Acute Myeloid Leukemia Presenting As Spontaneous Splenic Rupture: A Case Report.

Spontaneous splenic rupture (SSR) is a rare but potentially life-threatening condition that may occur in the setting of underlying splenic pathology, including hematologic malignancies such as acute myeloid leukemia (AML). We report the case of a 28-year-old man who presented with sudden-onset severe abdominal pain. Imaging revealed massive hemoperitoneum secondary to SSR. Laboratory investigations demonstrated severe anemia, thrombocytopenia, leukocytosis, and circulating blasts on peripheral smear, with normal coagulation parameters. The patient underwent urgent splenectomy, and histopatholo…

Mixed phenotype acute leukemia: 13 patients and a proposed management algorithm.

Mixed phenotype acute leukemia (MPAL) is a rare, heterogeneous group of acute leukemias of ambiguous lineage (2-5% of cases) with no consensus treatment. We retrospectively reviewed all patients with MPAL managed in a single Tunisian hematology department between January 2014 and December 2023. Diagnosis relied on multiparameter flow-cytometric immunophenotyping using the EGIL score and the WHO classifications; survival was estimated by the Kaplan-Meier method. Thirteen patients were included (median age 23 years, range 2-46; sex ratio ≈ 1.1). Twelve had a B/myeloid and one a T/myeloid phenoty…

PTBP1 controls oncogenic transcript processing and maintenance of acute myeloid leukemia.

RNA-binding proteins are critical regulators of gene expression in both normal physiology and cancer. Here we set out to systematically annotate the functions of RNA-associated proteins across multiple cancer types via domain-focused CRISPR screens targeting RNA-modifying enzymes and RNA-binding proteins. We utilized 3182 sgRNAs targeting 527 RNA-enzymatic and binding domains in 341 RNA-associated proteins and identified multiple RNA-binding proteins as dependencies in acute myeloid leukemia (AML), including the RNA splicing factor PTBP1 and the N6-methyladenosine reader RBM15, with a bias tow…

RNA Sequencing Technologies in Acute Lymphoblastic Leukemia: A Comparative Technical Review.

Acute Lymphoblastic Leukemia (ALL) is a highly heterogeneous hematological malignancy characterized by diverse genetic alterations. Advances in genomic and transcriptomic profiling have enabled refined ALL classification, improving clinical management and patient outcomes. This review provides a comparative evaluation of RNA sequencing methodologies for studying gene expression in ALL. Specifically, bulk RNA sequencing enables transcriptome-wide profiling at the population level, while targeted RNA sequencing provides enhanced sensitivity for detecting clinically relevant alterations. Moreover…

Targeting ZMIZ1 induces differentiation in acute myeloid leukemia via chromatin remodeling.

Acute myeloid leukemia (AML) is an aggressive hematologic malignancy characterized by profound transcriptional dysregulation and impaired hematopoietic differentiation. While differentiation therapy has revolutionized treatment in acute promyelocytic leukemia (APL), analogous strategies in non-APL AML remain elusive. Here, we identify ZMIZ1 (Zinc finger MIZ domain-containing protein 1) as a previously unrecognized transcriptional co-regulator that enforces differentiation blockade in AML from a targeted CRISPR-Cas9 screen. ZMIZ1 expression is elevated in AML patients and correlates with advers…

Long-term outcomes with continuous venetoclax for relapsed chronic lymphocytic leukemia.

Venetoclax was first approved for continuous use in relapsed/refractory (R/R) chronic lymphocytic leukemia (CLL), but the efficacy and consequences of very long-term BCL2 inhibition are unknown. We describe the frequency, characteristics, and outcomes of patients with R/R CLL treated with >5 years of continuous venetoclax. Long-term responders (>5 years of continuous therapy without progressive disease [PD]) were identified from a cohort of 86 patients with R/R CLL treated with continuous venetoclax with or without rituximab. Landmark analyses at 2 and 5 years assessed association between unde…

Maintenance Therapy in Acute Myeloid Leukemia: Current Perspectives and Future Directions.

The management of acute myeloid leukemia (AML) remains characterized by high relapse rates despite advances in induction and consolidation therapy. Relapse prevention represents a major unmet need, particularly in patients ineligible for allogeneic hematopoietic stem cell transplantation (allo-HSCT) or at high risk of post-transplant recurrence. This review examines current evidence supporting maintenance strategies following intensive chemotherapy or allo-HSCT, with emphasis on measurable residual disease (MRD)-guided approaches and targeted therapies. We summarize data from randomized and ph…

Targeting PRMT5 in Adult T-Cell Leukemia/Lymphoma: Opportunities and Challenges.

Adult T-cell leukemia/lymphoma (ATLL) is an aggressive T-cell malignancy caused by persistent infection with human T-cell leukemia virus type 1 (HTLV-1). ATLL remains difficult to treat despite intensive chemotherapy, antiviral therapy, and hematopoietic stem cell transplantation. The limited durability of current treatment strategies highlights the need for mechanism-based therapeutic approaches. Protein arginine methyltransferase 5 (PRMT5) is a type II arginine methyltransferase that regulates transcription, RNA splicing, DNA damage responses, and immune signaling through symmetric dimethyla…

Newly diagnosed acute myeloid leukemia in Fit patients: 2026 treatment algorithms.

The introduction of venetoclax (a BCL2 inhibitor) and targeted therapies, including inhibitors of CD33, FLT3, IDH1, IDH2, and menin, has expanded treatment options for newly diagnosed acute myeloid leukemia (AML). In younger, fit patients, the primary goal remains long-term survival, which in most cases is secured through allogeneic stem cell transplant. Transplant in first complete remission is recommended for FLT-ITD, TP53 mutated, KMT2A rearranged, AML with other adverse genetic abnormalities, and is considered in most intermediate-risk patients. It is also recommended in relapsed/refractor…

[Key points in selecting first-line therapy for chronic myeloid leukemia].

Chronic myeloid leukemia (CML) is driven by the BCR::ABL1 fusion gene, and the introduction of tyrosine kinase inhibitors (TKIs) has dramatically improved long-term survival. In Japan, five agents-imatinib (Glivec®), dasatinib (Sprycel®), nilotinib (Tasigna®), bosutinib (Bosulif®), and STAMP inhibitor asciminib (Scemblix®)-are currently approved for first-line therapy. Each has distinct efficacy and toxicity profiles, requiring individualized treatment decisions based on age, comorbidities, cardiovascular risk, fertility, adherence, and financial factors. Recent advances have shifted treatment…

Pirtobrutinib in Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma: A Systematic Review.

OBJECTIVES: Pirtobrutinib, a noncovalent Bruton tyrosine kinase inhibitor, was developed to overcome resistance mutations that limit covalent BTK inhibitors and to offer improved tolerability. Three phase III randomized trials (BRUIN CLL-321, CLL-313, and CLL-314) have evaluated pirtobrutinib against distinct comparators in different chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) populations, but no pooled evidence exists, as of now. METHODS: Following PRISMA 2020 guidance for systematic reviews without meta-analysis, we searched Embase, Scopus, and PubMed from inception to…

Chronic myeloid leukemia: incorporation of recent advances into current treatment algorithms.

Several approved and investigational BCR::ABL1 tyrosine kinase inhibitors (TKIs) and STAMP inhibitors are used for the treatment of chronic myeloid leukemia in chronic phase (CML-CP). In the frontline setting, multiple factors affect selection of therapy including the goal of treatment, cost of TKIs, CML risk, and patient's comorbidities. Achieving a complete cytogenetic response (BCR::ABL1 transcripts on the International Scale [IS] <1%) with TKI therapy within the first year is associated with normalization of survival, whereas achieving a deeper molecular response (BCR::ABL1 transcripts < 0…

Clinical Outcomes in Double-Exposed Chronic Lymphocytic Leukemia Patients in Italy.

B-cell receptor inhibitors (BCRi) and B-cell lymphoma-2 inhibitor (BCL2i) improved outcomes of patients with chronic lymphocytic leukemia (CLL), but relapsing after two inhibitors still represent an unmet clinical need. This multicenter real-world study analyzes outcomes of a cohort treated in Italy between May 2017 and September 2023 following prior exposure to both BCRi and BCL2i. The median follow-up after venetoclax initiation was 47 months (IQR 28-56). Of 153 double-exposed patients, 104 (68%) discontinued venetoclax and 53 of them (51%) received a subsequent treatment. Venetoclax was dis…

Bovine leukemia virus: an emerging concern for zoonotic cross-species transmission.

Bovine leukemia virus (BLV) is a retrovirus that causes leukemia-like disorders in cattle and sheep, as well as inflammatory conditions and reduced milk production in dairy cows, leading to substantial economic losses in the global cattle industry. BLV infection is widely prevalent among cattle populations worldwide. Emerging evidence has indicated a potential association between BLV infection and human breast cancer development. In several regions, BLV DNA has been detected in human breast tissue samples, particularly in tumor cells, suggesting that BLV may contribute to the development of br…

Beyond BCL-2: What drives venetoclax resistance in acute myeloid leukemia?

The BCL-2 inhibitor venetoclax has transformed outcomes for older or frail patients with acute myeloid leukemia (AML), and its resistance mechanisms are becoming better defined, including compensatory and lineage-associated switches toward MCL-1 or BCL-xL dependence, oncogenic signaling activation, blast phenotype, and differentiation stage. Additional putative mechanisms-such as emerging BAX mutations, mitochondrial structure remodeling, integrated stress response, and metabolic adaptations, including enhanced amino acid uptake and fatty acid oxidation to sustain oxidative phosphorylation-req…

3D Chromosome Remodeling in B-cell Development and Acute Lymphoblastic Leukemia.

UNLABELLED: The identification of molecular subgroups of pediatric B-cell acute lymphocytic leukemia (B-ALL) has proven to be a powerful tool in understanding disease pathogenesis and treatment stratification. Studies have suggested aberrant transcription factor function and epigenetic regulation can explain differences between B-ALL subtypes; however, the impact of 3D genome reorganization remains unclear. In this study, we used in situ Hi-C and RNA sequencing to profile the chromatin architectural landscape in healthy B-cell progenitors and B-ALL patient samples harboring prognostically rele…

Secondary B-acute lymphoblastic leukemia following lenalidomide maintenance in multiple myeloma.

Following the advent of immunomodulatory agents (IMiDs) in the treatment of multiple myeloma (MM), the progression-free survival has improved significantly. This has also led to increased recognition of second primary malignancy (SPM), like myelodysplastic syndrome (MDS)/acute myeloid leukemia (AML) among MM patients treated with IMiDs, especially lenalidomide. The occurrence of acute lymphoblastic leukemia (ALL) post lenalidomide therapy is rare. A 61 years old male with kappa light chain MM was treated with lenalidomide-dexamethasone induction for 6 cycles, followed by lenalidomide maintenan…

Philadelphia Chromosome-Positive B-cell Acute Lymphoblastic Leukemia: A Case Report.

B-cell acute lymphoblastic leukemia (B-ALL) with the Philadelphia translocation-positive genotype (Ph+) is a high-risk leukemia subtype in older adults, where comorbidity often limits the use of intensive management and complicates clinical decision-making. We present the case of a 77-year-old female with progressive fatigue, dizziness, dyspnea, and palpitations, whose laboratory evaluation revealed symptomatic anemia and circulating blast cells. Peripheral smear and flow cytometry confirmed B-ALL, and fluorescence in situ hybridization (FISH) demonstrated BCR-ABL translocation. Cardiac examin…

A case of true acute eosinophilic leukemia arising from myelodysplastic syndrome.

Acute leukemias of the myeloid lineage most commonly show myeloid and/or monocytic differentiation. Leukemias with erythroid, megakaryoblastic, and basophilic differentiation are more rare but are well documented. In contrast, acute leukemia showing marked morphologic and immunophenotypic features of eosinophilic differentiation is not a defined entity. When acute leukemias do show a prominent population of cells with eosinophilic differentiation, the eosinophil populations typically show morphologic features of mature eosinophils and the eosinophilic population may be clonally related to the …
