Comprehensive Review on the Protective Effects of Lycium barbarum Polysaccharide on Neurodegenerative Diseases.

Neurodegenerative diseases including Alzheimer’s disease, Parkinson’s disease, Huntington’s disease, ischemic stroke, and other related conditions, significantly impact the quality of life, particularly in low-income nations. The pathogenesis of these diseases is driven by neuroinflammation, oxidative stress, apoptosis, mitochondrial dysfunction, and regulation of autophagy. To date, no therapies or medications can fully reverse the progression of these diseases. Natural polysaccharides have demonstrated significant therapeutic potential in the treatment of neurodegenerative diseases. Lycium b

How to set up and manage a microbiome research facility.

BACKGROUND: The increasing number of therapeutic intervention studies in the microbiome field has sparked broad interest among clinicians and scientists to incorporate microbiome analyses in their research. However, microbiome study design, data generation, bioinformatics, and statistical data analysis are typically of a complexity that require specific domain expertise to avoid biases, spurious findings, and other analysis pitfalls. As such, academic hospitals and research institutes are increasingly recognizing the need for dedicated microbiome research facilities that support every step in

Multisystem developmental abnormalities and insulin resistance in ADTKD-HNF1B with the p.E105K variant: A clinical conundrum.

AIMS: HNF1B variants cause autosomal dominant tubulointerstitial kidney disease (ADTKD-HNF1B), but the phenotypic spectrum of variants of uncertain significance (VUS) remains incompletely defined. We aimed to characterise the multisystem phenotype associated with the HNF1B p.E105K variant and to examine the mechanism underlying insulin resistance in this setting. METHODS: A 15-year-old male proband with chronic kidney disease and multisystem developmental abnormalities underwent clinical, biochemical and genetic evaluation, including trio-based whole-exome sequencing. The identified variant w

Development of pharmacological interventions for the treatment of sarcopenia.

Sarcopenia is a multifactorial age-related muscle disorder of which its underlying pathophysiological mechanisms remain incompletely understood. To date, treatment strategies of sarcopenia have been largely confined to lifestyle interventions, underscoring an urgent need for pharmacological options that directly target the biological drivers of muscle deterioration. Despite increasing interest in drug development for sarcopenia, no specific pharmacological agent has yet received regulatory approval. This review provides a comprehensive synthesis of recent advances in investigational pharmacoth

Multimodal AI for predicting comorbid REM sleep behavior disorder in major depressive disorder.

REM sleep behavior disorder (RBD) is a robust prodromal marker of α-synucleinopathies: idiopathic RBD carries a 10-15-year phenoconversion risk of 80-90% to Parkinson's disease (PD) and related disorders. In major depressive disorder (MDD), comorbid RBD marks a subgroup at elevated prodromal PD risk, yet is frequently missed in psychiatric practice. Here, we developed a multimodal AI framework to detect comorbid RBD in MDD. From a clinical cohort of 329 patients, we obtained 261 video clips in 31 patients during reading and spontaneous-speech tasks, including 19 patients with MDD-RBD and 12 de

"They're intertwined really… they make each other worse": An interpretative phenomenological analysis of living with epilepsy in the context of homelessness.

People experiencing homelessness, or "unhoused people", are eight times more likely to have a diagnosis of epilepsy compared to the general population. However, little is known about how epilepsy is experienced and understood within the context of homelessness. This study used Interpretative Phenomenological Analysis to explore the lived experience of epilepsy among six unhoused men in Ireland. Four group experiential themes were developed: Diagnosis as an Explanation with No Way Out, Dual Loss of Autonomy, Public Seizures and Social Devaluation, and Living Within Constrained Survival Pathways

A Wavelet-Based Framework for Mapping Long Memory in Resting-State fMRI: Age-Related Changes in the Hippocampus From the ADHD-200 Dataset.

Functional magnetic resonance imaging (fMRI) time series are known to exhibit long-range temporal dependencies that challenge traditional modeling approaches. In this study, we propose a novel computational pipeline to characterize and interpret these dependencies using a long-memory (LM) framework, which captures the slow, power-law decay of autocorrelation in resting-state fMRI (rs-fMRI) signals. The pipeline involves voxelwise estimation of LM parameters via a wavelet-based Bayesian method, yielding spatial maps that reflect temporal dependence across the brain. These maps are then projecte

Sarcopenia and Walking Recovery in Older Adults: A Mini-Review of Evidence and Rehabilitation Implications.

Walking recovery is a central goal of geriatric rehabilitation, but it is shaped by multiple interacting factors rather than by muscle impairment alone. This mini-review examines whether sarcopenia-related weakness, low muscle quality, and reduced physical reserve identify a clinically important vulnerability phenotype relevant to delayed gait recovery in older adults after hospitalization, hip fracture, surgery, or cardiac procedures. We summarize current consensus definitions of sarcopenia, explain why muscle strength and physical performance are often more clinically informative than muscle

Machine Learning in Maternity: Early Diagnosis of Placenta Accreta Spectrum.

Placenta accreta spectrum (PAS) is a life-threatening obstetric condition marked by abnormal placental attachment and invasion into the uterine wall. Early and accurate prenatal diagnosis is crucial to improving maternal and fetal outcomes as it reduces morbidity, mortality, and complications such as severe hemorrhage and cesarean hysterectomy. Current diagnostic approaches utilize magnetic resonance and ultrasound imaging and rely heavily on clinician interpretation. This study aims to evaluate recent advancements in machine learning (ML) techniques for the early diagnosis of PAS using ultras

Multimodal diagnosis of Parkinson's disease with an internet-based collaborative agent architecture of medical language models.

Parkinson's disease (PD) remains one of the most prevalent neurodegenerative disorders, where delays in diagnosis compromise therapeutic outcomes and increase healthcare costs. Conventional unimodal approaches, based on voice, sensors, or imaging, face critical limitations, including small datasets, lack of reproducibility, and high infrastructure demands. To address these challenges, the proposed multimodal agent-based architecture integrates medical language models, audio signals, and neuroimaging, and is supported by data-machine learning pipelines and an edge-cloud infrastructure. The syst

Protocol for development of an AI-driven individualized frailty prediction and intervention framework for elderly patients with chronic kidney disease: causal feature learning and knowledge-distillation-based modeling study.

BACKGROUND: Frailty is a reversible geriatric syndrome marked by diminished physiological reserve and heightened vulnerability. In elderly patients with chronic kidney disease (CKD), frailty accelerates decline and mortality, yet individualized risk stratification and management remain limited by resource-intensive geriatric assessment and incomplete data. Integrating causal learning and model knowledge distillation may enable precision modeling of frailty risk and clinically meaningful decision support. METHODS: This multicenter bidirectional cohort study (ChiCTR2500095133) will recruit 1500

Human and large language model judgments of cognitive impairment from language: An explainable artificial intelligence approach.

INTRODUCTION: We examine how non-expert humans (young adults unfamiliar with dementia) and large language models (LLMs) perceive dementia in transcribed texts-recognizing signs that may indicate cognitive decline. Human perception is important, as it is often the driver for seeking medical evaluation. LLM perception is equally interesting given their potential as screening tools. METHODS: Humans and LLMs intuitively judged whether transcribed picture descriptions came from dementia patients or healthy controls. We represented texts using high-level, expert-guided features and used logistic re

Title: Role of O6-methylguanine DNA methyltransferase status as predictive factor for alkylatin-based chemotherapy in advanced neuroendocrine tumors: State of the art.

BACKGROUND: Temozolomide (TMZ) and other alkylating agents (ALK) yield high response rates when used in metastatic neuroendocrine tumors (NETs). Several series reported higher response rates when used in tumors with low expression of O6-Methylguanine DNA Methyltransferase (MGMT) with immunohistochemistry or promoter methylation. However, evidence is most consistent in pancreatic neuroendocrine tumors. Variations in MGMT assessment further complicate direct comparisons and clinical application. Thus, the predictive role of MGMT in patients with NETs treated with ALK is still debated. METHODS:
