The authors describe the use of duplex and/or color Doppler ultrasonography of the pelvis in three women to demonstrate the presence of venous malformations. One patient with a pulsatile vaginal mass was shown to have an arteriovenous malformation of the vaginal wall. The second patient was shown to have an unsuspected venous angioma in the endometrial cavity. The third patient was shown to have adnexal varices that closely mimicked hydrosalpinx. In the latter two cases, the duplex and color flow capabilities of an endovaginal probe were especially important.

Detailed measurements of the relative contributions of spine motion and pelvic motion during flexion-extension and free lifts (squat-type) are examined. The results are consistent with the conclusion that passive stretching of the ligamentous tissues transmits significant extensor moment in these activities, the power being supplied by the hip extensor complex acting on the pelvis. In addition, certain injuries result in measurable changes in the kinematic parameters that determine the ligamentous involvement, and these changes can be used to help evaluate spinal condition.

A 44-year-old woman presented with an acute abdominal syndrome associated with fever. Laparotomy revealed a mesenteric mass; pathology demonstrated a granuloma containing an adult threadworm (Enterobius vermicularis) and its eggs. Ectopic oxyuriasis is rare, and only three cases of peritoneal granuloma distant from the pelvis or the appendix have been reported previously. Our observation is the first case to which a clinical symptomatology could be ascribed. The pathogenesis and the treatment of this particular localization of oxyuriasis are discussed.

The authors substantiate the indications for various extraarticular surgical interventions taking into consideration a great diversity of pathologic changes in the proximal end of the femur and in the acetabulum. Preference is given to combined interventions on the bones of the pelvis and on the femur. The indications for the 5 major versions of the operation for pathologic dislocations and subluxations of the hip are presented. 50 of the 52 operated joints were examined 1 to 8 years later; in 36 cases the results were good, in 10 cases satisfactory and in 4 cases bad.

Co-administration of uracil and N-[4-(5-nitro-2-furyl)-2-thiazolyl]formamide (FANFT) to weanling female Fischer rats produced uracil stones in the bladder and significantly reduced the incidence of bladder tumors. Contrary to bladder tumors, the incidence of renal pelvic and ureteric tumors was increased by this regimen. Feeding of uracil alone produced bladder tumors, in addition to the hyperplasia of renal pelvis, ureter and bladder. The mechanism of uracil's effect on FANFT carcinogenesis is not known.

Magnetic resonance imaging has achieved clinical importance and has become the primary tool in the investigation of head, spinal cord, pelvis, cancellous bone, pericardium (with gating), gallbladder, lymph node masses, and very soon, in the evaluation of joints. In many areas, particularly with respiratory and ECG gating, this method promises to replace all other imaging modalities with the precision of information obtained. Today the exceptions are the alimentary tube and its mesentery, between the esophagus and rectum, and the peripheral lung.

We describe a case of a 75-year-old woman with squamous cell carcinoma in the right renal pelvis accompanied by multiple dysplasia in the upper urinary tract. Though it is difficult to make a preoperative diagnosis of squamous cell carcinoma, lavage cytology is particularly valuable. Renal arteriography and cross-sectional imaging are also useful to assess an accurate stage. We postulate that severe dysplasia could be a precursor of squamous cell carcinoma as well as transitional cell carcinoma.

A case is reported in which an anastomotic leak following the Swenson procedure for Hirschsprung's disease was complicated by the development of a pelvic abscess that communicated freely with the epidural space. The child presented with signs and symptoms of an epidural abscess, but his myelogram was normal. The diagnosis was made by urografin enema. During transurethral electroresection in the posterolateral bladder neck, trigone and posterior urethra, unintentional contractions of the thigh-adductor muscles may occur due to irritation of the obturator nerve. The sudden displacement of the bladder wall against the cutting loop may cause a perforation of the bladder. The authors describe the topographic relation of the bladder wall to the passage of the obturator nerve in the minor pelvis. The technique of obturator nerve block by local anaesthesia is described and its efficacy is demonstrated in 21 patients.

To determine the incidence of renal vein varices, an analysis of 132 consecutive renal venograms was performed. Varices were found in 8 patients (6%), always on the left side. Four were solitary, and four represented a dilated venous network adjacent to the renal pelvis and upper ureter. Only a single patient had hematuria attributed to renal vein varices. It is concluded that varices in association with hematuria cannot be assumed to be causative; they may equally well be incidental findings.

Retroperitoneal hemorrhage with associated intramural hemorrhage into the renal pelvis and proximal ureter is a well documented complication of coagulation defects. The urographic characteristics of this condition have been well documented by several authors [1-4], but to our knowledge the computed tomographic (CT) findings in patients with this condition have not been reported previously. We describe the CT findings in 2 such cases; one related to thrombocytopenia in leukemia, and the other to anticoagulant therapy in a patient with Budd-Chiari syndrome.

Intermittent hydronephrosis may appear as acute abdominal pain. Between episodes of pain, the patient may be asymptomatic, and the intravenous urogram usually will be normal. The condition is diagnosed from intravenous urograms taken either during an episode of pain or after hydronephrosis has been precipitated by hydration. There are many causes of intermittent hydronephrosis; however, the closed renal pelvis and nondistensible ureteropelvic junction are important factors in nearly all cases. Pyeloplasty is the best treatment and is usually curative.

Ultrasonic demonstration of the anatomy and pathology of solid and fluid-filled organs and structures of the upper abdomen, the retroperitoneum and the pelvis is now well established, as is its use for examination of the thyroid gland, breast, the orbit, the heart, the scrotum and soft tissue masses. Aspiration of cytological and histological material for tissue diagnosis and other purposes is performed in a number of centres. Echography and computed tomography of the abdomen are complementary and there is a trend towards combined units so that each service may be used more efficiently.

From the 24 683 autopsies performed at the Institute of Pathology of Zurich University from 1963 to 1973, all cases with hypernephroid carcinoma or carcinoma of the urinary tract were listed. For each type of carcinoma the extent of phenacetin abuse, the sex and the mean age of the patients were established. The positive relationship between phenacetin abuse and carcinoma of the renal pelvis and the urinary bladder confirms the results of other investigators. In addition, a relationship between phenacetin abuse and hypernephroid carcinoma is discussed.

Two cases of malakoplakia of the testis are presented. Malakoplakia (soft plaque) is an uncommon chronic inflammatory condition of granulomatous type, the common denominator of which would appear to be an altered reactivity of macrophages. Its hallmark is the presence of Michaelis-Guttman bodies. Until the late 1950s, when a case involving the testis was described, the condition was thought to involve only the mucosae of the bladder, ureters, and renal pelvis. The most recent review of malakoplakia of the testis included only 10 known cases.

The diagnosis and management of 12 patients with spontaneous non-traumatic urinary extravasation are described. It is important to distinguish extravasation of the fornical backflow type from that owing to frank rupture of the diseased renal pelvis. Psoas syndrome is a rare form of muscle irritation that causes pain and weakness in the lower extremities. A 52-year-old male presented to the emergency department with complaints of having had a seizure and weakness in the left lower extremity. A computerized tomography scan of the abdomen and pelvis revealed reversible thickening of the left psoas muscle, consistent with psoas syndrome. We report a case of psoas syndrome following a generalized seizure and discuss its clinical characteristics and differential diagnoses.

Retroperitoneal leiomyosarcoma (RPLMS) is rare and usually presents as a large abdominal mass with poor clinical symptoms. Radiological findings of an RPLMS arising in the pelvis of a woman resemble those of adnexal tumors. Herein, we present a case of RPLMS mimicking an adnexal tumor which was differentiated from having an ovarian origin as the right ovarian vein was passing through the tumor but there was no direct vascular connection with the tumor. Therefore, it is important to identify the ovarian vein to distinguish between these tumors.

Delayed pressure urticaria (DPU) is a variant of physical urticaria characterised by reproducible whealing on application of sustained pressure to the skin. Clinical manifestations include delayed cutaneous erythema, edema and subcutaneous swelling, typically occurring 4-6h after application of mechanical pressure. Dyspareunia is defined as persistent or recurrent pain in the genital area or within the pelvis that occurs just before, during, or after sexual intercourse. We report an unusual case of DPU manifesting as dyspareunia.

We present a case of aggressive angiomyxoma of the vulva. The patient presented with a persistent, enlarging vulvar mass, initially misdiagnosed as a Bartholin gland cyst. The patient underwent wide local excision, which resulted in total resection of the mass. Final pathology was consistent with aggressive angiomyxoma, a rare soft tissue tumor with a predilection for the female pelvis. Though rare, it is important to consider in the differential diagnosis of a pelvic mass, given the locally aggressive nature of this tumor and propensity for recurrence.
