Helicobacter pylori seropositivity among Swedish adults with and without abdominal symptoms. A population-based epidemiologic study.

BACKGROUND: The role of Helicobacter pylori in functional dyspepsia is unclear. The aim of this population-based study was to determine whether the prevalence of H. pylori infection is higher among people with dyspepsia or irritable bowel syndrome (IBS) than among symptomless persons after control for age, sex, and socioeconomic status. METHODS: In a postal questionnaire we asked a representative sample (20-79 years; n = 1260) from a Swedish municipality about abdominal symptoms in the preceding 3 months. A randomly selected subsample, 50 with dyspepsia, 50 with IBS, and 50 symptomless, match…

[Endoscopic diagnosis of Helicobacter pylori distribution in human gastric mucosa by phenol red dye spraying method].

Helicobacter pylori (H. pylori) is known to show an uneven and patchy distribution on human gastric mucosa, close association with chronic gastritis and peptic ulcer diseases. The aim of this study was to clarify the distribution of this organism under direct vision by phenol red dye spraying endoscopy. In order to maintain gastric pH around pH 4.0 to 5.0, premedication was necessary either by intravenous injection of H2-blocker, 30 min before the test, or by oral administration of Omeprazole 20 mg or a half dose of H2-blocker before bedtime the day before the test. After that, 0.05% phenol re…

Treatment of Helicobacter pylori-associated gastroduodenal disease in children. Clinical evaluation of antisecretory vs antibacterial therapy.

The charts of 54 children diagnosed with antral H. pylori were reviewed, to establish the incidence of gastroduodenal inflammation and compare therapeutic efficacies of antisecretory vs. antibacterial therapy. Histology demonstrated normal mucosa in three cases (6%) and gastric/duodenal inflammation (> or = Whitehead grade 3) in 51 biopsies (94%). 23/43 children (53%) initially responded to H2-blockers; however, by 10 mo, 13 had relapsed clinically. All of these patients subsequently responded to amoxicillin plus bismuth subsalicylate. Of the 20 children who failed to enter remission after an

A Three-Way Comparison of Nodular Lesions in H. pylori-Induced Gastritis, Non-Helicobacter pylori Helicobacter (NHPH)-Induced Gastritis, and NHPH-Induced MALT Lymphoma Reveals Their Distinct Endoscopic Predictors: Distribution for Bacterial Etiology and Morphology for Malignancy.

BACKGROUND: Nodular gastritis associated with Helicobacter pylori infection (HPi-NG) has long been recognized. Recently, similar mucosal changes have been reported with non-H. pylori Helicobacter (NHPH), including NHPH-induced gastric mucosa-associated lymphoid tissue lymphoma with a nodular gastritis-like appearance (NHPHi-MNG) and NHPH-induced nodular gastritis (NHPHi-NG). However, how bacterial species differences and the presence or absence of malignancy are reflected in endoscopic appearance has not been sufficiently investigated. This study aimed to clarify these relationships and to ide…

Helicobacter pylori eradication status is associated with lower motor severity despite similar levodopa pharmacokinetics in Parkinson's disease.

BACKGROUND: Helicobacter pylori (H. pylori) infection has been associated with impaired levodopa absorption and worse motor symptoms in Parkinson's disease (PD). However, the relationships among H. pylori infection, eradication history, levodopa pharmacokinetics, and motor severity remain unclear. METHODS: We retrospectively analyzed 40 patients with PD who underwent standardized levodopa/carbidopa pharmacokinetic testing. Participants were classified as HP-positive, never infected, or prior eradication. Plasma levodopa and carbidopa concentrations were measured serially after administration …

Population heterogeneity in Helicobacter pylori PMSS1 shapes variable mouse infectivity: derivation of the homogeneous reference strain PMSS2.

UNLABELLED: Experimental infection models are widely used to investigate host-microbe interactions, often under the assumption that bacterial populations are genetically uniform. Here, we examined population heterogeneity in the widely used Helicobacter pylori strain PMSS1 and its relationship to variation in mouse infectivity. Single-colony isolates derived from PMSS1 displayed substantial differences in colonization efficiency, indicating that pre-existing variation within the population contributes to infection outcomes. To distinguish the effects of initial population heterogeneity from ch…

In silico design of a VacA-based multi-epitope Subunit Vaccine Candidate for immunoprotection against Helicobacter pylori.

Helicobacter pylori, a globally prevalent gastric pathogen associated with gastric carcinoma, ulcers of the bowel, and persistent gastritis where the vacuolating cytotoxin A (vacA), a major virulence factor, that promotes cellular damage and immune evasion. Despite increasing antibiotic resistance, no licensed vaccine exists for effective prevention or treatment. In present study, we implemented an all-encompassing immunoinformatic strategy in developing a novel vacA- driven multi-epitope subunit vaccine (MESV). Utilising the IEDB database, cytotoxic as well as helper T-cell epitopes were dete…

Proteomic insights into Helicobacter pylori infection in stomach cells, revealing host response and host-targeted therapeutics repurposing.

BACKGROUND: Helicobacter pylori (H. pylori) is a globally prevalent gastric pathogen strongly associated with chronic gastritis, peptic ulcers, and gastric cancer. While bacterial factors have been extensively studied, host proteomic responses and their therapeutic potential remain largely underexplored. RESEARCH DESIGN AND METHODS: Current analyses employed a systematic proteomics-based data integration and harmonization approach (retrospective qualitative cohort study) to identify important differentially regulated host proteins. Proteomic datasets were curated from in vitro studies and ana…

Within-host diversity and phased variant analysis reveal structures and recombination of Helicobacter pylori subpopulations in stomach.

BACKGROUND: Helicobacter pylori (H. pylori) has a highly plastic genome and can generate substantial within-host diversity during chronic gastric colonization. However, the delineation of its within-host subpopulations, particularly regarding the emergence and spread of antibiotic resistance-conferring mutations, remains poorly understood. RESULTS: In this study, we enrolled 25 chronic gastritis patients from southern China, collecting multiple isolates from distinct gastric regions. Among them, 14 patients exhibited heterogeneity in antibiotic susceptibility across isolates (heteroresistant)…

Helicobacter pylori eradication in the face of escalating anti-microbial resistance: Geographic heterogeneity and emerging treatment strategies.

Helicobacter pylori (H. pylori) infection is a global health problem that affects more than half of the world population and leads to gastrointestinal disorders such as gastric ulcer, duodenal ulcer and gastric cancer. The available treatment options are inefficient because of patient non-adherence to the regimen due to the polypharmacy approach of treating H. pylori over longer periods. The bio-availability of antibiotics at the gastric mucosa, where the organism resides, is also a major concern. The continuous adaptation of bacteria such as genetic mutation toward beta-lactamase, macrolide a…

Immunological effects of coinfections with Toxoplasma gondii, Helicobacter pylori, and Epstein-Barr virus on anisakiasis associated phenotypes.

The hygiene hypothesis proposes that infections acquired through the fecal-oral route or contaminated food, such as Toxoplasma gondii, may protect against IgE-mediated allergic disorders. Epstein-Barr virus (EBV) has been negatively associated with atopy, while Anisakis simplex and Helicobacter pylori are associated with acute and chronic urticaria, respectively. This study aimed to explore the impact of coinfection status on anisakiasis-associated phenotypes. A total of 112 human subjects were categorized by Anisakis sensitization status and clinical phenotypes: 25 with gastroallergic anisaki…
