A colonoscopy and colonoscopic polypectomy service was established at Wellington Hospital, New Zealand in April 1975. Between April 1975 and March 1990 1157 polyps were either removed or biopsied and examined histologically. Twenty-five polyps were lost. Patient-age seemed to increase through the spectrum hyperplastic, tubular, tubulovillous, villous, and polypoid carcinoma. Sessile < 6 mm in diameter hyperplastic polyps were more numerous than small adenomatous polyps. We observed five cases of unusual extraperitoneal presentation of diverticulitis during an 11 year period. During that time, a total of 263 patients were operated for diverticulitis. Patients 1-4 presented with an inflammatory spread of diverticulitis through the abdominal wall; in patient 5 multiple abscesses were present in the left thigh. Inadequate resection and previous radiotherapy in patient 4 might have contributed to the complicated course. The use of intestinal segments for bladder replacement or augmentation is an alternative in the management of urologic patients. An operative technique for performing a permanent end sigmoid colostomy without recourse to laparotomy is presented. The results from 16 patients have shown a very low morbidity. The technique was unsuccessful in three patients, each needing a formal laparotomy.

The Altemeier procedure remains one of the better alternatives in elderly patients with rectal prolapse too fragile to undergo an abdominal operation. The circular stapler was studied first in a dog model then in humans to ascertain whether it added anything to the previously well-described technique. A case of inflammatory polyposis of the ileum after ileosigmoid anastomosis is reported. Two features are noteworthy. First, the polyps were localized in a blind ileal loop. A retrospective study of three hundred patients with colorectal cancer seen during a five-year period at Wellington Hospital is presented. The age incidence, anatomical distribution, tumour stage and incidence of intestinal obstruction differ from those reported in most other series. There is no correlation between delay in presentation and Dukes stage. It is suggested that colorectal cancer is a more aggressive disease in New Zealand than it is elsewhere in the world. The dangers of translating disease satistics from one country to another are emphasized.

Sixteen bladder exstrophy patients were treated by diverting the urine into a non-refluxing colon conduit. There were 4 new, previously unoperated cases and 12 had been diverted earlier by ileal conduit. Subsequently, 11 conduits were detached from the abdominal wall and joined end-to-side to the rectosigmoid colon. This method has proved satisfactory to date but long-term assessment will be needed.
