In a 12-year old girl suffering from autosomal recessive axonal Charcot-Marie-Tooth (CMT) neuropathy, pes cavovarus was treated with botulinum toxin injection in the tibialis posterior. The patient underwent a clinical evaluation, video analysis of spatiotemporal gait parameters and dynamic foot plantar pressure assessment before treatment and then two weeks, three months and six months thereafter. The video gait analysis revealed a decrease in varus during the swing phase of gait. The dynamic foot plantar pressure decreased by 50% in the excessive pressure at the side of the foot six months a…

CONTEXT: Skeletal muscle weakness and impaired gait function are common risk factors for disease and even death. Therefore, identification of the modifiable causes of skeletal muscle weakness should have high priority. Knowledge regarding optimal vitamin D treatment in cases of pancreatic insufficiency is scarce.
CASE REPORT: We report a case of a slow decrease in ability to walk distances more than 100 m during the previous 6 months. Low exocrine pancreatic function resulting in phosphorus, magnesium and vitamin D deficiency was found. Medical treatment with peroral pancreatic enzymes, phosp…

BACKGROUND AND AIM: In order to increase understanding of the biomechanical consequences of equinus foot on gait, we developed an orthosis to induce an adjustable degree of unilateral equinus. The aim of this study was to evaluate its feasibility and consequences on 3D ankle kinematics and kinetics.
TECHNIQUE: 3D gait analysis was carried out in 10 healthy children without the orthosis, with the non-adjusted orthosis and with the orthosis adjusted to +10°, 0°, -10°, -20° of ankle dorsiflexion and maximum plantarflexion (MP). The amount of dorsiflexion at initial contact was close to the gonio…

We assessed the validity of the hip-knee-ankle angle measured statically during three-dimensional (3-D) gait analysis and the tibial angle using an inclinometer compared with the mechanical axis on radiographs. Eleven individuals (20 knees) with radiographic knee osteoarthritis (OA) participated in this study. We determined the following: the lower-limb mechanical axis using weightbearing long-leg radiographs; hip-knee-ankle angle using the techniques of 3-D gait analysis in a static standing position; and tibial alignment using an inclinometer. The mean mechanical axis (+/- standard deviation…

The treadmill is frequently used in research and clinical assessments for gait analysis. To evaluate mechanical energy and dynamics of walking, the fluctuations of the treadmill speed have to be taken into account. A new algorithm is presented in this study to determine instantaneous treadmill speed using solely the kinematic data of subjects. The algorithm uses an automatic detection of heel contact (HC) and toe off (TO) during treadmill walking. Kinematic data were collected from two groups (healthy adult, n=11; hemiplegic adult, n=9). The gait events determination is validated by comparison…

BACKGROUND: Periacetabular osteotomy improves radiographic predictors of osteoarthrosis and diminishes pain and functional impairment. No changes in function quantified by gait analysis have yet been documented. We evaluated the functional outcome of periacetabular osteotomy in relation to gait.
METHODS: The gait pattern of 9 women (median age 39 years) with hip dysplasia who were treated with unilateral periacetabular osteotomy was analyzed before periacetabular osteotomy and an average of 1.5 years afterwards. Data were collected using 5 video cameras and 2 force plates. An inverse dynamics…

BACKGROUND: Dopa-responsive dystonia is a genetically determined disorder with early onset. The dystonia usually manifests as a disturbance of gait with fatigue and may be confused with spasticity. The diagnosis is based on clinical recognition and response to l-dopa, which is usually complete and long lasting. The most common genetic defect involves the gene for GTP cyclohydroxylase I.
MATERIAL AND METHODS: We describe a Norwegian family in which three generations are affected.
RESULTS: All those affected had gait disturbance from childhood; the disturbance became worse during the day and a…

Eighteen patients (mean age of 66.5 years) with normal pressure hydrocephalus (NPH) underwent a ventriculo-peritoneal shunt surgery. Prior to operation a cerebrospinal fluid tap-test (CSF-TT) was performed with measurements of gait pattern and psychometric functions (memory, visuo-motor speed and visuo-constructive skills) before and after the removal of 50 ml CSF by lumbar puncture (LP). Fifteen patients improved and 3 were unchanged after surgery. Short duration of disease, gait disturbance preceding mental deterioration, wide temporal horns and small sulci on CT-scan were associated with go…

Normal-pressure hydrocephalus (NPH) is a common cause of potentially reversible dementia. It can present with psychiatric manifestations that may hinder its diagnosis. A 68-year-old man presented with a paranoid psychosis and mild cognitive impairment, but not neurological signs or classic "triad." Gait disturbance and urinary incontinence developed later in the course of illness after the diagnosis of NPH had already been made on computed tomographic (CT) scanning. A lumbo-peritoneal shunt was performed, followed by full remission of psychotic symptoms, as well as considerable improvement in …

Rectus femoris transfer was performed in 78 children (105 sides) with cerebral palsy (CP) at the same time as other surgical procedures as appropriate. The transfer was either medial to the sartorius (62 sides), semitendinosus (19 sides), or the gracilis (14 sides) muscles, or laterally to the iliotibial band (10 sides). Gait analysis performed before and 1 year after operation demonstrated increased knee range of motion (ROM) with increased extension at initial contact and in midstance and maintained knee flexion in swing. There were no statistically significant differences between the four t…

The recent attempt to show that the Hadar australopithecine female 'Lucy' (AL 288-1) had hindlimbs too short to allow a modern pattern of striding bipedal gait has important implications for understanding the origin of bipedalism, if not for the more general problem of hominid origins. Combined with previous claims that Lucy had a forelimb unusually long in proportion and ape-like in morphology, the additional contention of a relatively short hindlimb would suggest a very different pattern of gait from the norm of today because the effectiveness of the pendulum action of the lower limb during …

Conventional bracing places constraints on the determinants of gait, which in the borderline patient, may derange gait as much or more than the postural deformity being treated. A small number of children with cerebral palsy, spina bifida, and muscular dystrophy were treated with below-knee orthoses (BKO) along with elastic bracing of the remainder of the limb. The elastics were used to augment muscle power in weakness, oppose spastic muscles, supply antagonism to unopposed muscle, and improve function. The key elements in this system include proximal attachment of elastics to the below-knee o…

Anterior transfer of the long toe flexors was carried out for the treatment of spastic equinovarus foot deformity in both adults and children. Adults included those with hemiplegia subsequent to a stroke, spastic hemiplegia due to cerebral palsy and spastic spinal paraplegia. Most of the children had cerebral palsy. The transfer was indicated for an equinovarus foot with persistent activity of the toe flexors, which produced curling of the toes in the swing phase of the gait or a fixed hammer toe deformity. Fifty six patients were followed up for more than four years. In all cases correction o…

Dynamic alignment refers to the process of adjusting prosthetic components during the user's gait. Recently, there has been increasing focus on this area due to its critical role in restoring natural limb movement and meeting individual user needs. This review aims to examine the various devices, methods, and techniques reported in the literature for assessing dynamic alignment. The review is structured to address dynamic alignment in human prostheses, including techniques for measuring forces and moments, the effects on gait cycle parameters, external influences, and alignment adjustment devi…

This paper presents design of a novel modular lower-limb gait exoskeleton built within the FP7 BioMot project. Exoskeleton employs a variable stiffness actuator in all 6 joints, a directional-flexibility structure and a novel physical humanrobot interfacing, which allows it to deliver the required output while minimally constraining user's gait by providing passive degrees of freedom. Due to modularity, the exoskeleton can be used as a full lower-limb orthosis, a single-joint orthosis in any of the three joints, and a two-joint orthosis in a combination of any of the two joints. By employing a…

Some orthopedic complications have been reported in the hereditary neuropathies. However, the association of the hip dysplasia with this category of neuropathy is rarely recognized. We present a 13-year-old boy with the progressive weakness of the lower extremities, difficulty in walking, climbing stairs, and rising from floor; a wide-based, hyper-extended and waddling gait similar to a myopathic process. Hip radiography showed dysplastic acetabulae with hip subluxation, broken Shenton's lines, and valgus femoral necks. In electrodiagnosis, there was a significant neuropathic process (absent a…
