From 1977 to 1989, 28 patients, 18 men and 10 women, underwent 29 proximal vertebral artery reconstructions. Indications were vertebrobasilar insufficiency in 17 patients and TIA or stroke in 6. Five were asymptomatic. Procedures performed were endarterectomy in 20, reimplantation in 5, venous bypass in 3, and decompression in 1 case. Concomitant procedures were coronary artery bypass grafting (6) and carotid, subclavian, or innominate artery revascularization (14). There was no operative mortality. Laparoscopic cholecystectomy was carried out in 340 patients in the period November 9, 1990, to November 8, 1991. Preoperatively sonographical selection was in use. We reviewed the records of 277 patients aged 80 or older that had been considered for surgery during the period 1980-1990. We analyzed the surgical risk (Goldman and ASA score), patient history, type of surgery and the post-operative course (Karnofsky index) in order to identify the risk factors and to permit objective and practical selection of patients amenable to surgical treatment. Primary palmar hyperhidrosis is a functionally and socially disabling condition. Upper thoracic sympathectomy is the best curative treatment. Several surgical approaches have been suggested and, recently, less invasive techniques have been communicated. We present here the clinical results with a second-generation porcine bioprosthesis, the Carpentier-Edwards supra-annular valve (CESA). Two-hundred and twenty-two CESA bioprostheses were implanted in 189 patients during a four-year period (from 1984 to 1987), either as an isolated procedure or associated to mitral or tricuspid repair. A prospective study was carried out to compare blood loss from various types of nasal surgery, to investigate how accurately this loss was estimated during surgery and to find out how soon any losses were replenished. A review of 1111 transurethral resections of the prostate (TURP) revealed a mortality of 0.5% (6 patients). A stricture frequency of 3% was observed as a complication after the operation. Anatomical repair of transposition of the great arteries (TGA) seems more attractive than the more conventional atrial baffle procedures because of resultant left ventricular-aortic continuity. The results of anatomical repair in 16 consecutive neonates with TGA and intact ventricular septum were reviewed; special consideration was given to technique and guidelines to avoid complications. Abdominal operative procedures in properly selected patients have a 90 per cent 5-year success rate in alleviating stress incontinence. The best overall method of selection is by radiographic and preferably fluoroscopic characterization of the response of the urethra and bladder to a change in intra-abdominal pressure. Operative treatment was applied in 139 patients with hemodynamically significant bilateral stenosis of the carotid arteries due to atherosclerosis. Deaths following tracheal resection anastomosis for tumoral or iatrogenic disease have become fairly rare (four deaths in 122 cases in our experience).
