[A case control study on the treatment of distal radioulnar joint instability combined with distal radius fractures between fixation and non-fixation distal radioulnar joint].

OBJECTIVE: To explore the options on treatment of distal radioulnar joint instability combined with distal radius fracture. METHODS: From June 2007 to December 2009, 264 patients with unstable distal radius fractures were treated with open reduction and internal fixation, in which 42 patients combined with distal radioulnar instability. Distal radioulnar joints of 20 patients were fixed with Kirschner wire or plaster cast at supinator position, and other distal radioulnar joints of 22 patients were not fixed. Range of motion of wrist joints and grip strength were observed; function of wrist w

Treatment of bicondylar tibia plateau fractures using locked plating versus external fixation.

Bicondylar tibial plateau fractures can be difficult to treat due to the extent of articular cartilage, metaphyseal bone, and soft tissue injury. The purpose of this study was to compare the outcomes of open reduction and locked plating vs fine-wire external fixation of 58 consecutive bicondylar tibial plateau fractures at a level I trauma center. All bicondylar tibial plateau fractures were classified as Schatzker V/VI or AO/OTA type 41C. Twenty-eight patients in one group were treated using a locked plating system, and 30 patients in another group were treated with a hybrid or circular exter

Sub-trochanteric fractures of the femur: results of ORIF at KMCTH.

Sub-trochanteric fractures of the femur are not commonly encountered as compared to trochanteric or neck of femur fractures. Relatively younger persons are injured more and a considerable violence is required for this fracture to occur. These fractures, treated conservatively by methods like traction, splints or casts, are often associated with complications like non-union, mal-union, shortening of the limb etc. Thus, we undertook this study of 10 consecutive cases of sub-trochanteric fractures of the femur treated by open reduction and internal fixation and bone grafting in Kathmandu Medical

Epidemiological features of supracondylar fractures of the humerus in Chinese children.

We studied 450 children with a supracondylar fracture of the humerus in the period 1991 to 1995, and were able to collect full management details in 403 of them (253 boys and 150 girls). The median age at presentation was 6 years (6.6 years in boys, and 5 years in girls), with the nondominant humerus 1.5 times more commonly injured. Fifteen percent of children presented more than 1 day after the injury. Garland type III fractures constituted 45% of cases, type I 30%, and type II 24%, with flexion type fractures present only in 1% of the children. A nerve injury was associated with the fracture

Calcaneal fractures: operative versus nonoperative treatment.

BACKGROUND: The purpose of this report is to compare the late results after operative and nonoperative treatment of intra-articular calcaneal fractures. METHODS: From 1982 to 1992 we treated 250 calcaneal fractures of which 193 were intra-articular. Because of a change in operative indication, most fractures were treated nonoperatively before 1989 and operatively thereafter. Follow-up was possible at an average of 5.1 years for 124 intra-articular calcaneal fractures. Out of these, two groups of 33 nonoperatively and 30 operatively treated cases were formed matching each other with regard to

Circular External Fixation in the Treatment of Complex Tibial Plateau Fractures.

Complex tibial plateau fractures continue to challenge orthopaedic surgeons because of the difficulty in obtaining an anatomic reduction of the joint, axial alignment of the articular segment, while avoiding complications. Despite overall good results, significant complications persist with internal fixation, particularly in cases with compromised soft tissues, open fractures, bone loss, extensive comminution, compartment syndrome, extensor mechanism disruption, or significant osteopenia. Circular external fixation offers advantages in these situations because of its unique biomechanics, adapt

Arthroscopic-assisted treatment of a reversed hill-sachs lesion: description of a new technique using cerament.

Purpose. Impaction fractures of the anterior aspect of the humeral head, the reversed Hill-Sachs lesion, are common in posterior shoulder dislocation. We present a new technique to address these lesions arthroscopic-assisted with the use of a bone substitute. Methods. We report the case of a 45-year-old male with a reversed Hill-Sachs lesion after posterior shoulder dislocation. Initially a glenohumeral arthroscopy is performed to address concomitant intra-articular injuries. Guided by the k-wire a cannulated sizer was inserted for reduction of the fracture under arthroscopic visualization. Fo

Traumatic injuries of the paranasal sinuses.

High-resolution imaging, based on CT, has become the expected standard of imaging in severe sinofacial trauma. Imaging must include both bone and soft-tissue detail. Intracranial complications such as hematoma, contusion, and dural tear must be noted and followed appropriately. Early orbital assessment must be included to allow surgical decompression of hematoma or fracture reduction before irreversible changes to the visual pathway occur. Clinical assessment and initial, limited plain films offer an invaluable overview to set up priorities in resuscitation and subsequent direction for more de

Rare Presentation of Ulnar Nerve Palsy in Closed Both Bone Forearm Fracture in Pediatric Population.

INTRODUCTION: Ulnar nerve injury in closed both bone forearm fracture is rare. Most nerve injuries are neuropraxia and rarely the nerve is trapped or is transected. Most of the time recovery is spontaneous but sometimes requires surgical exploration. We are reporting a case of a 14-year-old boy with closed both bone forearm fracture with ulnar nerve palsy due to entrapment and laceration between ulnar bone fracture fragment. CASE REPORT: A 14-year-old boy presented in emergency department elsewhere with a left forearm closed injury due to fall while playing where he was diagnosed with both bo

Functional Outcomes of Surgical Intervention for Tibial Plateau Fractures: A Study From a Tertiary Care Facility.

Background Among the most prevalent types of intra-articular fractures, tibial plateau fractures (TPFs) are common. These fractures can involve medial, lateral, or both plateaus and occur with varying degrees of articular depressions and displacements. The objective of this study was to examine the surgical treatment of TPFs to achieve a pain-free and flexible knee joint, avert the onset of osteoarthritis, and link radiological results with fracture type and functional outcomes. Methodology In this prospective, interventional study, 68 patients with TPFs were classified according to Schatzker'

Results of the Use of Bioabsorbable Magnesium Screws for Surgical Treatment of Mason Type II Radial Head Fractures.

BACKGROUND: In Mason classification type II radial head fractures, compared to plate fixation, fixation with cannulated headless screws and absorbable pins has been reported to provide more favorable postoperative outcomes, including less postoperative limitation in range of motion. The fact that radial head fractures are less prone to weight-bearing during fracture union further supports the use of absorbable screws as a suitable alternative treatment option in radial head fractures. This study aimed to perform fixation through open reduction using bioabsorbable magnesium screws for Mason typ

Facial Fractures and Mixed Dentition - What Are the Implications of Dentition Status in Pediatric Facial Fracture Management?

INTRODUCTION: The stage of maturation of the pediatric facial skeleton at the time of injury has a significant impact on both facial fracture patterns and management strategies. For instance, the relative prominence of the pediatric cranium during the early years of life affords protection to the structures of the midface, whereas delayed aeration of the frontal sinuses may predispose younger patients to frontal bone fractures. The dentition status of a pediatric patient may have similar implications in the setting of facial fracture. In this study, the authors examine the effect of dentition

Transient Radial-Dominant Neuropathy Following the Use of Sterile Exsanguination Tourniquet During the Posterior Plating of a Humeral Shaft Fracture: A Case Report.

Tourniquet-related nerve injury is an uncommon but clinically important complication of extremity surgery. Postoperative radial nerve palsy after humeral shaft fixation is usually attributed to fracture-related injury or surgical manipulation, particularly when a posterior approach is used. We present a case of transient radial-dominant neuropathy following the use of a sterile exsanguination tourniquet during the posterior plating of a distal humeral shaft fracture. A 27-year-old male construction worker with an AO/OTA (Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Associatio

Clinical Efficacy of Combined Internal and External Fixation with Nanosilver Dressing and Vacuum Closure for Gustilo Type I/II Open Tibial Fractures.

Gustilo type I and II open tibial fractures (OTFs) are associated with significant soft tissue injury and a high risk of infection, often resulting in prolonged healing and postoperative complications when treated using conventional internal or external fixation. This study introduces a clinically validated approach that integrates nanosilver antibacterial dressing (NSAD) and vacuum-assisted closure (VAC) with closed reduction and intramedullary nail fixation (CRINF) to improve wound and fracture healing outcomes. A total of 300 patients with Gustilo type I/II OTFs were enrolled and randomly a

Effect of compression on mandibular fracture haematoma-derived cells.

Mechanical stress induces a variety of biochemical and morphological reactions in bone cell biology. This study aimed to investigate appropriate pressures of osteogenesis on the biological responses of 3-dimensional cultured human mandibular fracture haematoma-derived cells by compressive loading. Six patients with mandibular fractures who underwent open reduction and internal fixation were included in the study. During the operation, fracture haematomas that formed fibrin clots were manually removed before irrigation. First, pressures were applied to human mandibular fracture haematoma-derive
