[Congenital hypoplastic left ventricle--a case report].

A case of congenital hypoplasia of left heart ventricle is presented with regard to its clinical and pathophysiological aspects. A detailed prenatal ultrasonic or echocardiographic diagnosis seems to be absolutely necessary. Each sonographer should at least be able to advise of any suspicion of fetal heart abnormalities.

The teratogenic effect of phenobarbital on the embryonic chick heart.

Phenobarbital was observed to produce cardiovascular malformations in embryonic chicks. Malformations included simple ventricular septal def … cardiovascular malformations compared with lesser doses and with saline. A significant reduction in heart rate and abnormal rhythm of the heart were observed in embryos treated with teratogenic doses of phenobarbital. No arrhythmia nor significant changes in heart rate were observed in embryos exposed to subteratogenic doses of phenobarbital or to saline.

New index of cardiac contractility during stress testing with treadmill.

Experimental demonstration that the external phonocardiogram is similar to the third derivative (rate of acceleration) of LV pressure, and t … htfold increase during maximal stress. Comparison of the changes in amplitude of S1 with changes of heart rate, blood pressure, and cardiac output revealed that the first showed the most constant proportional increase while sometimes one or the other parameter showed only a moderate change. The possible causes of this apparent lack of reaction is discussed. …

Ambulatory electrocardiographic recording in mild or moderate myotonic dystrophy and myotonia congenita (Thomsen's disease).

Ambulatory electrocardiographic recording (AER) was performed on 27 adult patients with mild or moderate myotonic dystrophy (MD) and in 12 adult patients with mild myotonia congenita (MC) to determine whether characteristic arrhythmias and conduction disturbances occur in the early stages of these myotonic disorders. In MD and MC, AER showed at least one finding regarded as normal in 93% and 0% of cases, respectively. The most common abnormalities in MD were first degree atrioventricular (A-V) block (in 70% of cases) and QTc greater than or equal to 0.46 s (in 33%). Abnormal sinus intervals an

Review article. Congenital complete heart block.

Isolated congenital complete heart block has a heterogenous aetiology. In approximately one-third of the patients in the present material the mothers had symptoms or signs of connective tissue disease. The mortality is highest in the neonatal period, much lower during childhood and adolescence and increases slowly later in life. There are elderly patients who are completely asymptomatic but patients are also encountered with signs and symptoms of more or less severe myocardial damage. Stokes-Adams attacks may occur at any age. About 10% of patients below 15 years of age in this material and ab

Sick sinus syndrome in childhood.

The clinical and electrocardiographic findings in five children with the sick sinus syndrome and an otherwise normal heart are described. There were three boys and two girls. Their age at onset of either bradycardia or symptoms ranged from 1 day to 7 years. In one patient, the youngest ever reported with this syndrome, bradycardia was noted before birth. Four children presented with neurological symptoms--attacks of dizziness, fainting spells, or syncope. One boy, treated for epilepsy before the underlying arrhythmia ws diagnosed, died suddenly while playing. One child had near-fatal syncope c

Effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker in awake dogs.

The effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker was evaluated in 15 awake dogs with experimentally induced A-V junctional rhythm. The duration of asystole after overdrive (D.A.O.) in these dogs was prolonged significantly in accordance with increase in the drive rate, and the mean +/- SD of the D.A.O. reached 4.7 +/- 1.1 seconds (N = 15) after overdrive at 2.5 times the spontaneous heart rate. After administration of atropine (0.4 mg/kg; i.v.) to eight dogs, the mean +/- SD of the D.A.O. at the same rate decreased from 4.5 +/- 0.9 to 3.4 +/- 1.2 seconds…

The effect of propranolol upon chick embryo cardiogenesis.

The teratogenic effects of propranolol HCl on cardiac development were studied in chick embryos of days 3 and 4 of incubation. Propranolol w … e incidence of cardiac anomalies in the controls was very low. Propranolol was observed to slow the heart rate in the experimental embryos. It is suggested that slowing of heart rate at the early stages of heart development caused aberrant bloodstream flow patterns which probably resulted in the genesis of cardiac anomalies. The results of this study indicat …

Effect of propranolol on left ventricular function, segmental wall motion, and diastolic pressure-volume relation in man.

Precise quantitation of the effects of the non-selective beta adrenergic blocking drug propranolol (3.15 mg/kg body weight) on left ventricular function, segmental wall motion, and diastolic pressure-volume relation in man has been performed. High fidelity left ventricular pressure measurements and simultaneous single-plane angiocardiograms were recorded on a video disc and volumes calculated by a light-pen computer system. Systolic segmental wall motion was computer analysed using the long axis-quadrasection method. Patients were transvenously atrially paced to maintain a constant heart rate.…

Clinical applications of the threshold tracking pacemaker.

The concept of automatic threshold tracking is the most recent adaptation of artificial cardiac pacemakers to physiologic needs. Earlier pacemaker innovations were primarily in the area of timing of impulse delivery, whereas more recent changes have focused on alteration of the amplitude and duration of the pacemaker stimulus. Matching of this stimulus to the cardiac excitation threshold is important because a stimulus greater than necessary is wasteful of battery energy and may produce arrhythmias, whereas a stimulus of less than threshold intensity fails to pace the heart. Because of the wid

Freezing of gaze during action preparation under threat imminence.

When confronted with threatening stimuli, animals typically respond with freezing behavior characterized by reduced movement and heart rate deceleration. Freezing-like responses during threat anticipation have also been observed in humans and are associated with anxiety. Recent evidence yet suggests that freezing does not necessarily reflect helpless immobility but can also aid the preparation of a threat escape. To investigate which further behavioral responses human freezing encompasses, we presented 50 young adults (10 male) with aversive stimuli that could sometimes be avoided while measur…

Psychophysiological Reactivity in Child Sexual Abuse.

Sexual abuse has physiological and emotional implications. The purpose of this study is to evaluate the neurobiological sequels of childhood sexual trauma by monitoring physiological variables among sexually abused girls and women compared to controls. We assessed posttrauma and traumatic life events of 35 females sexually abused in their childhood (age range 7-51 years) and 25 control females (age range 7-54 years). Electroencephalography, frontalis electromyography, electrodermal activity, and heart rate parameters were recorded while watching sets of pictures representing neutral and trauma…

The fetal heart monitor tracing in pregnancies complicated by fetal thyrotoxicosis.

Fetal tachycardia caused by the transplacental passage of thyroid stimulating immunoglobulin is well established, but very little information exists regarding the fetal heart rate appearance of this form of tachycardia. Two cases of fetal tachycardia are described in pregnancies complicated by fetal thyrotoxicosis where maternal thyroid history was unknown at the time of testing. For each pregnancy, the fetal heart monitor tracing is depicted and described. In both cases, the initial fetal heart rate pattern demonstrated a sustained baseline of 170 to 180 beats per minute with moderate variabi…

Comparison of the effects of thiopentone and propofol on the electrocardiogram of dogs.

Sixty-four dogs were randomly assigned to receive either thiopentone or propofol and their electrocardiograms were recorded immediately before and shortly after they were anaesthetised. Thiopentone caused a marked increase in QT and JT intervals, a flattening of the T-wave and an increase in precordial QT dispersion. Propofol induced a less marked increase in QT and JT intervals, corrected for heart rate. Both agents induced an increase in heart rate and a decrease in heart rate variability, consistent with reduced vagal tone. Shortly after anaesthesia was induced, thiopentone affected ventric…

Characterization of shock in a hamster model of hantavirus infection.

Human hantavirus cardiopulmonary syndrome (HCPS) due to Andes, sin nombre and other hantaviruses is characterized by severe pulmonary capillary leak and cardiogenic shock. Hamsters, the only animal manifesting HCPS-like disease, were instrumented with radiotelemeters that enabled ambulatory intracarotid blood pressure recording within an animal biosafety level-4 facility. Following infection with Andes virus, blood pressure and heart rate decreased slowly in a biphasic manner during the first 7 days of infection, followed by a rapid fall in pressure and rapid increase in heart rate during the …

Trigeminal depressor response during percutaneous microcompression of the trigeminal ganglion for trigeminal neuralgia.

Percutaneous microcompression of the trigeminal ganglion for trigeminal neuralgia was performed 23 times on 21 patients. Significant abrupt drops in heart rate and blood pressure (P less than 0.0002) occurred when the needle entered the foramen ovale or upon balloon advancement or inflation. In 16 of 23 (70%) procedures, the heart rate fell abruptly to 60 or less, by a mean of 38%. Mean arterial blood pressure decreased transiently by 31% during 12 of 23 (55%) procedures. Our findings of transient bradycardia and hypotension upon mechanical stimulation or compression of the mandibular nerve or…
