Anti-interferon-γ Autoantibody-mediated Adult-onset Immunodeficiency with Disseminated Mycobacterium abscessus Complex Infection: A Report of Three Cases.

Interferon-γ serves as the pivotal cytokine for macrophage activation. Anti-interferon-γ autoantibody (AIGA)-mediated adult-onset immunodeficiency (AOID) represents a phenocopy of primary immunodeficiency, characterized by recurrent disseminated infections predominantly caused by Mycobacterium abscessus complex (MABC) and other nontuberculous mycobacteria. Disseminated MABC (dMABC) in AOID poses significant therapeutic challenges, requiring long-term multidrug antimicrobial therapy combined with AIGA-targeted immunotherapy. This report details three cases of AOID with dMABC. All patients exhib

Beyond Hotspot Mutations: Diagnostic Relevance of High Frequency, Low Frequency, and Disputed rpoB Variants in Rifampicin-Resistant Mycobacterium tuberculosis.

Rifampicin-resistant tuberculosis (RR-TB) remains a major threat to global TB control, primarily driven by mutations in the rpoB gene of Mycobacterium tuberculosis (Mtb). Most resistance-conferring mutations occur within the 81-base pair RIF resistance determining region (RRDR), particularly at codons S450L, H445Y/D, and D435V, which are strongly associated with high level resistance. However, increasing evidence of low-frequency and disputed variants both within and beyond the RRDR reveals a broader genetic spectrum that contributes to diagnostic uncertainty and variable phenotypic outcomes.

Apoptosis inhibitor of macrophage suppress immune responses via IL-10 production and delay bacterial clearance in Mycobacterium avium infection.

Non-tuberculous mycobacteria infections, including Mycobacterium avium, are increasingly recognized as a growing public health concern, even among immunocompetent individuals. These infections are a significant cause of chronic pulmonary disease, and they are characterized by the formation of foamy macrophages (FMs) that facilitate bacterial persistence. Previously, we reported that apoptosis inhibitor of macrophage (AIM), a protein secreted by macrophages, promotes lipid droplet accumulation in M. avium-infected macrophages. However, the precise role of AIM in modulating immune responses rema

Mycobacterium tuberculosis complex lineages and drug resistance patterns among tuberculosis patients with or without diabetes mellitus in southern Ghana.

Drug-resistant (DR) tuberculosis (TB) and diabetes mellitus (DM) are intersecting epidemics that complicate management of both diseases and worsen patient outcomes. We conducted a prospective cohort study of 758 GeneXpert-confirmed pulmonary TB patients, of whom 75 had DM. Demographic, clinical, radiographic, and anthropometric data were collected at baseline. Sputum samples were cultured for mycobacterial isolation, and the obtained isolates were characterized for Mycobacterium tuberculosis complex (MTBC) lineage and drug-susceptibility testing using spoligotyping and microplate alamar blue a

Relationship among ultrasound features, pus Mycobacterium tuberculosis load, and efficacy of antituberculosis drugs in patients with necrotizing tuberculous lymphadenitis.

For patients with necrotizing cervical tuberculous lymphadenitis (CTL) who have formed abscesses and are unwilling to undergo surgery, early and accurate assessment of drug therapy should be performed to guide subsequent clinical adjustments. This study investigated 22 patients with necrotizing CTL who underwent chemotherapy at our hospital from February 2020 to December 2022. They were diagnosed based on the positive results of pathogen detection methods (acid-fast bacillus smear, mycobacteria culture, Gene X-pert, and next-generation sequencing). Based on the 6-month treatment outcomes, the

Molecular Detection of Mycobacterium Leprae Using RLEP Loop-Mediated Isothermal Amplification and a Restriction Enzyme to Ensure Amplification Specificity.

Early and accurate diagnosis of leprosy is important but remains a significant challenge. Loop-mediated isothermal amplification (LAMP) is a process for the amplification of nucleic acids at a constant temperature and has been used to develop field-friendly tests for many diseases. In the present study, we describe the development of a colorimetric LAMP assay targeting Mycobacterium leprae-specific 450 bp conserved region of the repeat sequences known as RLEP. In addition, the amplicons of LAMP were subjected to restriction analysis using the enzyme EcoRV for specificity. This method has the p

Computational introspection of impact of mutation A90V and A90E in DNA gyrase a of Mycobacterium tuberculosis in developing resistance against Delafloxacin.

Quinolone group antibiotics have proven to be an effective drug against Mycobacterium tuberculosis. However, point mutation in DNA gyrase, one of the primary targets for quinolone antibiotics, has led to conformational alterations in the binding sites, leading to a change in drug pharmacodynamics. This has contributed to the development of drug-resistant strains such as multidrug-resistant strains (MDR) and an extreme drug-resistant strains (XDR). In this study, our objective was to find the impact of mutation A90V and A90E on DNA Gyrase A of Mycobacterium tuberculosis. Ten quinolone antibioti

Discovery of a tetrazole-thiourea derivative as a potential active agent against multidrug-resistant Staphylococcus aureus and Mycobacterium tuberculosis.

BACKGROUND: Rifampicin-resistant Mycobacterium tuberculosis (RR-TB) and methicillin-resistant Staphylococcus aureus (MRSA) underscore the need for new antibacterial chemotypes active across M. tuberculosis M. tuberculosis and Gram-positive pathogens. Both thiourea and tetrazole are well-established scaffolds in the scientific community for their promising antibacterial properties. The combination of tetrazole and thiourea moieties into a single molecular framework presents a promising strategy to overcome antimicrobial resistance. METHODS: We designed and synthesized a series of tetrazole–thio

A Comprehensive Review of Genetic Mechanisms of Mycobacterium tuberculosis Resistance to Injectable Agents, Second-Line Drugs, and Novel Therapeutics.

Tuberculosis (TB) has been one of the deadliest infectious diseases since ancient times and remains a major threat to public health, causing significant morbidity and mortality. The global TB outbreak in 1993 prompted the World Health Organization to declare the disease an emergency priority. However, public neglect and the ability of M. tuberculosis to develop drug resistance have led to the emergence of multidrug-resistant, extensively drug-resistant, and totally drug-resistant strains. These resistances are mainly caused by genetic mutations in antibiotic target genes, which reduce the effe

Identification of a Novel Nucleic Acid Target for the Rapid and Specific Detection of Mycobacterium Simiae Using Comparative Genomic Analysis.

BACKGROUND: Mycobacterium simiae (M. simiae) is a non-tuberculous mycobacterium (NTM) that closely resembles Mycobacterium tuberculosis (M. tuberculosis) in clinical and biochemical characteristics, notably its niacin-positive phenotype. This similarity frequently leads to misdiagnosis and inappropriate treatment with first-line anti-tuberculosis drugs, to which M. simiae is frequently resistant. Current diagnostic methods are expensive or need complex equipment, highlighting the urgent need for a rapid, specific, and accessible molecular target to identify M. simiae accurately. METHODS: In t

New Advances in the Development and Design of Mycobacterium tuberculosis Vaccines: Construction and Validation of Multi-Epitope Vaccines for Tuberculosis Prevention.

Mycobacterium tuberculosis (Mtb) vaccines are designed to prevent infection, prevent reactivation of latent infection, and/or provide adjuvant therapy to standard TB treatment for active Mtb. Emerging vaccine technologies include reverse vaccinology, DNA and RNA vaccines, subunit vaccines, and multi-epitope vaccines. Currently, many different types of vaccine candidates are in clinical trials, though, to date, BCG remains the only approved Mtb vaccine. Mtb has a complex genome with numerous antigens, but not all are equally effective in eliciting immunity, so a critical challenge is the select

Discovery of highly potent α-keto ester-based peptidomimetic inhibitors of the Hip1 protease for the treatment of Mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb), the bacterium responsible for tuberculosis, is the leading cause of death due to a single infectious agent. Given the alarming increase in drug-resistant cases, therapeutic agents targeting novel Mtb drug targets are urgently needed. Hip1, a serine protease required for Mtb survival in macrophages and tolerance to various antibiotics, has been identified as an attractive therapeutic target. In the current study, we describe the design and synthesis of highly potent (pM range K i) peptidomimetic α-keto ester inhibitors of Hip1. We also report the first two X-ra

AI-Based Analysis of Ziehl-Neelsen-Stained Sputum Smears for Mycobacterium tuberculosis as a Screening Method for Active Tuberculosis.

Tuberculosis is the primary cause of death due to infection in the world. Identification of Mycobacterium tuberculosis in sputum is a diagnostic test, which can be used in screening programs-especially in countries with a high incidence of tuberculosis-to identify and treat those persons with the highest risk of disseminating the infection. We previously developed an algorithm which is able to automatically detect mycobacteria on tissue; in particular, our algorithm identified acid-fast bacilli on tissue with 100% specificity, 95.65% sensitivity, and 98.33% accuracy. We tested this algorithm o

Differential requirement of Formyl Peptide Receptor 1 in macrophages and neutrophils in the host defense against Mycobacterium tuberculosis Infection.

Formyl peptide receptors (FPR), part of the G-protein coupled receptor superfamily, are pivotal in directing phagocyte migration towards chemotactic signals from bacteria and host tissues. Although their roles in acute bacterial infections are well-documented, their involvement in immunity against tuberculosis (TB) remains unexplored. This study investigates the functions of Fpr1 and Fpr2 in defense against Mycobacterium tuberculosis (Mtb), the causative agent of TB. Elevated levels of Fpr1 and Fpr2 were found in the lungs of mice, rabbits and peripheral blood of humans infected with Mtb, sugg

Diagnosis of tuberculous pleural effusion in a tertiary care hospital of central India: The role of xpert Mycobacterium tuberculosis/rifampicin.

BACKGROUND: In India, 15%-20% of tuberculosis (TB) cases are categorized as extra-pulmonary TB, and tuberculous pleural effusion (TPE) is the second-most common type after tuberculous lymphadenitis. However, the paucibacillary nature of TPE makes its diagnosis challenging. As a result, relying on empirical anti-TB treatment (ATT) based on clinical diagnosis becomes necessary for achieving the best possible diagnostic outcome. The study aims to determine the diagnostic utility of Xpert Mycobacterium tuberculosis/rifampicin (MTB/RIF) for the detection of TB in TPE in high incidence setting of Ce
