Porphyrias comprise a heterogeneous group of predominantly genetically determined metabolic diseases which are due to a dysfunction in heme biosynthesis. Variegate porphyria and hereditary coproporphyria are referred to as neurocutaneous porphyrias because affected patients can develop both cutaneous symptoms on light-exposed body sites and potentially life-threatening acute neurovisceral symptoms, thereby mimicking several other diseases. In this overview, we provide an update on pathogenesis, clinical manifestation, diagnosis, and therapy of these two types of porphyria.

The wide spectrum of congenital cytomegalovirus (CMV) disease and known differences in the biology and in vitro growth of CMV strains continue to drive studies in search for specific viral genetic determinants that may predict severity of congenital CMV disease. Several CMV genes have been studied in detail in congenitally infected children, but the complexity of the viral genome and differences in the definition of symptomatic disease versus asymptomatic CMV infection continue to raise questions related to what constitutes a pathogenic CMV strain.

Toxic epidermal necrolysis (TEN) is a severe cutaneous drug reaction with a mortality rate of approximately 30%. The hallmark of TEN is widespread epidermal sloughing due to keratinocyte apoptosis. Multiple genetic associations between TEN and specific ethnic populations have been determined. The pathophysiology of TEN has yet to be fully elucidated; however, current pathogenic models implicate Fas ligand, granulysin, and reactive oxygen species. The value of current therapies, such as intravenous immunoglobulin and corticosteroids, remains under evaluation.

Mutations in the cancer susceptibility genes BRCA1 and BRCA2 are associated with significantly increased risks of breast and ovarian cancer. Fortunately, effective strategies are available to reduce these risks, including genetic testing, which is an important consideration in determining management of patients with a strong family history of cancer. This article reviews the current evidence for risk-reducing strategies in BRCA1 and BRCA2 mutation carriers and outlines future research directions. In particular, screening controversies and current guidelines are discussed, as are issues related

Duchenne muscular dystrophy (DMD) is a genetically determined disease. The chief characteristic of this clinical syndrome is a progressive degeneration of particular groups of muscles. Initially there is physical impairment, contractures, and muscular atrophy. Subsequently, the disease immobilizes the patient (ages 9 to 14 years) and severe respiratory failure occurs due to chest and vertebral column deformity. We present principles for the dental care of patients with DMD. It is important to introduce dental and prophylactic treatment at a very early age for children with DMD.

Formation of a phenotype during development of the human being may result from a specific dialogue between genes and environmental factors. Expression of particular genes is controlled not only on the transcriptional level but also on the level of accessibility of genetic information through the influence for remodelling of chromatin. We characterized Rett syndrome and its molecular basis as an example of the relation between genes and environment and their influence on epigenetic processes determining the gene expression. Examples of importance of DNA methylation in psychiatric disorders were

The murine model of infection with Leishmania major has allowed the demonstration in vivo of the importance CD4+ T cell subsets, distinguishable by the pattern of cytokines they produce, on the outcome of infectious diseases. Genetically determined resistance and susceptibility to infection with this parasite are the result of the development of Th1 and Th2 response, respectively. In this short paper, we present some results obtained in our group pertaining to the analysis of the mechanisms, operational during the early phase of this infection, responsible for the maturation of these functiona

Primary immunodeficiency disorders are genetically determined failures of immune defense that increase susceptibility to infectious agents. This article reviews the salient features of equine primary immunodeficiency disorders, summarizes the molecular mechanisms of each disorder, and updates information that facilitates diagnosis and management of affected horses. The central theme is to encourage clinicians to ask, "I wonder if this horse has an underlying primary immunodeficiency disorder?" when caring for horses suffering from chronic and recurring infections and responding poorly to stand

The HDCTs constitute a heterogeneous group of rare genetically determined diseases, the best known of which are Ehlers-Danlos and Marfan syndromes and osteogenesis imperfecta. Hypermobility is a feature common to them all, but it is also a feature that is highly prevalent in the population at large. Symptomatic hypermobile subjects (whose symptoms are attributable to their hypermobility) are said to be suffering from the benign joint hypermobility syndrome, which has many features that overlap with the HDCTs. It is not yet known whether there is a variety of hypermobility (symptomatic or other

In summary, the child who develops the symptoms of the specific rash, proximal muscle weakness, or fatigue should seek medical care promptly. With the advances in physical and medical therapy, many of the consequences of the disease can now be ameliorated. Data suggest that JDMS and PM may each have a different pathophysiology, but more evidence is needed. The next few years will be exciting as there is a national effort by an increased number of investigators to determine the epidemiologic and genetic factors that influence JDMS disease susceptibility and severity.

This article describes some of the genodermatoses encountered in the black populations in Africa, starting with ainhum, which appears to be genetically determined. Palmoplantar keratodermas are common in Africa but are difficult to classify and their hereditary nature is not always recognized. Of the ichthyoses, the lamellar type is commonest, with the so-called bathing-suit distribution apparently unique to Africans. Albinism and xeroderma pigmentosum are important causes of cutaneous malignancy in the tropics. Neurofibromatosis and tuberous sclerosis are relatively common, whereas hypermelan

Bronchial hyperresponsiveness (BHR) is a major feature of asthma that is determined by both genetic and environmental factors. The assessment of BHR can be a valuable means of assessing asthma severity and also response to therapy. BHR can be measured with a variety of provocative agents including exercise, methacholine, histamine, hypotonic and hypertonic saline and cold air. Methacholine and histamine challenges have been well studied and standardized. They are extremely sensitive indicators of BHR, but this is not synonymous with asthma. This requires that the physician interprets the clini

DNA single strand-breaks were examined in lymphocytes of patients treated with metronidazole (CAS 443-48-1) for trichomonas vaginalis infection before, on day one, and day seven of treatment. Recommended dosage of metronidazole induces DNA single strand-breaks. Single strand-breaks were repaired after therapy. No irreversible DNA damage remains. The DNA repair mechanisms are genetically determined. Therefore a mutagenic effect of metronidazole must be discussed in patients with DNA repair defects.

We report on the appearance of porphyria cutanea tarda in a patient with systemic LE and in two patients with discoid LE. A review is given on the corresponding literature. It is suggested that both lupus erythematosus and porphyria cutanea tarda have multifactorial inheritance. The cause of coexistence can be common genes responsible for the genetic determination which also predispose to the occurrence of both diseases. The question of etiology still remains obscure. This coexistence raises, however, a more practical question as regards the therapeutic modalities for the two diseases.

Thirty probands with the diagnosis of hysterical psychopathy were studied using clinical and genealogic techniques. An analysis of the obtained data made with the help of modern geneticomathematical methods identified a significant correlation between the type of psychopathy in probands and the corresponding type of personality disorders in the first-kin relatives of the probands. The calculated rate of heredity in relation to the risk of hysterical pathology manifestation (in the framework of quasi-permanent multifactor model) (79.9%) suggests that the formation of hysterical psychopathy is t

Overuse injury (RSI) of the upper limb is a common and distinct clinical entity brought about by hard continuous use of the limb. Pain is felt in the muscles and joint ligaments whose biological tolerance has been exceeded; this tolerance varies individually and must be genetically determined. These structures are normally non-tender. Physical signs in overuse injury consist of tenderness in these injured structures. Such signs in the intrinsic muscles of the hand and ligaments of the wrist joint and carpometacarpal joint of the thumb are virtually diagnostic of overuse injury.
