (mean, 30 mo). No major complications such as notable malalignment, nonunion, infection, or t

OBJECTIVES: To evaluate the effect of calcar screw use in proximal humeral fractures with unstable medial support treated with locked plates.
METHODS: Standard osteotomies were performed in 36 cadaveric humeri to create a surgical neck fracture proximal humeral model. For static testing, 12 pairs of humeri were divided into 4 groups: normal alignment and varus deformity groups with and without 5-mm medial deficiencies. Calcar screw function was measured in each group by axial, shear, and torsion stiffness tests. Another 6 pairs of humeri with 5-mm medial deficiencies were subjected to cyclic

A prospective cohort study of the Ludloff osteotomy, stabilized with either lag screws or a locking plate, was undertaken from May 2001 to November 2010, involving patients treated for hallux valgus with a first intermetatarsal angle greater than 15°. All patients were assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) and Roles and Maudsley scores before and after surgery. The cohort included 119 procedures in 112 patients, 91 (76.47%) of which were fixated with lag screws and 28 (23.53%) with locking plates. The participants were categorized as active (n = 69 procedures)

Sagittal rotational malunion after distal radius fractures was identified in eight patients by the presence of a "hinge" point on the volar cortex on the lateral radiograph, and the ulnar head being shorter than the anterior lip of the radius on the posterior-anterior radiograph. The surgical correction consisted of preplating the distal fragment with a volar locking plate before an osteotomy through the "hinge" point, and correcting the dorsal tilt of the distal fragment. Any dorsal defect was filled with cancellous bone graft from the olecranon. Pain, range of motion and grip all improved. D

The diagnosis and management of mandible fractures in the pediatric patient population can pose multiple challenges to the oral and maxillofacial surgeon. Resorbable plates and screws for fixation in this population are both well tolerated and effective. They enable realignment and stable positioning of rapidly healing fracture segments, while obviating any potential impediments to long-term metal retention.

Open reduction and internal fixation (ORIF) with refined titanium plates and screws have revolutionized the surgical treatment of complex facial fractures, but its outcome remains relatively unknown. The purpose of this study is to review the results of modern surgical treatment of complex facial fractures involving at least two of the three zones of the face. From 1996 to 2002, 54 consecutive patients (41 males, 13 females; age 5-74 years) with complex facial fractures were treated by the plastic surgery service in our institution. All patients had their fractures repaired within 10 days afte

Between 1984 and 1986, 10 patients who sustained a femur fracture about a hip prosthesis were treated with open reduction and internal fixation using compression plating. Nine of the 10 fractures healed in an average time of 5 months. Other than one nonunion, no significant complications were noted. Follow-up study, ranging from 13 to 44 months (average, 26 months), was obtained on all 10 patients. One had a loose femoral component, which was attributed to inadequate stem size. None of the other patients had either clinical or radiographic evidence of loosening. Based on Harris hip scores, the

The contour of the hypoplastic or displaced zygoma can be corrected by two basically different techniques. For reconstruction, onlays with alloplastic or autologous material are advocated. In post-traumatic cases an osteotomy may be considered. The two techniques are compared. It appears that the longterm results achieved in 7 cases with the onlay technique are aesthetically inferior to those of the 20 cases treated by osteotomy. Since the osteotomy also has technical advantages, it should also be preferred in cases of marked hypoplasia, if need be in combination with onlays.

Fetal bone development occurs through the conversion of avascular cartilage to vascularized bone at the growth plate. This requires coordinated mobilization of osteoblast precursors with blood vessels. In adult bone, vessel-adjacent osteoblast precursors are maintained by mechanical stimuli; however, the mechanisms by which these cells mobilize and respond to mechanical cues during embryonic development are unknown. Here, we show that the mechanoresponsive transcriptional regulators Yes-associated protein (YAP) and transcriptional co-activator with PDZ-binding motif (TAZ) spatially couple oste
