3',5'-cyclic adenosine monophosphate (cAMP) is a fundamental secondary messenger capable of rapidly amplifying and propagating cellular signals in response to various extracellular stimuli. cAMP plays a significant role in hormone-mediated regulation of renal fluid and electrolyte balance. Impaired signaling of cAMP has been linked to a variety of pathological ramifications in the kidneys. This review explores the physiological functions of exchange proteins directly activated by cAMP (Epac) in renal water balance and the regulation of solute transport in the renal tubule. Additionally, the in

Cystic fibrosis (CF), a genetic disorder caused by pathogenic variants in the CFTR gene, is associated with various complications including cystic fibrosis-related diabetes (CFRD). CFRD is an entity distinct from type 1 or type 2 diabetes. We report a rare case of diabetic ketoacidosis (DKA) in a pediatric patient with CFRD, occurring during a significant pulmonary exacerbation. The patient's management involved addressing fluid and electrolyte imbalances, careful monitoring of nutritional status, and correction of hyperglycemia with insulin. This case serves as a reminder to consider DKA in t

A 71-year-old man with bothersome lower urinary tract symptoms from an enlarged prostate underwent monopolar transurethral resection of the prostate (TURP). Following the 75-min procedure, he developed hypertension, pallor, and shivering. Laboratory tests demonstrated hyponatremia, anemia, thrombocytopenia, leukocytosis, and acute liver and kidney injury. Prompt recognition and management of electrolyte and organ involvement led to rapid improvement. However, unique to this case, mild anemia and elevated creatinine remained for years. This case and review emphasize the need to anticipate multi

A 69-year-old man underwent upper and lower gastrointestinal endoscopic examinations for a detailed evaluation of diarrhea that had persisted for several months, which revealed multiple polyposis in the stomach, duodenum, colon, and terminal ileum. The histopathological findings also led to a diagnosis of Cronkhite-Canada syndrome (CCS). On admission, the patient had hypoalbuminemia and electrolyte abnormalities, including hypokalemia, and developed paroxysmal atrial fibrillation (Paf), although there were no apparent organic lesions in the heart. The patient's sinus rhythm was restored by ele

Muscle disease has various clinical manifestations that range from exertional and non-exertional rhabdomyolysis, fasciculations, weakness, rigidity, stiffness, gait abnormalities, poor performance, and alterations in muscle mass and tone. Neurogenic disorders and non-neurogenic disorders such as primary muscle disease can cause muscle atrophy and changes in muscle tone. Myotonic disorders can have a genetic (eg, inherited channelopathies) or acquired (eg, electrolyte derangements) origin. Normal muscle enzyme activities do not rule out a myopathic disorder as the underlying cause of muscle atr

Pseudohypoaldosteronism type 1 (PHA1) is a rare, heterogeneous group of disorders characterized by resistance to aldosterone action. We report the case of a 17-day-old Saudi female infant who presented on the third day of life with persistent hyperkalemia, hyponatremia, and metabolic acidosis. Initial evaluation for congenital adrenal hyperplasia was unremarkable. Genetic testing confirmed a novel homozygous variant (c.1522C>T p.(Arg508) chr 12:6458147 in SCNN1 A) in the SCNN1A gene, consistent with the diagnosis of PHA1B, a genetically confirmed subtype of PHA1. The loop of Henle plays a key role in kidney function, especially in retaining solutes and concentrating urine. Diuretic agents, crucial for managing fluid overload and related conditions like hypertension and heart failure (HF), enhance water and electrolyte excretion. The history of diuretics dates back to 1775 with the discovery of Digitalis, evolving significantly with the introduction of chlorothiazide in 1957, which revolutionized diuretic therapy. Loop diuretics, developed in the 1950s and including drugs like torsemide, furosemide, and ethacrynic acid, are potent agents acting on the l

Electrochemistry has been widely used in energy storage, metallurgy, chemical synthesis and other fields since its birth using electricity as one of the cheapest reagents. However, compared with the revival of electrochemical synthesis of small molecules, the electrochemical polymerization for preparing macromolecules is still rudimentary due to formidable challenges. Herein, A unique electrochemical polymerization is disclosed between CS2 and diamines at room temperature in an electrolyte-free manner to access poly thioureas. Polythioureas have high selectivity for four metal ions, including

We report the case of a term neonate who was somnolent at birth with ventilatory distress and experienced 2 seizures shortly after delivery. Laboratory tests revealed the neonate had a serum sodium of 113 mmol/L. The seizures stopped after treatment with midazolam, and the sodium was corrected slowly with 3% hypertonic saline without further sequelae. The severe neonatal hyponatremia and seizures were attributed to maternal consumption of excessive amounts of coconut water during labor. This case demonstrates the importance of careful consideration of both fluid volume and fluid electrolyte co

BACKGROUND: Tumor lysis syndrome (TLS) is an oncological emergency associated with hematological malignancies or highly proliferative solid tumors, commonly after chemotherapy. It is rarely associated with transient abnormal myelopoiesis.
OBSERVATION: We report a rare case of a neonate with transient abnormal myelopoiesis and tumor lysis syndrome, complicated with concomitant heart failure due to an underlying atrioventricular septal defect. Hyperhydration was contraindicated due to heart failure. The patient was managed conservatively with full recovery.
CONCLUSION: Tumor lysis syndrome sho

Hepatic encephalopathy (HE) is a common complication in patients with advanced liver disease. It is a brain dysfunction characterized by neurological and psychiatric symptoms that significantly affects quality of life, morbidity and mortality of patients. HE has various precipitants that can potentially promote its onset, alone or in combination. Among the historically well-known precipitants, such as infections, gastrointestinal bleeding, dehydration, electrolyte disorders and constipation, recent studies have highlighted the role of malnutrition and portosystemic shunts as new precipitating

We report here the first fully automated capillary electrophoresis (CE) system that can be operated underwater. The system performs sample acquisition and analysis by coupling CE to contactless conductivity detection. Using 5 M acetic acid as the background electrolyte (BGE), inorganic cations and amino acids at concentrations as low as 5.2 μM can be separated and identified. This technology could be augmented to include a variety of other detection modes. This system serves as an early prototype for potential future underwater explorers on ocean worlds of the outer solar system such as Europa

Early diagnosis of the spiked helmet sign is challenging. This ST-elevation myocardial infarction mimic was first described in 2011 by Littmann and colleagues and was linked to severe non-coronary pathologies, with a high risk of mortality. We present a case of a 60-year-old female patient who developed severe erysipelas with sepsis associated with severe hypokalemia. She had a spiked helmet sign on her routine electrocardiogram at hospital admission. We performed a coronary angiogram that showed no culprit artery. She developed afterward an ischemic stroke. Through intensive management of the

We herein report a 10-month-old female infant with a 4-month history of diarrhea with electrolyte abnormalities and growth impairment. A 4-cm right adrenal tumor was detected by computed tomography. No metastasis or accumulation on I123 -metaiodobenzylguanidine scintigraphy was recognized in the tumor. A vasoactive intestinal peptide-secreting neuroblastic tumor was suspected, and octreotide was started, but the diarrhea persisted. Tumor extirpation was laparoscopically performed. After tumor removal, the symptoms improved immediately, and her growth caught up by 9 months after surgery. A mini

A 79-year-old Caucasian woman was admitted to the hospital with a 1-week history of general deterioration, describing malaise, abdominal pain, vomiting and diarrhea. Concomitantly, she presented with urinary retention. Laboratory tests revealed severe hyponatremia secondary to the syndrome of inappropriate antidiuretic hormone secretion (SIADH). Patient reported a tick bite 1 month earlier, followed by erythema migrans. The diagnosis of Lyme disease was immediately suspected and confirmed by positive IgM and IgG serology. Symptoms and electrolyte disturbances completely resolved with a 2-week

Patients with hip fractures are typically elderly individuals with several co-morbidities. Upon admission to the hospital, they often present with acute pain, electrolyte disturbances, anaemia, coagulopathy, and delirium. Long waiting times for surgery are associated with increased morbidity and mortality. The balance between the number of clinical tests and optimisation, which may (i.e., fewer complications and better survival) or may not (i.e., more complications and increased mortality due to unnecessary surgical delay) benefit the patient, has been a preoperative challenge. This summary wi

Coronavirus disease 2019 has rapidly enveloped the world in a pandemic after emerging in Wuhan, China, in December 2019. We describe a 49-year-old man presenting with fever, cough, dyspnea, and myalgia diagnosed with coronavirus disease 2019 along with rhabdomyolysis and acute kidney injury. The creatine phosphokinase was elevated to 23,800 U/L before trending down to normal levels. Rapid identification and treatment with aggressive intravenous hydration and correction of electrolyte abnormalities remain key to successful management. In a pandemic, often atypical presentations of this new dise

In its classical view, the renin angiotensin system (RAS) was defined as an endocrinesystem involved in blood pressure regulation and body electrolyte balance. However, the emergingconcept of tissue RAS, along with the discovery of new RAS components, increased thephysiological and clinical relevance of the system. Indeed, RAS has been shown to be expressed invarious tissues where alterations in its expression were shown to be involved in multiple diseasesincluding atherosclerosis, cardiac hypertrophy, type 2 diabetes (T2D) and renal fibrosis. In thischapter, we describe the new components of
