Oral avascular bone necrosis associated with chemotherapy and biphosphonate therapy.

BACKGROUND: Oral avascular bone necrosis is an important adverse effect of chemotherapy and biphosphate therapy. OBJECTIVE: To report our experience in oral avascular bone necrosis in cancer patients assigned to undergo chemotherapy. PATIENTS AND METHODS: Fourteen patients presenting oral avascular bone necrosis were selected from the clinical files of five Stomatological Clinics in Brazil. Clinical data as well as treatment and prognosis information were obtained from all 14 patients. RESULTS: Twelve patients (86%) were submitted to biphosphonate therapy. The most important symptom was pai

Avascular necrosis and protease inhibitors.

Avascular necrosis (AVN) indicates ischemic death of the bone due to insufficient arterial blood supply. The incidence rate of AVN is higher in HIV-infected patients than in the general population. Although the exact etiology of AVN remains unclear, the literature has shown a relationship between AVN and exposure to highly active antiretroviral therapy (HAART). It should be noted, however, that AVN has been reported before the era of HAART, thus suggesting the involvement of other causative factors as well. Three case reports based on patients attending the infectious disease clinic are presen

Kienböck's disease with congenital capitohamate coalition in an ulnar ray defect: a case report.

We report a patient who developed Kienböck's disease with congenital capitohamate coalition and absence of the little finger: an ulnar ray abnormality of the hand without hypoplasty or defect of the ulna at the forearm. Kienböck's disease combined with an ulnar ray abnormality is a rare situation and the force distribution is unclear. The etiology of the Kienböck's disease is still obscure, so numerous surgical procedures have developed. Capitohamate fusion is one of them, but the case reported here highlights the fact that capitohamate fusion may be a questionable treatment for Kienböck's dis

Surgical outcome of radial osteotomy for Kienböck's disease-minimum 10 years of follow-up.

PURPOSE: Long-term results of radial osteotomy for Kienböck's disease seldom are seen in the literature. The purpose of this study was to report the minimum 10-year results and to compare them with the 5-year results to determine whether the favorable intermediate-term results were maintained. METHODS: Twenty-five patients who underwent radial osteotomy were followed-up for a mean period of 14.5 years. They were examined for pain, grip strength, and wrist range of motion (ROM). Through a review of clinical records, 5-year postoperative results were collected. The carpal height ratio and Ståhl

Decision making in glenohumeral arthroplasty.

Prosthetic replacement arthroplasty for glenohumeral arthritis is a well-developed and well-described technique with good and excellent results. The surgeon is faced with many decisions to make, however, regarding choice of implant, implant fixation, soft tissue management, and options for glenoid resurfacing. In general, when the precise cause of the arthritic condition is identified, the choices become more straightforward. For advanced osteoarthritis of the shoulder joint in an older patient with asymmetric posterior erosion of the glenoid, a total shoulder arthroplasty renders the best rel

Avascular necrosis after osteotomy of the talar neck to correct residual club-foot deformity in children. A long-term review.

In order to determine the incidence of avascular necrosis after osteotomy of the talar neck, we re-evaluated 11 patients (16 feet) with idiopathic club foot who had undergone this procedure at a mean age of eight years (5 to 13) to correct a residual adduction deformity. All had been initially treated conservatively and operatively. The mean follow-up was 39 years (36 to 41). Surgery consisted of a closing-wedge osteotomy of the talar neck combined, in 14 feet, with lengthening of the first cuneiform and a Steindler procedure. At follow-up eight feet were free from pain, three had occasional m

Preiser's disease: arthroscopic treatment of avascular necrosis of the scaphoid.

We report the case of a 50-year-old female patient with Preiser's disease (avascular necrosis of the scaphoid) who, after a 2-year history of wrist pain, underwent arthroscopic debridement of the necrotic scaphoid using a standard technique. Osteoarthritic changes of the articular cartilage, partial rupture of the scapholunate ligament, local synovitis, and loose fragments were documented. The patient reported significant improvement in pain relief and complete relief of mechanical symptoms at 31-month follow-up examination. Radiographs demonstrated no progression of collapse of the scaphoid o

Involvement of the foot and ankle in patients with Gaucher disease.

Of 48 children with type I Gaucher disease treated at our hospital, 11 had involvement of the foot and ankle that first appeared in adolescence. Follow-up ranged from 3 to 27 years (average, 10 years). We reviewed their clinical and radiographic histories. Patients presented with four types of pain: six (7 events of pain) complained of dull pain defined as nonspecific; seven (11 events) had severe pain caused by bone crisis; two (2 events) had moderate progressive pain caused by pathological fractures; and one had painful swollen ankles caused by degenerative arthritis. Greater awareness of th

[Treatment of avascular necrosis of talus with vascularized bone graft].

In order to study the curative effect of vascularized bone graft in the treatment of avascular necrosis of talus, 24 patients were treated with vascularized bone grafts, in which 9 cases had received 1st cuneiform bone graft with a malleolaris anteriomedialis, 4 cases with the 1st cuneiform bone graft with the medial tarsal artery and 11 cases with vascularized cuboid bone graft with the lateral tarsal artery. All of the patients were followed up for 3-5.5 years. The clinical observation and X-ray examination showed that function of the ankle joint was completely or almost normal in 16 cases,

Characteristics of calcaneal bone infarction: an MR imaging investigation.

OBJECTIVE: Bone infarction (BI) of the calcaneus is an uncommon entity which has received little mention in the recent literature. In this paper, we review the MR images of six calcanei with BI, which demonstrate a pattern of presentation that may explain the etiology of BI at this unusual location. DESIGN: A retrospective review was performed of the transcribed reports of the foot or ankle MR examinations at our institution. MR images of examinations with any marrow signal abnormality were reviewed for presence of BI and its distribution. PATIENTS: Based on MRI criteria, four patients had c

Avascular necrosis secondary to postoperative steroid therapy.

Hypothalamic and pituitary tumours may present with vague symptoms owing to excess or lack of hormone production, including diabetes insipidus. Corticosteroids are commonly employed to limit cerebral oedema and at much lower doses to treat secondary hypocorticalism. Continuation of steroids at inappropriately high doses predisposes to the development of avascular necrosis as in the case we describe in a young woman of 34 years. This is a potentially preventable crippling disorder. When prescribing steroids the lowest effective dose should be used.

Iatrogenic deformities of the first ray.

Surgical correction of a hallux valgus deformity is common among podiatric and orthopedic surgeons. Complications following hallux valgus surgery are not uncommon. Although many of these complications can be avoided by a comprehensive understanding of the pathomechanics of a hallux valgus deformity and continual honing o surgical skills, some are inevitable. This article provides a comprehensive overview and insight into the diagnosis and treatment of the more common complications encountered with hallux valgus surgery.

[A prospective study of early diagnosis of lunate necrosis by means of MRI].

AIM: Early recognition of lunate necrosis by means of MRI. METHODS: 49 patients with typical symptoms and normal radiographs were examined. Coronary T1 and T2 weighted images were obtained. In addition, an attempt was made to quantify fluid shift and measure of bone marrow oedema by means of extreme T2 weighting (TR 5950 ms, TE 150 ms). RESULTS: Three instances of necrosis were found; in two cases it was possible to demonstrate marrow oedema. Oedema preceding necrosis could not be demonstrated by quantifying a grey scale. CONCLUSIONS: The pattern of pain in the region of the lunate is not t

[Osteo-articular complications of heart transplantation].

Heart transplantation is an effective means of treating irreversible heart failure in selected patients. Preventing organ rejection requires immunosuppressor treatment with corticosteroids, azathioprine and/or cyclosporine. Bone and joint complications are frequent and increase overall morbidity directly related to anti-rejection therapy. Corticosteroids favour osteopenia which can be detected by measurement of bone density. The risks include spontaneous wedge fractures of the spine and aseptic necrosis. The frequency of complications has been reduced with the use of cyclosporine allowing a re

Severe osteoarthrosis after fracture of the mandibular condyle: a clinical and histologic study of seven patients.

The factors that induce complications subsequent to fractures of the mandibular condyle have long been controversial. A clinical and histologic study of seven patients (nine temporomandibular joints [TMJs]) who suffered from severe TMJ problems subsequent to condylar fractures showed that the complications were related to comminution of the condyle, sagittal fracture of the condyle, and subcondylar fracture dislocation. Alterations in these TMJs all involved secondary osteoarthrosis. Additionally, aseptic necrosis, bifid deformity of the condyle (two of the three sagittally fractured condyles)

Magnetic resonance imaging of the wrist.

Until recently magnetic resonance imaging of the musculoskeletal system tended to be confined to the larger joints, such as the knee and shoulder. Advances in magnetic resonance technology, especially in the area of improved software and development of surface coils, have made feasible the imaging of smaller joints, such as the wrist. This article reviews the potential applications of magnetic resonance in evaluating disorders of the wrist.
