Non-operative management of distal humerus fractures in the elderly: a review of functional outcomes.

Comminuted distal humerus fractures in the elderly have traditionally been managed by ORIF or total elbow arthroplasty (TEA). This poses a treatment dilemma in elderly patients where anaesthetic and surgical risks combine with poor bone and wound healing. We aimed to assess the functional outcomes in patients managed non-operatively, with TEA being used as the salvage procedure. Retrospective analysis of patients over 65 years presenting to our unit between 2005 and 2015 was undertaken. Sixty-two patients were identified, 38 had died, and 5 were lost to follow-up leaving 5 with immediate TEA a…

Does shoulder impingement syndrome affect the shoulder kinematics and associated muscle activity in archers?

BACKGROUND: Archery related injuries, such as shoulder impingement syndrome are caused by repeated motion of the shoulder. The aim of this study was to analyze differences in the shoulder kinematics and the associated muscle activity between archers with shoulder impingement and uninjured archery players. METHODS: Thirty male archers, who were divided into an impingement group and an uninjured group, were included in this study. The angle of scapular elevation, shoulder joint abduction, horizontal extension, and elbow joint flexion as well as the electromyographic activity of the upper trapez…

Prophylaxis and treatment of chronic synovitis in haemophilia patients with inhibitors.

Prophylaxis is paramount to try to avoid the development of haemophilic synovitis. The best treatment for synovitis in patients with inhibitors is radioactive synoviorthesis (rhenium for ankle and elbows, yttrium for knees). With both methods (prohylaxis and radioactive synoviorthesis), we can delay the development of severe haemophilic arthropathy, that eventually will require major orthopaedic surgery.

Leukemic Arthritis Mimicking Septic Arthritis in a Pediatric Patient.

CASE: A 7-year-old girl presented with a painful effusion in her right knee, fever, and a negative joint aspirate. One week later, she presented again with an effusion in her left elbow, elevated inflammatory markers, and a joint aspirate that raised concerns for septic arthritis. Irrigation and debridement of the elbow were performed, and the cultures returned negative results. Subsequently, the child experienced ongoing migratory polyarthropathy over several months, which was ultimately identified as leukemia. The initiation of chemotherapy resolved her symptoms. Although the orthopaedic lit…

Combined Pediatric T-condylar Humeral Fracture and Monteggia Fracture-Dislocation in the Upper Extremity: A Case Report.

Monteggia fractures are characterized by a fracture of the ulna associated with radial head dislocation. T-condylar fractures of the humerus are intra-articular injuries that disrupt the distal humeral epiphysis. This report describes a rare case of a pediatric patient with an ipsilateral T-condylar humeral fracture and a Monteggia fracture-dislocation. A 10-year-old girl presented with elbow pain and deformity after falling while playing basketball. No open wounds or neurological deficits were observed, and the radial artery was palpable. The radiograph revealed a pediatric T-condylar humeral…

Very High Movement Speed in a Hitting Action Affects Interjoint Coordination But Not Spatial Accuracy.

Performance of ballistic movements to drive a ball toward a spatial target in different sports requires high movement speed associated with … nt speeds induced the lowest peak and contact velocity variabilities of the handle displacement and elbow extension. Assessment of interjoint coordination between the shoulder, elbow, and wrist indicated higher relative phase values in very high speed compared with lower movement speeds. Comparison of wrist range of motion and muscle strength in assembly workers with and without lateral epicondylitis.

BACKGROUND: Lateral epicondylitis (LE), also called tennis elbow, is a common musculoskeletal disorder that causes pain in the elbow area and is highly prevalent in assembly workers who repeatedly move their wrists. OBJECTIVE: The purpose of this study was to compare the wrist ROM and muscle strength of assembly workers with and without LE. METHODS: Forty-five male assembly line workers (23 with LE) participated in the study. Participants had their wrist range of motion (flexion, extension, ulnar deviation, and radial deviation) and strength (wrist flexors, extensors, and hand grip) measured…

The excitability of ipsilateral motor evoked potentials is not task-specific and spatially distinct from the contralateral motor hotspot.

OBJECTIVE: The role of ipsilateral descending motor pathways in voluntary movement of humans is still a matter of debate, with partly contradictory results. The aim of our study therefore was to examine the excitability of ipsilateral motor evoked potentials (iMEPs) regarding site and the specificity for unilateral and bilateral elbow flexion extension tasks. METHODS: MR-navigated transcranial magnetic stimulation mapping of the dominant hemisphere was performed in twenty healthy participants during tonic unilateral (iBB), bilateral homologous (bBB) or bilateral antagonistic elbow flexion-ext…

Perturbation-induced electromyographic activity is predictive of flexion synergy expression and a sensitive measure of post-stroke motor impairment.

The goal of this study was to explore whether the stretch reflex-induced muscle activity is correlated with the expression of the flexion synergy and, therefore, can serve as a quantitative indicator of post-stroke motor impairment. Eleven stroke participants stroke were recruited for this study. Their forearm was connected to a robotic device that applied continuous position perturbations to the paretic elbow joint. The magnitude of EMG activity of the spastic biceps brachii was measured. The expression of the flexion synergy was determined using the increase of synergistic elbow flexion torq…
