Comparison of effects on voice of diode laser and cold knife microlaryngology techniques for vocal fold polyps.

AIM: To compare the effects on voice of endolaryngeal microsurgery (EMS) with cold instruments and a new method, "diode laser," for vocal fold polyps. PATIENTS AND METHODS: Fifty-one patients with vocal fold polyps suffering from dysphonia who were treated in the Erciyes University Department of Otolaryngology were included in the study. Voice analysis was performed in a soundproof room, holding the microphone 15 cm away from the patients' mouth and by recording a sustained [a] vowel for at least 10 seconds. Fundamental frequency (F0), Jitter, Shimmer, and noise-to-harmonic ratio (NHR) parame

Comparison of techniques for double-lumen endobronchial intubation: 90° or 180° rotation during advancement through the glottis.

BACKGROUND: During endobronchial intubation with a double-lumen endobronchial tube (DLT), the DLT is conventionally rotated through 90° when the bronchial tip is just past the vocal cords. This study was performed to investigate if rotation of the DLT through 180° decreases postoperative hoarseness, sore throat, or vocal cord injuries. METHODS: Patients (n=164) undergoing thoracic surgery were randomized into two groups. Just after the bronchial tip passed the glottis, left-sided DLTs were rotated 90° (Group 90, n=84) or 180° (Group 180, n=80) counterclockwise and advanced. In the Group 180,

Lysis of interarytenoid synechia (Type I Posterior Glottic Stenosis): vocal fold mobility and airway results.

BACKGROUND: The Type I Posterior Glottic Stenosis (PGS-I) is a well-described but uncommon clinical entity. Despite this, there is little known about the outcome of surgical treatment. METHODS: Retrospective case series. RESULTS: Thirteen cases met inclusion criteria. All but one patient had a tracheostomy at the time of initial evaluation. At the postoperative visit, seven patients (54%) had completely normal vocal fold motion. Of the verbal patients, 6 (50%) had normal vocal function as reported by both the patient/caregiver and the physician, and 10 (83%) patients were successfully decann

Electrophysiologic recurrent laryngeal nerve monitoring during thyroid and parathyroid surgery: international standards guideline statement.

Intraoperative neural monitoring (IONM) during thyroid and parathyroid surgery has gained widespread acceptance as an adjunct to the gold standard of visual nerve identification. Despite the increasing use of IONM, review of the literature and clinical experience confirms there is little uniformity in application of and results from nerve monitoring across different centers. We provide a review of the literature and cumulative experience of the multidisciplinary International Neural Monitoring Study Group with IONM spanning nearly 15 years. The study group focused its initial work on formulati

Airway complications from topical mitomycin C.

OBJECTIVE: Topical application of mitomycin C appears to be a useful adjunct in reducing cicatricial scarring of the airways. Human and animal studies have demonstrated the efficacy and safety of mitomycin C topically in the treatment of airway stenosis at concentrations ranging from 0.4 mg/mL to 10 mg/mL. Although no reports of mitomycin C toxicity have been reported in the otolaryngology literature, the ophthalmologic literature has documented serious, vision-threatening complications resulting from the use of topical mitomycin C. The purpose of this study is to report complications related

A study of an electrodiagnostic technique for the evaluation of equine recurrent laryngeal neuropathy.

The electrodiagnostic measurement of the thoracolaryngeal reflex (TLR) ('slap test') latency was compared to 5 other diagnostic techniques used for evaluation of laryngeal function, namely laryngeal muscle palpation, resting and immediately postexercise endoscopic examinations, and palpable and endoscopic responses to the TLR. Compared to resting endoscopy, the electrodiagnostic measurement of TLR latency was not found to be an accurate test for the evaluation of recurrent laryngeal neuropathy (RLN), nor was laryngeal muscle palpation or the endoscopic response to the TLR. Twenty-five (71%) of

Laryngopharyngeal sensory deficits in patients with laryngopharyngeal reflux and dysphagia.

There are no reliable means of quantifying the edema that results from acid exposure to the posterior larynx in patients with laryngopharyngeal reflux (LPR). However, it is possible to quantify laryngopharyngeal sensitivity in these patients by endoscopic administration of air pulses to the laryngeal mucosa in order to elicit the laryngeal adductor reflex. The purpose of this study was to determine whether patients with LPR have sensory deficits in the laryngopharynx, and whether treatment of these patients with a proton pump inhibitor (PPI) results in resolution of sensory deficits. History of Head and Neck Cancer as a Predictor of Recurrence and Second Primary Tumors After Transoral Videolaryngoscopic Surgery in Patients With Hypopharyngeal Squamous Cell Carcinoma.

BACKGROUND: Surveillance after transoral videolaryngoscopic surgery (TOVS) for early-stage hypopharyngeal squamous cell carcinoma (HPSCC) remains unvalidated, particularly regarding posttreatment local recurrence and second primary head and neck squamous cell carcinoma (HNSCC). METHODS: We retrospectively reviewed 84 patients with early-stage HPSCC treated with TOVS at a single high-volume center. Post-TOVS "HNSCC events" were defined as local recurrence or second primary HNSCC diagnosed after TOVS. Event-free survival (EFS) was defined as time to first post-TOVS HNSCC event or death. RESULT

Phasegram Analysis of Vocal Fold Vibration Documented With Laryngeal High-speed Video Endoscopy.

INTRODUCTION: In a recent publication, the phasegram, a bifurcation diagram over time, has been introduced as an intuitive visualization tool for assessing the vibratory states of oscillating systems. Here, this nonlinear dynamics approach is augmented with quantitative analysis parameters, and it is applied to clinical laryngeal high-speed video (HSV) endoscopic recordings of healthy and pathological phonations. METHODS: HSV data from a total of 73 females diagnosed as healthy (n = 42), or with functional dysphonia (n = 15) or with unilateral vocal fold paralysis (n = 16), were quantitativel
