Reproductive ecology is defined as "the study of causes and mechanisms of the effects of environmental risk factors on reproductive health and the methods of their prevention and management." Major areas of concern, within the purview of this paper, relate to adverse pregnancy outcomes, effects on target tissues in the male and the female, and alterations in the control and regulatory mechanisms of reproductive processes. Teratogenic potential of chemicals, released as a result of accidents and catastrophes, is of critical significance. Congenital Minamata disease is due to transplacental feta

Perinatal morbidity and mortality are still high in cases of intra-uterine growth retardation. Present screening methods select large risk groups and have a low positive predictive value. Instruments which could be used as indicators for those cases within the risk group needing close fetal monitoring would be valuable. The aim of this prospective study was to evaluate fetal ponderal index with respect to signs of fetal distress and neonatal outcome. By means of a risk scoring system, 73 pregnancies with increased risks of intra-uterine growth retardation were compared with 61 controls. By mea

Breast-feeding as a route of HIV-1 transmission during infancy but also as a protective measure against early childhood morbidity has been investigated prospectively in children born to HIV-1-seropositive mothers and control children born to age- and parity-matched HIV-1-seronegative women. The mothers of all study children had been enrolled antenatally at a maternity hospital in Kinshasa, Zaire, which served a relatively affluent group of women who sometimes chose not to breast-feed their infants. In 106 children born to HIV-1-seropositive women, the rate of HIV-1 transmission was 21% in 28 i

The syndrome of haemolysis, elevated liver enzymes and low platelets (HELLP Syndrome) is a consequence of severe preeclampsia/eclampsia. The clinical course is characterized by an unusual presentation with abdominal pain, and manifestations of inadequate haemostasis and excessive bleeding are common. Maternal and perinatal morbidity and mortality are high. We report our experience with 33 patients over a five-year period. The mean gestational age (GA) of the pregnancies was 34 +/- 2.8 wk including 11 patients who delivered 12 neonates of less than 34 wk GA. The most common presenting complaint

This article has provided an overview of the effects of HIV on renal function. Most transmission of the virus occurs by sexual, blood, and perinatal contact. CD-4 positive cells, especially those that are integral components of the immune system, serve as the reservoir for the reproduction of the virus. The resulting effect is destruction of the immune system with eventual multisystem failure and death. Renal complications arise from several factors, notably the compounding effects of chronic dehydration, malnutrition, infection, and use of nephrotic agents. Acute renal complication can be rev

Cocaine was previously regarded as a soft drug causing only mild damage. Its use during pregnancy, however, creates a variety of grave medical problems which necessitate immediate attention not only on the part of internists and psychiatrists but also, and more particularly, by obstetricians and pediatricians. The pregnancy of a cocaine-using woman must be carefully managed and regarded as a high-risk one. This in view of the numerous obstetric risks caused by the drug, notably premature separation of the placenta, increased incidence of stillbirths, congenital malformations, premature births,

The authors studied over a period of four years emergencies in pregnancy. These were seen in patients who were brought in from an outside hospital to a perinatal centre or when transferred from hospitals to a general maternity unit or to a high risk unit for neonatal and maternity care. 382 pregnant women needed to be looked after (293 from outside hospital and 89 transferred from another hospital). Treatment was given especially in the 3rd trimester of the pregnancy to three groups: before labour (60 cases); during labour (211 cases) and; after delivery (30 cases). There were in the same cate

A study of all the mothers in the Northern region in 1983 whose pregnancies ended at between 24 and 31 weeks' gestation was undertaken. Thes … period of gestation at delivery was taken into account. Perinatal mortality and morbidity in infants of diabetic mothers have declined continuously during the past decades, due to advances in prenatal care, obstetric management and intensive neonatal care. This retrospective study reports on complications during pregnancy and fetal outcome in those insulin-dependent-diabetic (IDDM) pregnancies admitted to our institution from 1978 to 1985. Age, weight, prepregnant weight and weight-gain of those patients are compared with normal uncomplicated pregnancies (of the same period). In the diabetic group we noticed a high incidence of abortion (30%) as we…

A prenatal diagnosis of congenital diaphragmatic hernia (CDH) is associated with a poor prognosis, especially if diagnosed early in gestation and associated with polyhydramnios. Although uncommon, it is important to rule out associated anomalies, so that elective termination can be considered. If CDH is an isolated anomaly, the outcome is related to the degree of pulmonary hypoplasia caused by viscera displacing the lung in utero. Unfortunately, our ability to assess lung size, and thus prognosis, is limited. Most babies with CDH either die shortly after birth or survive to lead normal lives,

Questions related to the mechanism of human labor are of central clinical importance nowadays, since the majority of perinatal mortality and morbidity is due to disregulation of uterine contractility mainly premature onset of labor. During delivery of spontaneous or induced onset endogenous prostaglandin F-synthesis increases dramatically and reaches a maximum at the time of placental separation. These increased amounts of prostaglandin F and E lead to myometrial contractions and to a reduction of the cervical resistance. Post partum, prostaglandins lead to the contracture of the myometrium ne…

To assess the risks and benefits of vaginal delivery in patients with prior cesarean section, the English literature on this subject from 1950 to 1980 was reviewed. The following conclusions were reached: 1) Properly conducted vaginal delivery after cesarean section is relatively safe, with a 0.7% incidence of uterine rupture, 0.93 perinatal mortality, and no maternal deaths due to uterine rupture. 2) Of those patients allowed a trial of labor, 66.7% were successfully delivered vaginally. Successful vaginal delivery occurred in 74.2% of those patients with a nonrecurrent indication for their p…

The programming of labor (setting the best possbile time under optimal conditions) was carried out in the University Women's Hospital, Freiburg i. Br. between 1970 and 1975 in 1121 cases. No perinatal mortality was reported. The secondary frequency of cesarean section for this primarily uncomplicated labor was 3.21% (general hospital statistics for this time period: 6.29%). The most important factors for the strict standardized method of induction is the observation of the pelvic score (desired: greater than 6) and a moderate dose of oxytocin or prostaglandin based on the essential amniotomy. …

OBJECTIVE: Congenital heart diseases (CHDs), the most common congenital anomalies, significantly impact pediatric morbidity and mortality. This 15-year retrospective study investigates prenatal features and postnatal outcomes in CHD cases, focusing on factors influencing surgical intervention, mortality, and long-term care.
MATERIALS AND METHODS: We retrospectively reviewed 170 perinatally diagnosed CHD cases at Taichung Veterans General Hospital between January 2008 and October 2023. Diagnosis was primarily based on two-dimensional grayscale and Doppler echocardiography, with speckle trackin

OBJECTIVE: To evaluate prenatal and postnatal outcomes of fetuses with transposition of the great arteries (TGA) and to identify prognostic factors affecting neonatal survival.
METHODS: This retrospective study included fetuses prenatally diagnosed with TGA between January 2015 and December 2024 at a tertiary referral center. Diagnosis was based on fetal echocardiography demonstrating normal atrioventricular connections with ventriculo-arterial discordance and a parallel course of the great arteries. Perinatal management included planned delivery at a tertiary center, prostaglandin E1 infusio
