Understanding and Addressing Wait Times in Specialized Geriatric Services: Findings From a Multi-Phase Engagement Process.

Persistent wait times in Specialized Geriatric Services increase risks for older adults living with frailty and place additional pressure on primary care. While workforce shortages contribute to delays, variation across programs suggests that access is also influenced by how referral, triage, assessment, follow-up, and care integration are organized. The Regional Geriatric Program of Toronto conducted a multi-phase engagement initiative including a targeted literature scan, stakeholder interviews and surveys, and a regional summit to identify system-level barriers and prioritize opportunities

Event-Based Fault-Tolerant Control for Wheel Driving Intelligent Vehicle System.

This study investigates a fault-tolerant control scheme for the autonomous intelligent vehicle, which is actuated by four independent motors. The main purpose is to realize the trajectory tracking control, to avoid collision accidents, and to save communication resources. First, by using the event-triggered sampled output, the state, fault efficiency index, and disturbance information are acquired through a novel observer. The convergence performance of the proposed observer is proved theoretically. Subsequently, to achieve the control purpose of this study, an event-based adaptive sliding mod

Mechanistic machine learning for prediction of prime editing outcomes.

Prime editing (PE) can make specific local changes to genomic DNA in living systems but its efficient application currently requires extensive optimization of PE guide RNA (pegRNA) sequences. Here we present OptiPrime, a machine learning model of PE efficiency based on current understanding of PE mechanisms. OptiPrime achieves state-of-the-art accuracy on PE efficiency prediction and enables prediction of nicking guide RNA (PE3) and dual pegRNA (twinPE) outcomes. We validate that OptiPrime has learned the determinants of mammalian mismatch repair (MMR) and is well suited for nominating MMR-eva

Chemogenomic maps reveal a PRDX1-dependent iron-damage axis in the DNA damage response.

The DNA damage response (DDR) is a sophisticated network of cellular pathways whose perturbation leads to genome instability and is a key hallmark of oncogenesis. Here, we present data from 32 genome-scale loss-of-function CRISPR interference chemical-genetic screens with inhibitors targeting core constituents of the DDR machinery (PARP, ATR, ATM, DNAPK and WEE1), as both single agents and in combination with poly(ADP-ribose) polymerase inhibitors. These experiments identify >1,000 genes whose perturbation modifies the DDR and provides a rich resource to the DDR community. In addition, this co

Preparedness deserts: a framework for understanding and addressing geographic inequities in bystander response to out-of-hospital cardiac arrest.

Geographic disparities in out-of-hospital cardiac arrest (OHCA) survival are well-documented, with predominantly Black and low-income communities experiencing significantly worse outcomes than affluent White neighborhoods. These disparities persist independently of patient-level factors, implicating structural determinants that shape bystander response capacity. This commentary introduces the concept of preparedness deserts as an analytic framework for moving beyond documentation of disparities toward targeted intervention. The framework distinguishes between training access and equipment acce

Family medicine and primary health care in Liberia.

This country profile examines the development of family medicine (FM) and primary health care (PHC) in Liberia, a low-income West African nation with significant health and socioeconomic challenges. Liberia's PHC system is structured across three tiers and relies heavily on community health workers to serve dispersed and underserved populations. Family medicine is an emerging specialty, crucial for strengthening PHC and advancing universal health coverage, especially in rural areas facing workforce shortages. The Family Medicine Specialty Training Program, launched in 2017, addresses the defic

Changes in Carabidae Biodiversity in the Perennial Flowering Strip and Adjacent Arable Crops in Organic Conditions.

Landscape simplification resulting from agricultural intensification threatens biodiversity and ecosystem services on arable land. Semi-natural habitats, such as flower strips, can mitigate these effects by providing habitat and resource diversity and by modifying the microclimate at the agricultural landscape scale. As effective natural enemies in arable fields, ground beetles (Carabidae) play a key role not only in pest regulation, but also in weed control through seed consumption. The study found that FS serve as crucial habitats supporting Carabidae-characterized by a higher Shannon-Wiener

Silicon enhancement of nitrogen use efficiency in plants: insights from soil nitrogen transformation, plant nitrogen uptake, assimilation, and remobilization.

Severe nitrogen (N) deficiency inhibits crop growth and yield, while low use efficiency of N fertilizer causes resource waste and environmental harm. Enhancing crop N use efficiency (NUE) is thus a critical scientific challenge. Ecofriendly and cost-effective silicon (Si) fertilizer can enhance NUE across the entire soil-plant continuum. Si-mediated improvements in soil physicochemical properties and microbial community structure underlie enhanced mineralization and suppressed denitrification. Si-cell wall interactions support symbiont recognition, nodule initiation, and formation of functiona

Molecular correlates of sleep deprivation in the mouse brain identified by meta-analysis of microarray data.

Studying the transcriptional changes in the brain following sleep deprivation has provided insight into the molecular mechanisms that differ between sleep and wake. Individual studies are limited in their ability to detect differentially expressed genes due to small sample size. Here we performed a meta-analysis of published brain expression data, totalling 173 microarrays across 245 mice. 498 genes were identified as significantly changing with sleep-deprivation at q < 0.01, 96 of which were previously identified by the original studies. Of the remaining 402 novel candidate sleep genes, 14 we

The phenotypic landscape of the mycobacterial cell.

The Mycobacteriales are an order of diverse bacteria that thrive in many environmental and host-associated niches. Because the most notorious member of this clade, Mycobacterium tuberculosis, is a major human pathogen, research on Mycobacteriales has focused on pathogenesis, and, as a consequence, many fundamental aspects of Mycobacterial biology remain understudied. Here, we address this gap by performing a genome-wide CRISPRi chemical genomics screen using a diverse set of >35 antibiotics, detergents, and other anti-microbials predominantly targeting the cell envelope of Mycobacterium smegma

The Impact of Hurricane Maria on Puerto Rico's Long-Term Care Settings.

In September 2017, Hurricane Maria struck Puerto Rico, devastating the island for years to come. Puerto Rico has a rapidly aging population and does not have the long-term care (LTC) infrastructure of the United States mainland. This research describes experiences of individuals working in, residing in, and providing disaster response to LTC settings in Puerto Rico during and after Hurricane Maria. From January to March 2019, N = 20 in-person interviews were conducted with nursing home staff in three Puerto Rican nursing homes; in-home caregivers; and individuals providing disaster response. T

Guidance for chest-CT in children and adults with cystic fibrosis: A European perspective.

The European Cystic Fibrosis Society-Clinical Trials Network (ECFS-CTN) herein proposes guidance for the use of chest CT-scans for the regular monitoring of lung disease in CF. Statements were completed in a 3-step process: the questions were identified via an anonymous online survey, followed by a comprehensive literature search, and a final Delphi process. The guidance recommends the use of ultra-low dose CT scans (effective radiation dose, 0.08 mSv; equivalent to 2 to 4 chest X-rays), tracking of patients' cumulative radiation and effective communication strategies using "de-medicalized" in

Single-nuclear transcriptomics of lymphedema-associated adipose reveals a pro-lymphangiogenic stromal cell population.

Chronic lymphedema is a progressive, disfiguring disease that results from dysfunction of the lymphatic vasculature, causing distal accumulation of interstitial fluid, localized development of tissue edema, and expansion of subcutaneous adipose tissue (SAT). As the molecular mechanisms governing SAT remodeling in this disease are unclear, we performed single-nucleus RNA sequencing on paired control and affected SAT biopsies from patients with unilateral lymphedema. Lymphedema samples were characterized by expansion of SAA + adipocytes, pro-adipogenic stem cells, and proliferation of lymphatic
