Factors promoting and impeding exercise behaviour in patients with colorectal cancer and comorbid diabetes during chemotherapy in Zhejiang, China: a qualitative study.

OBJECTIVE: To explore factors influencing exercise behaviour during chemotherapy in patients with colorectal cancer and comorbid diabetes, providing reference for developing management plans and clinical practice. DESIGN: This was a qualitative study using a descriptive phenomenological approach. Semi-structured interviews were conducted with a guide informed by existing literature on exercise behaviour in chronic disease populations; the Health Belief Model (HBM) served as a sensitising framework to ensure broad coverage of relevant experiential domains. Data were analysed using Colaizzi's s

Integrative computational analysis of public fecal lipidomics and transcriptomics datasets suggests a candidate association between the COX-2 pathway and CE(20:4) in colorectal adenoma-carcinoma progression.

BACKGROUND: The fecal lipidomic changes underlying the colorectal adenoma-carcinoma sequence remain incompletely characterized, particularly regarding the continuous metabolic trajectory and heterogeneity of precancerous adenomas. We aimed to use publicly available multi-omics data and an integrative computational framework to identify candidate lipidomic features associated with a CRC-like metabolic state in adenomas. METHODS: We developed an extreme-phenotype machine learning strategy using fecal lipidomics from healthy controls and colorectal cancer (CRC) patients (Study ID: ST003798) to b

Could artificial intelligence-powered colonoscopies change the future of colorectal cancer screening?

Colorectal cancer is a major cause of cancer-related mortality worldwide, underscoring the importance of early and effective colorectal cancer screening to improve survival rates. Traditional colorectal cancer screening methods include non-invasive tests, such as the fecal immunochemical test (FIT), as well as diagnostic procedures like colonoscopy. Colonoscopy remains the gold standard for detecting and treating precancerous polyps and early-stage cancer, regardless of whether it is used as the first screening test or the second test following a positive FIT. However, its effectiveness can be

The impact of advanced medical therapies on time to resection and colorectal cancer outcomes in ulcerative colitis patients undergoing colectomy.

BACKGROUND: We aimed to evaluate the impact of advanced medical therapies (biologicals and small molecules) on time to colectomy and oncological outcomes in ulcerative colitis (UC). METHODS: This cohort study included UC patients who underwent colectomy between 2003 and 2022 at 2 referral centers in Belgium and the Netherlands. Exposure was the use of advanced medical therapies. Primary outcomes were time to colectomy and colorectal cancer (CRC) rate, compared between 4 periods: P1 (2003-2007), P2 (2008-2012), P3 (2013-2017), and P4 (2018-2022). Secondary outcomes were oncological outcomes, i

Disaggregated Colorectal Cancer Mortality among Asian American Subgroups between 2005 and 2020.

BACKGROUND: Colorectal cancer is the second leading cause of cancer death in Asian Americans. Asian Americans are a diverse, heterogeneous population composed of groups with differing cancer risk factors. Few prior studies have analyzed colorectal cancer mortality by disaggregated Asian racial subgroup. METHODS: Using 2005 to 2020 US national mortality records linked to American Community Survey one-year population estimates, we report age-standardized mortality rates per 100,000 person-years, standardized mortality ratios (SMR), and average annual percent change trends for the six largest As

Targeting DAP5 Disrupts Alternate Mode of Translational Initiation in Tregs and Potentiates Antitumor Immunity.

Regulatory T cells (Tregs) can thrive in the harsh tumor microenvironment (TME) to dampen antitumor immunity. Chronic stresses within TME compromise canonical cap-dependent translation (CDT), which compromises effector T cell function but not Treg persistence. Death-associated protein 5 (DAP5/eIF4G2), a non-classical translational scaffold, has been reported to support human Treg differentiation in vitro, but its functions in thymic Treg development, peripheral Treg homeostasis, and tumor-infiltrating Treg (ti-Treg) fitness remain unclear. Here, it is shown that DAP5 expression positively corr

Early-onset colorectal cancer as an emerging disease of metabolic dysregulation.

Colorectal cancer (CRC) is one of the most common malignancies and the second leading cause of cancer-related death worldwide. Early-onset CRC (EOCRC), diagnosed in adults under the age of 50 years, has emerged as a pressing public health concern owing to its alarming rise in incidence since the 1990s. This trend, observed in the USA and at least eight other high-income countries, starkly contrasts with the declining incidence rates of late-onset CRC (age 50 years and above), largely attributed to early disease detection and lifestyle changes. Concurrent with the rising number of cases of EOCR

Real-World Adherence to Repeat Colorectal Cancer Screening With the Multi-Target Stool DNA Test in a Large, Insured, and Average-Risk Population.

INTRODUCTION: Guidelines recommend starting average-risk colorectal cancer (CRC) screening at age 45 years, and thereafter repeating screening tests at regular intervals. However, US screening rates are currently suboptimal. This study evaluated adherence to repeat CRC screening with the non-invasive multi-target stool DNA (mt-sDNA) test among US adults. METHODS: This was a retrospective claims-based analysis of individuals aged 45-75 years at average risk for CRC who had previously completed the mt-sDNA test. Individuals received the repeat mt-sDNA kit based on a point-of-care order. The pri

Denoising of fluorescence lifetime imaging data via principal component analysis.

Fluorescence Lifetime Imaging Microscopy (FLIM) quantifies autofluorescence lifetime to assess cellular metabolism, therapeutic efficacy, and disease progression. These dynamic and heterogeneous processes complicate signal analysis. Fit-free analysis methods such as phasor analysis are increasingly used due to limitations of fit-based approaches. However, incorporating photon-counting shot noise often leads to moderate-to-high uncertainty in detecting subtle changes. Common noise-reduction strategies can introduce errors and cause data loss. We developed noise-corrected principal component ana

Appendectomy Mitigates Ulcerative Colitis Activity and Delays Colorectal Cancer Onset: A Retrospective Cohort Study.

BACKGROUND/AIM: Ulcerative colitis (UC) is an idiopathic inflammatory disease with rising global incidence, influenced by genetic, environmental, and immunological factors whose interactions remain unclear. Although the appendix appears to influence UC onset and disease activity, few studies have investigated how the timing of appendectomy affects disease activity and colorectal cancer risk. This study aimed to clarify the impact of appendectomy on disease activity and colorectal cancer risk in patients with UC. PATIENTS AND METHODS: We retrospectively analyzed 368 patients with UC treated at

Roseburia intestinalis Modulates Immune Responses by Inducing M1 Macrophage Polarization.

In recent years, the gut microbiome has been recognized as one influential factor in cancer development. Particularly in colorectal cancer (CRC), several studies observed a major imbalance of the intestinal microbiota, marked by a reduction in beneficial bacterial species, such as Roseburia intestinalis, and an increase in opportunistic pathobionts, like Peptostreptococcus stomatis. We previously observed that specific Eubacteriales, including R. intestinalis, were significantly reduced in CRC patients and have a potent anti-tumor immune effect when applied as oral monotherapy in mice. Here, w

Relationship between negative emotions and physical activity engagement after colorectal cancer: a network analysis study.

BACKGROUND: From the moment of diagnosis, colorectal cancer patients face negative emotions, which can persist into the post-cancer period. Maintaining physical activity during and after colorectal cancer is increasingly recommended to preserve both physical and psychological health. This study aims to examine the links between negative emotions and motivation to engage in physical activity (PA) in the post-colorectal cancer recovery period, using network analysis. METHODS: Sixty-three people in the post-colorectal cancer recovery period and 62 people with no cancer history completed question

Deep Learning and Image Generator Health Tabular Data (IGHT) for Predicting Overall Survival in Patients With Colorectal Cancer: Retrospective Study.

BACKGROUND: Recent advances in artificial intelligence (AI) have contributed to improved predictive modeling in health care, particularly in oncology. Traditional methods often rely on structured tabular data, but these approaches can struggle to capture complex interactions among clinical variables. Image generator for health tabular data (IGHT) transform tabular electronic medical record (EMR) data into structured 2D image matrices, enabling the use of powerful computer vision-based deep learning models. This approach offers a novel baseline for survival prediction in colorectal cancer by le

Germline Sequencing of Familial and Sporadic Early-Onset Colorectal Cancer: A Novel Pattern of Genes.

The majority of early-onset colorectal cancers (EOCRCs) are not substantiated by germline variants in the main CRC predisposition genes (the "DIGE" panel). To identify potentially novel EOCRC-specific predisposition genes, we analyzed 585 cancer pathway genes in an EOCRC patient cohort (n = 87, diagnosis ≤ 40 years, DIGE-), and compared their variant spectrum to the GnomAD cancer-free database. We identified high-impact variants (HVs) in 15 genes significantly over-represented in EOCRC. Among the 32 unrelated patients with a CRC family history (i.e., with a potentially dominant transmission pa

Demographics and Clinical Characteristics of Patients With CPUE: A Retrospective Study.

GOALS: This study examines the demographic and clinical characteristics of patients with CPUE (colonic polyposis of unknown etiology) to determine who may benefit from increased surveillance. BACKGROUND: CPUE refers to individuals with 10 or more colonic adenomas with no identifiable genetic mutation as determined by multigene panel testing. Unlike hereditary syndromes, such as familial adenomatous polyposis, CPUE lacks well-established guidelines for polyp and colorectal cancer surveillance, relying heavily on expert opinion. STUDY: This single-center retrospective study included 75 individ

Spillover effect of a dietary intervention on physical activity in a randomized controlled trial with colorectal cancer patients.

BACKGROUND: Randomized controlled studies (RCTs) targeting dietary changes may also lead to other, untargeted changes in lifestyle habits, as spillover effects. In particular, the isolated impact of the dietary intervention may be difficult to separate due to spillover effects from changes in physical activity and physical function. Therefore, the aim of this study was to investigate the spillover effect of a one-year dietary intervention in post-surgery colorectal cancer patients by comparing the changes in physical activity and physical function between the diet intervention group and the co

Using a Mobile Health App (ColonClean) to Enhance the Effectiveness of Bowel Preparation: Development and Usability Study.

BACKGROUND: Colonoscopy is the standard diagnostic method for colorectal cancer. Patients usually receive written and verbal instructions for bowel preparation (BP) before the procedure. Failure to understand the importance of BP can lead to inadequate BP in 25%-30% of patients. The quality of BP impacts the success of colonoscopy in diagnostic yield and adenoma detection. We developed the "ColonClean" mobile health (mHealth) app for Android devices. It incorporates visual representations of dietary guidelines, steps for using bowel cleansing agents, and observations of the last bowel movement

The rules of different B cell subtypes in colorectal cancer: friends or foes?

Tumor-infiltrating B cells (TIBs) are the most important cell type involved in the immune response. TIBs display considerable intratumor heterogeneity due to genetic variation, epigenetic differences and transcriptional plasticity in the tumor microenvironment (TME). Owing to the unique anatomical location of CRC, the B cell subpopulation exhibits more extensive heterogeneity. Many studies have shown that TIBs have gradually become a key predictor of immunotherapy for malignant cancers, including CRC. TIBs have essential functions, including antigen presentation and antibody secretion, and the
