[Clinical Utility of Tapentadol].

Tapentadol(TP)is a new strong opioid analgesicthat has both m-opioid receptor(MOR)effects and norepinephrine reuptake inhibitor(NRI)effects. In comparison with the existing strong opioid analgesics, the mechanism of action suitable for palliation of neuropathic pain is expected to be better for TP. The analgesic effect and side effects of this drug were tested in 10 cases of exacerbation of neuropathic pain at our hospital, and the sedative response rate was 70%. The main side effects were somnolence 44.4%, nausea 33.4%, and constipation 11.1%. The side effects on the digestive system were con

Acute and long-term VNS effects on pain perception in a case of treatment-resistant depression.

Vagus Nerve Stimulation (VNS) is approved by the FDA for treatment of both epilepsy and depression. Recent work has shown that VNS acutely affects pain perception in humans, actually increasing pain sensitivity momentarily while the device is firing. It is unclear how this acutely increased sensitivity might change over time with treatment and how it might relate to longer-term therapeutic effects of VNS on pain. We describe a patient with treatment-resistant depression and a history of severe lumbar degenerative disease with resultant chronic low back pain. His depression and pain symptoms bo

Clinical holistic medicine: chronic pain in the locomotor system.

Most pains from the locomotor system arise due to involuntary, chronic tensions in the muscles or other tissues. When the patient is motivated, the pain is easily cured in most of the cases by using the tools of consciousness-based medicine, primarily therapeutic touch, conversation, and coaching the patient in a positive philosophy of life. The pains are often caused by "blockages" that may cause problems other than just pain. Often it turns out that the blocked areas develop actual physical damage over time: a slipped disk in the back, articular degeneration, or osteoarthritis when the carti

Post-stroke pain case study: clinical characteristics, therapeutic options and long-term follow-up.

Central post-stroke pain (CPSP) is a syndrome characterized by sensory disturbances and neuropathic pain. In 40%-60% of CPSP patients, the onset of central pain following a stroke occurs more than 1 month after the stroke, creating a source of diagnostic uncertainty or significant delay in treatment since healthcare providers familiar with CPSP may no longer be caring for the patient when the symptoms occur. In addition to chronic pain, the presence of somatosensory abnormalities is the most important diagnostic corollary of CPSP. Neuropathic or central pain has been estimated to occur in up t

Pain status of patients with severe haemophilic arthropathy.

Patients with severe haemophilia A growing up before the establishment of prophylactic treatment frequently developed significant haemarthropathies. The goal of the following study was to clarify the role of haemarthropathic pain for haemophilic patients. Furthermore, we aimed to determine to what degree daily activities are influenced by the impairment and which therapeutic modalities are used in pain management. Using a questionnaire we consulted 71 haemophiliacs concerning their complaints and how they were treated in 1999 (average age 43 years; range 21-63 years). The pain in the large joi…

Treatment of chronic pain by epidural spinal cord stimulation: a 10-year experience.

Epidural spinal cord stimulation by means of chronically implanted electrodes was carried out on 121 patients with pain of varied benign organic etiology. In 116 patients, the pain was confined to the back and lower extremities and, of these, 56 exhibited the failed-back syndrome. Most patients were referred by a pain management service because of failure of conventional pain treatment modalities. Electrodes were implanted at varying sites, dictated by the location of pain. A total of 140 epidural implants were used: 76 unipolar, 46 Resume electrodes, 12 bipolar, and six quadripolar. Patients …

Chiropractic distractive decompression in the treatment of pelvic pain and organic dysfunction in patients with evidence of lower sacral nerve root compression.

Chiropractic theory postulates that organic dysfunction could be the result of neurological disorganization secondary to mechanical disorders of the spine. Few studies have documented the efficacy of chiropractic manipulative therapy in treating mechanically induced organic dysfunction. Lower sacral nerve root compression (LSNRC) as the result of lumbar disc lesion has been identified as a cause of pelvic pain and organic dysfunction (PPOD). Ten cases of PPOD with accompanying evidence of LSNRC in patients with low back pain as a result of a clinically established lumbar disc lesion are presen

Pain in the neck, face, and head. Role of the consultation-liaison psychiatrist.

The special nature of pain in the face, head, and neck is not emphasized in the psychiatric literature on chronic pain. Although chronic pain of all types and locations share many features the psychological and symbolic significance of the head in the development of self-esteem, body image, and interpersonal relationships often confers special characteristics of pain on this area. As psychiatric consultation is not likely to be requested for patients with head, face, and neck pain in the absence of blatant "psychiatric" problems, it behooves the psychiatrist to exercise his liaison functions t

The clinical concept of chronic pain.

In this article, the authors present a rationale for considering pain to be a major health problem in the United States, based on epidemiologic data. They then define the nature of pain disorders in terms of their acute and chronic forms. By extending the definition of pain into a psychogenic/organic dichotomy, the authors introduce the concept of personality variables and psychodynamic considerations that have been attributed to patients with chronic pain. Finally, the overlap and evidence for depression as a coexisting variable with chronic pain is considered. Throughout the article, the aut

Longitudinal Outcome Evaluations of Interdisciplinary Multimodal Pain Treatment Programs for Patients With Chronic Primary Musculoskeletal Pain: A Living Systematic Review.

BACKGROUND AND OBJECTIVES: Interdisciplinary Multimodal Pain Treatment (IMPT), grounded in the biopsychosocial model, is strongly recommended for patients with chronic primary pain. A base review found that IMPT participation leads to sustained improvements in wellbeing, but substantial heterogeneity in program dose and content limited conclusions about optimal design. This first update as a living systematic review (LSR) aimed to assess the long-term effects (≥ 12 months post-treatment) of IMPT for chronic primary musculoskeletal pain and explore patterns in treatment effectiveness to inform

A nurse-led transitional pain service: opioid tapering and early postoperative screening for neuropathic pain characteristics - an observational cohort study.

OBJECTIVES: Fast-track and outpatient surgery have significantly reduced postoperative hospital stays across many surgical specialties. As a result, patients are increasingly discharged with strong opioid prescriptions, contributing to the global opioid crisis. Careful follow-up and opioid tapering are essential. While multidisciplinary Transitional Pain Services (TPS), involving pain specialists, psychologists, and physiotherapists, have shown promise, their widespread implementation is limited by costs and complexity. To address these barriers, we implemented a nurse-led TPS, supervised by a

A Multicentre Cluster Randomised Stepped Wedge Trial of Opioid Sparing Analgesia in Emergency General Surgery.

BACKGROUND: Emergency general surgery (EGS) is associated with high postoperative pain burden and historically relies on opioid-centric analgesia, which can delay recovery and increase opioid-related adverse events and longer-term exposure. Evidence for scalable, system-level opioid-sparing pathways in pragmatic emergency surgical settings remains limited, particularly in Asian tertiary health systems. METHODS: We conducted a pragmatic, multicentre and cluster-randomised stepped-wedge trial in 12 Chinese tertiary hospitals comparing usual care with a structured multimodal opioid-sparing perio

Bilateral Intercostal Cryoanalgesia After Sternotomy for Coronary Artery Bypass Grafting.

BACKGROUND: Effective opioid-sparing analgesia after median sternotomy remains an unmet need in cardiac surgery. We evaluated whether bilateral intercostal cryoanalgesia, added to standard multimodal analgesia, was associated with postoperative pain, opioid consumption, recovery and early hospital-resource use after isolated coronary artery bypass grafting (CABG). METHODS: This single-centre, non-randomised comparative service evaluation included 60 patients undergoing isolated CABG through median sternotomy between 1 November 2025 and 30 May 2026 (30 cryoanalgesia; 30 standard care). Postoperative Effects of Celecoxib on Opioid Use and Pain Control After Benign Oropharyngeal Surgery.

OBJECTIVES: To establish how celecoxib use after tonsillectomy and adenoidectomy (T&A) and modified uvulopalatopharyngoplasty (UPPP) affect pain scores and opioid consumption. METHODS: Patients undergoing T&A or UPPP with tonsillectomy (May-October 2024) completed surveys on postoperative days (POD) 0, 1, 5, and 10 to assess pain scores and opioid use. All patients received a multimodal pain regimen, and cohorts were based on the prescription of celecoxib. The noncelecoxib cohort was a historical control group obtained via study data from Butkus et al. (December 2020-January 2023). Wilcoxon r

Perioperative Management of Patients on Buprenorphine.

PURPOSE OF THE REVIEW: Our review discusses the pharmacologic profile of buprenorphine, then dives into the current literature regarding buprenorphine's use to treat chronic pain, opioid use disorder, and acute postoperative pain. We aim to focus on how that literature may influence the perioperative management of patients on buprenorphine and prompt reconsideration of buprenorphine as a frontline opioid analgesic. RECENT FINDINGS: Buprenorphine has been used effectively for both acute and chronic pain management, for the treatment of opioid withdrawal, and for the treatment of opioid use dis…

Relative contribution of real/sham spinal manipulation performance, changes in cortisol levels, and patient expectations and fear behaviors in modulating short-term pain relief in people with neck pain: A secondary analysis of a randomized clinical trial.

BACKGROUND: Spinal manipulation (SM) is a widely practiced, hands-on, non-invasive manual therapy aimed at alleviating musculoskeletal pain and improving functional capacity. Although the clinical effects of this intervention are well known, most reviews conclude that the underlying mechanisms needs further investigation. Therefore, the aim of this study was to analyze the individual contribution of SM execution, expectations, fear behaviors and neuroendocrine changes on short-term pain intensity variance following the interventions. DESIGN: Experimental study. METHODS: A secondary analysis
