Suppression of Melatonin Secretion in Totally Visually Blind People by Ocular Exposure to White Light: Clinical Characteristics.

Late-onset neuromyelitis optica spectrum disorder in AQP4-seropositivepatients in a Chinese population.

BACKGROUND: Increasing rates of AQP4-seropositive neuromyelitis optica spectrum disorder (NMOSD) have been reported in late-onset patients ( … The median age at onset was 57.5 years (range 50-70). There were similar onset frequencies between optic neuritis (ON) and longitudinally extensive transverse myelitis (LETM). Its prognosis is highly variable among affected individu … upper and lower extremity dysfunction were associated with more disability while presentation with optic neuritis had better prognosis. We provide a comprehensive clinical, radiological and virological analysis of four patients with Epstein-Barr virus (EBV) infection of the n … gomyeloradiculitis had residual lower extremity weakness, and one of these patients later developed optic neuritis. The patient with acute encephalomyeloradiculitis had a moderate flaccid paraparesis, and the patient with subacute meningomyeloradiculitis was left with sensory loss in the feet. Compared with neurological disease caused by other herpes viruses, th …

[Current use and possible future applications of the magnetization transfer technique in neuroradiology].

Magnetization transfer (MT) imaging is a special MR technique used for selective suppression of the MR signal of protons bound on macromolecules. The most important applications in neuroradiology are (1) detection of subtle changes in otherwise normal-appearing cerebral white matter, for instance in multiple sclerosis (MS), Wallerian degeneration, and hydrocephalus, (2) differentiation of white matter lesions with high signal on T (2)-weighted MR-images, like MS plaques, brain infarctions, and brain edema, (3) follow-up of cerebral white matter diseases using volumetric MT techniques, and (4)

An MT MRI study of the cervical cord in clinically isolated syndromes suggestive of MS.

Cervical cord magnetization transfer ratio (MTR) histograms were obtained from 45 patients at presentation with clinically isolated syndromes (CIS) suggestive of multiple sclerosis (MS). The mean values of MTR histogram-derived metrics were not different between CIS patients and healthy control subjects or between patients with and without evidence of disease dissemination in time. Only three patients showed significantly lower cord MTR values than control subjects. These findings suggest the absence of intrinsic structural damage of the cervical cord soon after the onset of CIS suggestive of

Predicting secondary progression in relapsing-remitting multiple sclerosis: a Bayesian analysis.

With the aid of a Bayesian statistical model of the natural course of relapsing remitting Multiple Sclerosis (MS), we identify short-term clinical predictors of long-term evolution of the disease, with particular focus on predicting onset of secondary progressive course (failure event) on the basis of patient information available at an early stage of disease. The model specifies the full joint probability distribution for a set of variables including early indicator variables (observed during the early stage of disease), intermediate indicator variables (observed throughout the course of dise

Multiple sclerosis: an immune legacy?

The aetiology of multiple sclerosis suggests that its occurrence depends on a combination of factors, including viral infection in early childhood, genotype and an initiating event within the central nervous system. The resulting damage may be caused by events initiated by free radicals. Free radicals themselves may cause damage to myelin and also may trigger the arachidonic acid cascade, to produce compounds that are thought to initiate and augment T-cell activity. Repair of the damaged tissue is normally achieved by protective enzymes that remove damaged lipid from the myelin.

Transretinal membrane formation in diffuse unilateral subacute neuroretinitis.

BACKGROUND: Diffuse unilateral subacute neuroretinitis (DUSN) is characterized by unilateral visual loss with vitreous inflammation, optic disc swelling, and the presence of gray-white lesions in the deep retina, and can be associated with intraocular nematode infection. To date, no cases of transretinal membrane formation in DUSN have been reported. METHODS: A 22-year-old woman was examined for a 2-week history of unilateral decreased vision and neuroretinitis. A subretinal nematode was identified and a diagnosis of DUSN was made. The nematode was destroyed with laser photocoagulation. An ep

Patients with a history of optic neuritis (ON) had increased (worse) K-D scores. CONCLUSIONS: The K-D …

The Gulf War era multiple sclerosis cohort: age and incidence rates by race, sex and service.

We characterize here a new nationwide incident cohort of multiple sclerosis from the US military-veteran population. This cohort provides an update to the only other US nationwide incidence study of multiple sclerosis performed during the 1970s. Medical records and data from the Department of Defense and Department of Veterans Affairs for cases of multiple sclerosis who served in the military between 1990, the start of the Gulf War era, and 2007 and who were service-connected for this disorder by the Department of Veterans Affairs from 1990 on, were reviewed. A total of 2691 patients were conf

[A retrospective study of patients with early onset of multiple sclerosis in Omskaya Oblast].

To study the features of multiple sclerosis (MS) with child age-at-onset, we examined 254 patients with confirmed diagnosis of MS. The disease developed at the age younger than 16 years in 27 patients. A control group included 303 healthy volunteers. Correlations between the rate of disease progression and the age-at-onset were found. The more benign course of MS was noted in patients with earlier disease onset. There were more women and the disease frequently manifested with retrobulbar nephritis in the group with early onset. The results indicated that a G allele of -308G/A polymorphism of t

Determining abnormal interocular latencies of multifocal visual evoked potentials.

PURPOSE: To describe methods for measuring interocular latency differences of multifocal visual evoked potentials (mfVEP) and for determining regions with abnormal interocular latencies in patients. METHODS: The mfVEPs from 100 individuals with normal visual fields and normal fundus examinations were analyzed. Individuals ranged in age from 21.6 to 92.4 years. The stimulus was a 60 sector, pattern-reversing dartboard display. Each sector had 16 checks, 8 white (200 cd/m2) and 8 black (< 1 cd/m2). Interocular latency was measured as the temporal shift producing the best cross-correlation value

Spatial vision in visually asymptomatic subjects at high risk for multiple sclerosis.

OBJECTIVE: To investigate the state of spatial vision in visually asymptomatic subjects at high risk for multiple sclerosis. METHODS: Fifteen subjects suffering a first neurological episode suggestive of multiple sclerosis in clinical presentation, immunological profile, and magnetic resonance imaging, were examined with a new, sensitive test of spatial vision, rarebit perimetry. None had symptoms or signs of optic neuropathy. RESULTS: Results of rarebit perimetry were significantly worse than those of 15 age matched normal controls (p=0.01); seven patients (47%) were outside normal limits.

Combined paresis and restriction of the extraocular muscles after orbital fracture: a study of 16 patients.

Sixteen patients with combined paresis and restriction of extraocular muscle(s) orbital fracture repair were studied before and after in order to determine the clinical features and management of such patients. All 16 patients showed limited ductions of the involved eye in the field of action of the entrapped, paretic muscle and of the antagonist muscle after orbital fracture. Single extraocular muscles (13 patients) and two extraocular muscles (three patients) were demonstrated adjacent to the fracture site on orbital computed tomography (CT). In three patients prior to orbital surgery, a dev
