The Marfan syndrome--analysis of growth and cardiovascular manifestation.

Forty-eight children and adolescents (mean age 10.5 years, range 1.25-18 years) with clinical evidence of Marfan syndrome were studied. Height and weight percentiles were established. Cardiac dimensions and morphology were studied by M-mode and 2D-echocardiography. At diagnosis left atrial and left ventricular end-diastolic diameter and left ventricular posterior wall thickness were within normal limits except in a few adolescent patients. Interventricular septum was thickened in about 20% and aortic diameter increased in 56% of the patients. An additional 13% of patients developed aortic dila

Giant left atrium and mitral valve replacement: risk factor analysis.

Giant left atrium (GLA) associated with mitral valve disease (MVD) has been reported as a significant risk factor in mitral valve surgery with mortality ranging from 8%-32%. Plication of the left atrium has been suggested to reduce postoperative left ventricular failure, respiratory failure and mortality. The 203 consecutive patients with MVD operated upon between 1980 and 1986 were reviewed and divided in two groups: group A without GLA (165 patients) and group B with GLA (38 patients = 19%). The pertinent preoperative and intraoperative notes and the early and late postoperative course were

Spectrum of renovascular hypertension in the young in north India: a hospital based study on occurrence and clinical features.

Occurrence of renovascular hypertension and its etiology is described in this work as seen in north India. Out of 2395 young hypertensives admitted for evaluation during the years 1978-1983, 91 patients (3.8%) were found to be having renovascular hypertension. In all patients suspected diagnosis of RVH was confirmed by the angiography of aorta and main vessels. Idiopathic aortoarteritis, diagnosed in 54 patients (59.4%) was the commonest cause of renovascular hypertension. In this group renal arterial involvement was found to be bilateral in 26 patients. Twenty-eight patients had isolated rena

Post-pacing T loop abnormalities.

Vectorcardiograms of paced and unpaced beats were recorded from 18 patients with implanted pacemakers to investigate the characteristics of post-pacing T inversions. The intraventricular conduction during sinus rhythm of unpaced beats was normal in 13 of 18 patients and abnormal in the other 5 (RBBB in 3, LBBB in 2). The directions of the maximum QRS vector in paced beats (max QRSp.V) and maximum T vector in unpaced beats (max T unp.V) were studied. In 8 of 13 patients with normal intraventricular conduction (group A) and in 3 patients with right bundle branch block (RBBB), the differences bet

Cardiorespiratory response to exercise in Ebstein's anomaly.

Fourteen patients with unrepaired Ebstein's anomaly underwent maximal exercise testing between October 1982 and April 1984. Compared with control subjects, these patients had significantly lower values for total work performed, exercise time, maximal oxygen uptake, blood oxygen saturation at rest and during exercise, and heart rate and systolic blood pressure during maximal exercise. They had a significant increase in heart rate at rest. The ventilatory equivalent for oxygen was increased significantly both at rest and during exercise. Thus, patients with Ebstein's anomaly have decreased exerc

Right ventricular subendocardial infarction in a patient with pulmonary hypertension, right ventricular hypertrophy, and normal coronary arteries.

Right ventricular infarction is usually associated with coronary artery disease and concomitant left ventricular infarction. Isolated right ventricular subendocardial necrosis was discovered at autopsy in a 52-year-old woman with pulmonary hypertension, right ventricular hypertrophy, and normal coronary arteries, who died with septicemia 41 days after mitral valve replacement. This represents the first well-documented report of isolated right ventricular subendocardial infarction associated with normal coronary arteries.

Caplike double-horned double-outlet right ventricle. Report of two cases.

Two patients with a rare form of double-chambered, double-outlet right ventricle (DORV) are described. Both patients had associated pulmonary stenosis, ventricular septal defect, and a large tricuspid valve whose chordal attachments crossed the right ventricular outflow tract. The angiocardiographic, echocardiographic, and operative findings were distinctive. The right ventricle was positioned as a "cap" sitting atop the ventricular septal defect at the cephalad end of the left ventricle. The right ventricle, with its two "horns," was likened to a rider straddling a saddle. Tricuspid chordal a

[Carcinoid heart disease secondary to a tumor of the small intestine. Apropos of a case with associated tricuspid and pulmonary insufficiency].

The authors report a case of carcinoid heart disease secondary to a tumour of the small bowel with liver metastases. There were severe lesions of the endocardium on the right side of the heart, with gross pulmonary and tricuspid incompetence. The left side of the heart did not escape but the fibrous plaques were limited to the papillary muscle of the mitral valve, and had no effect upon the haemodynamics. The pathogenesis of the strange cardiac lesion may be partly explained in terms of the toxicity to the endothelium of bradykinin. In spite of the serious nature of the valvular damage is it r

Myocardial perfusion imaging and gated cardiac blood pool scanning: clinical application.

Myocardial perfusion imaging with thallium-201 and gated cardiac blood pool scanning are finding increasing use in clinical cardiology. These noninvasive techniques have been found useful in detecting myocardial infarction independent of the electrocardiogram and determining the site and extent of the infarct as well as its effect on left ventricular function. These studies provide important prognostic data and are proving to be of value in evaluating patients with cardiogenic shock. Neither the thallium-201 myocardial perfusion image nor the gated cardiac blood pool scan can distinguish betwe

Surgery of Marfan's syndrome and related conditions of the aortic root (annulo-aortic ectasia).

The surgical treatment of 14 patients with annulo-aortic ectasia and aortic valve regurgitation secondary to cystic medial necrosis is presented. Resection of the aneurysm and replacement with a tubular graft, and aortic valve replacement with a rigid prosthesis, are advocated for definitive treatment. Eighty-six per cent of the patients survive and are doing well six months to four and a half years after surgery.

Replacement of the mitral valve, aortic valve, and ascending aorta with coronary transplantation in a child with the Marfan syndrome.

A girl with the Marfan syndrome is described who developed mitral regurgitation, an aneurysm of the ascending aorta, and severe aortic regurgitation. Complete surgical correction required mitral valve, aortic valve, and ascending aorta replacement with transplantation of the origin of the coronary arteries. The cardiovascular complications of the Marfan syndrome are discussed and the previously described surgical techniques for dealing with aortic or mitral involvement are briefly reviewed. A simpler and safer method for replacement of the complete ascending aorta with coronary artery transpla
