Reliability of radiologic assessment following total elbow arthroplasty: a varying observer agreement.

BACKGROUND: Total elbow arthroplasty (TEA) is used to treat a variety of elbow pathologies. Survival rates following TEA remain relatively low, and complications are often reported. Radiographic evaluation has been used to detect complications. However, standardized assessment for evaluation is missing, and reliability of investigating different parameters has never been investigated. Therefore, this study aimed to evaluate intra- and interobserver reliability of the most commonly used radiologic parameters following TEA. METHODS: Anteroposterior and lateral radiographs of 29 patients who und

Unicompartmental Knee Arthroplasty Has Lower Infection, Conversion, and Complication Rates Compared to High Tibial Osteotomy.

INTRODUCTION: Isolated medial knee osteoarthritis can be surgically treated with either unicompartmental knee arthroplasty (UKA) or high tibial osteotomy (HTO). Proponents of UKA suggest superior survivorship, while HTO offers theoretically improved alignment and joint preservation delaying total knee arthroplasty (TKA). Therefore, we compared complications in a large population of patients undergoing UKAs or HTOs. We specifically assessed 90 days, 1 year, and 2 years: (1) periprosthetic joint infection (PJI) rates, (2) conversion to TKA rates, as well as (3) complication rates. METHODS: A re

Medial patellofemoral ligament reconstruction using a bone groove and a suture anchor at patellar: a safe and firm fixation technique and 3-year follow-up study.

BACKGROUND: Graft fixation is critical to the restoration of the medial patella of femoral ligament function and long-term success. Numerous fixations at the patella have been described, while the complications including patellar fractures, violation of the posterior patella and delay of tendon-to-bone healing remain significant challenges. Here, we describe a safe and firm fixation at the patellar for medial patellofemoral ligament (MPFL) reconstruction and explore the safety angle of drilling the suture anchor at different morphology of the patellar. Moreover, we evaluate the results at a 3-

Off-road cycling injuries. An overview.

Off-road bicycles, commonly called 'mountain bikes', have become increasingly popular worldwide since their introduction in the western US in the late 1970s. This popularity is partly because these vehicles can be ridden on a wide variety of terrain which is not accessible to other types of bicycle. Although early versions were rather crude, off-road bicycles today typically include high strength, lightweight frames with a wide array of available suspension and braking systems. Virtually all aspects of the technology continue to evolve, including components and protective equipment. As the pop

Long-term patient-reported outcomes and quality of life after tibiofemoral knee dislocations in patients with open growth plates: a retrospective case series from a Level 1 trauma center.

BACKGROUND: Tibiofemoral dislocations in children and adolescents are rare, and treatment guidelines are primarily based on adult data and algorithms. This case series of four pediatric and adolescent patients aims to add to the limited literature and highlights key aspects of diagnosis, treatment, and patient-reported outcomes. METHODS: We conducted a retrospective-prospective single-center case series of pediatric and adolescent patients (< 18 years, open growth plates) treated for tibiofemoral knee dislocations at a Level 1 trauma center between 2017 and 2019. Clinical, radiographic, and i

Management of shoulder prosthetic joint infections with humeral bone loss using a customizable articulating long stem spacer (The Frankenspacer).

BACKGROUND: With an increasing number of shoulder arthroplasties being performed, prosthetic joint infection (PJI) of the shoulder and proximal humeral bone loss (PHBL) remain challenging complications for surgeons to manage. While various strategies exist for managing patients with shoulder PJI or PHBL, there is limited information regarding treatment options and outcomes in patients with combined PJI and PHBL. The purpose of this study is to describe a novel technique for treatment of this unique patient population along with its risk of complications and reinfection in patients undergoing s

Transfer of osteochondral shell autografts to salvage femoral head impaction injuries in hip trauma patients.

INTRODUCTION: Femoral head impaction defects are observed with variable severity, as a result of traumatic hip dislocations which can be caused by traffic accidents or seen in professional athletes amongst other mechanisms. Compression of the articular cartilage and the subchondral bone into the femoral head results in irregular articular surfaces influencing the outcome with predisposition to osteoarthritis, and being predictive for the need for delayed total hip replacement. This study reports the outcome after a minimum follow-up (FU) of five years in a consecutive series treated with trans

Skeletal Maturity Might Not Be a Factor in Optimizing Outcomes in Total Hip Arthroplasty.

Introduction Total hip arthroplasty (THA) is rarely indicated in the skeletally immature population. In these instances, there is concern for implant survival compared to the traditional older population. There has been a steady rise in the use of THA in the pediatric population due to improvements in surgical techniques. While the outcomes in THA for skeletally immature patients have been described in the literature, there are no population studies looking at this procedure in a skeletally immature individual. Therefore, the purpose of this study was to compare 10-year implant survivability f

[Management of patients after pacemaker implanation (author's transl)].

Although pacemaker therapy is characterized by a high degree of reliability, follow-up of pacemaker patients is necessary to provide early detection of pacemaker failure and optimal setting of programable pacemakers as well as to render adequate treatment of underlying disease. The most common cause of pacemaker failure is battery depletion. In spite of the available data on the mean life-expectancy of the various batteries, the individual time of depletion cannot be predicted with accuracy. Furthermore, a defect in the electronic circuitry and/or electrode may still be rarely encountered even

Orthogonal Radiographs After Shoulder Reduction: Are We Meeting the Standard?

Background Anterior shoulder dislocation is a common emergency presentation. Post-reduction radiographs are essential to confirm concentric reduction and exclude associated injuries. Guidelines from the British Elbow and Shoulder Society (BESS) and local hospital policy recommend orthogonal views, including anteroposterior and either axillary or scapular Y projections. Failure to obtain adequate imaging risks missed malreduction, overlooked fractures, and unnecessary repeat imaging. Methods We conducted a retrospective service evaluation of 22 consecutive patients with anterior shoulder disloc

Adoption of Robotic Arm-Assisted Total Hip Arthroplasty Results in Reliable Clinical and Radiographic Outcomes at Minimum Two-Year Follow Up.

INTRODUCTION: Longevity and success of total hip arthroplasty (THA) is largely dependent on component positioning. While use of robotic platforms can improve this positioning, published evidence on its clinical benefits is limited. Therefore, the aim of this study was to assess the clinical outcomes of THA with robotic surgical assistance. MATERIALS AND METHODS: We conducted an analysis of robotic arm-assisted primary THAs performed by a single surgeon utilizing a posterior approach. A total of 99 patients (107 cases) who had a minimum two-year follow up were identified. Their mean age was 61

Arthroscopic Reduction and Suspensory Button Fixation of Bony Bankart Lesion.

BACKGROUND: Bony Bankart lesions (anteroinferior glenoid avulsion fractures) often occur after anterior shoulder dislocations, with an incidence up to 33% in first-time dislocaters. Detachment of the labrum and underlying glenoid bone piece can lead to recurrent instability, especially with increasing glenoid bone loss. Treatment is generally arthroscopic bony Bankart repair in acute injuries with viable bone fragments and minimal glenoid bone loss (<13.5%). However, there is no gold standard for arthroscopic fixation. INDICATIONS: Given the acuity of the lesion with a sizeable bony Bankart f

Incidence and risk factors for pseudosubluxation of the humeral head following proximal humerus fracture.

BACKGROUND: Humeral head pseudosubluxation (HHPS) in relation to the glenohumeral joint is a common finding following fractures of the proximal humerus. The temporary inferior subluxation of the humeral head may be secondary to a transient axillary nerve neuropraxia, pain inhibition of the deltoid, or hemarthrosis or capsular disruption that alters the physiologically negative pressure in the glenohumeral joint. Despite the frequency of this finding, it is not well described in the literature. This study sought to describe the incidence, risk factors, and rate of resolution of HHPS following p
