The authors describe a case with a closed posterior elbow dislocation associated with a distal radial fracture and complete transsection of the brachial artery. The patient had a pulseless distal upper extremity and immediate gross swelling of the elbow and forearm. As closed reduction was not possible, open reduction had to be performed through an anteromedial approach to the elbow. End-to-end suture of the brachial artery was successful. The posterior interosseous nerves of 58 cadaver forearms of black South Africans were dissected to study their order of branching. In the Centre for Child Traumatology in Brno the authors followed up during the last 10 years 22 patients with suspected compartment syndrome of the upper extremity. Incisions of intrafascial spaces were made in 12 patients-9x on the forearm and 3x on the hand. Another 10 patients had conservative treatment. Thrombocytopenia with absent radius syndrome (TAR) is a rare disorder appearing at birth or soon after. The heredity pattern is autosomal recessive, although some patients in successive generations have been reported. Ischemic necrosis of the upper extremities caused by invasive mucormycosis developed in a patient with soil contamination of severe burn wounds. An arteriogram of the arm showed complete obstruction of blood flow in the forearm. Histologic specimens showed nonseptate branching hyphae obliterating the arterial lumens. Cutaneous mucormycosis affects patients who are immunocompromised, including victims of multiple trauma and burns. From 1964 to 1980, 91 replantations of severed limbs were performed at Peking Ji Shui Tan Hospital, The People's Republic of China.
