INDIVIDUAL ARTICLE: Appraisal of Ruxolitinib 1.5% Cream as a First-Line Topical Therapeutic Agent for Adolescents and Adults With Mild-to-Moderate Atopic Dermatitis.

Dermatology has entered the long-awaited paradigm shift from steroidal to non-steroidal therapeutics for the topical treatment of atopic dermatitis. Topical Janus kinase (JAK) inhibitors have garnered a strong recommendation for the treatment of adult atopic dermatitis (AD) by the American Academy of Dermatology in the most recent updated guidelines as of 2023. Ruxolitinib 1.5% cream is the only FDA-approved topical JAK inhibitor available in the US and is approved for the short-term and intermittent chronic treatment of mild-to-moderate AD in adolescents and adults aged >= 12 years with up

Copanlisib in combination with rituximab and bendamustine for transplant-ineligible relapsed/refractory diffuse large B-cell lymphoma patients: Results from the phase II multicentre FIL Copa-BR trial from Fondazione Italiana Linfomi (FIL).

Treatment options for relapsed/refractory (R/R) diffuse large B-cell lymphoma (DLBCL) patients ineligible for autologous stem cell transplant (ASCT) or chimeric antigen receptor (CAR)-T-cell therapy remain limited. The PI3K inhibitor copanlisib has shown activity as a single agent in DLBCL. This phase II, single-arm, multicentre trial evaluated copanlisib with rituximab and bendamustine (copa-BR) in ASCT- and CAR-T-ineligible R/R DLBCL. Patients received six cycles of copa-BR, followed by up to 12 cycles of copanlisib maintenance. The primary end-point was 12-month progression-free survival (P

A Child with Chronic Mucocutaneous Candidiasis Harbors a Novel Gain-of-Function Mutation in STAT1.

OBJECTIVE: Germline heterozygous gain-of-function (GOF) mutations in STAT1 impair IL-17-mediated immunity, resulting in carriers' susceptibility to chronic mucocutaneous candidiasis (CMC). JAK inhibitors have shown therapeutic effectiveness in patients with STAT1-GOF mutations. METHODS: The mutation was detected using whole-exome sequencing (WES) and confirmed by Sanger sequencing. The functional impact of the mutation was verified by luciferase reporter assay. The phosphorylation level of STAT1 in patient cells, the phenotyping of leukocyte subtypes, and serum cytokine levels were determined

Identification of Novel Benzopyridonyl Cyanovinylpyrimidines as Potential Broad-Spectrum Antibacterial Members.

The diversity of chemically synthesized scaffolds provides the channel for discovering antibacterial agents with novel structures or targets to conquer microbial resistance. This work developed a unique type of novel benzopyridonyl cyanovinylpyrimidines (BCPs). Most of BCPs gave high and broad-spectrum antibacterial activity; especially, amino BCP 31f and methylthio BCP 44a exhibited higher bacteriostatic potency (MICs = 0.25-1 μg/mL) against Staphylococcus aureus strains than ciprofloxacin. Low hemolysis, cytotoxicity, drug resistance, good plasma stability, and rapid bactericidal effects rev…

Genotype-phenotype correlations in NTHL1-associated tumour syndrome: case report and literature review.

NTHL1-associated tumour syndrome, formerly known as NTHL1-associated polyposis, is a rare autosomal recessive tumour predisposition in which biallelic carriers of pathogenic NTHL1 variants develop multiple, predominantly adenomatous, polyps and have an increased risk of colorectal cancer and extracolonic tumour manifestations. The Nth like DNA glycosylase 1 (NTHL1) gene encodes for an enzyme involved in the base excision repair pathway which plays an important role in the maintenance of genomic integrity in the cell. Upon identification of our first Swiss biallelic NTHL1 carrier, we performed

Fostamatinib and the risk of acute aortic dissection in immune thrombocytopenia.

Immune thrombocytopenia (ITP) is a rare autoimmune disorder characterized by platelet destruction. While most patients respond to first- or second-line therapies, a small subset is multirefractory. Fostamatinib, an oral spleen tyrosine kinase (SYK) inhibitor, is a therapeutic option in these cases. We report a case of an acute aortic dissection (AAD) occurring in a patient without traditional cardiovascular risk factors, 3 months after fostamatinib initiation. A 70-year-old woman with a 30-year history of ITP, unresponsive to multiple therapies, was treated with fostamatinib and achieved a com

Dual-ligand engineered exosome regulates WNT signaling activation to promote liver repair and regeneration.

WNT signaling is an essential pathway regulating tissue morphogenesis and regeneration. However, harnessing the pathway for regenerative medicine has been challenging due to the lack of approaches to identify and deliver specific WNT ligands to the target tissue. Herein, we reported that WNT and R-spondin (RSPO) proteins could be transported on engineered exosomes and activate the pathway synergistically. We showed that WNT3A and RSPO1 co-treatment could effectively regulate hepatic cell fate and uncovered functional crosstalk with the PPARα signaling pathway. Moreover, dual-ligand-carrying ex

Pharmacokinetic Monitoring of JAK Inhibitor and Tacrolimus for Safe and Effective Management of Graft-Versus-Host Disease After Pediatric Liver Transplantation.

BACKGROUND AND AIM: Though liver transplantation (LT) is effective for pediatric patients with end-stage liver disease, it is associated with complications such as graft-versus-host disease (GVHD). In part due to its rarity and lack of standardized therapy, post-LT GVHD is associated with a high mortality rate. While ruxolitinib (a JAK inhibitor) is approved for use in steroid-refractory GVHD, its safety/efficacy post-LT is undefined. Thus, this case study reports the use of ruxolitinib in a pediatric patient with post-LT GVHD. METHODS: A 5-month-old male underwent a deceased donor left later

Endothelin antagonists for hypertension: has their time finally arrived?

There have been few new treatments introduced for hypertension in the last thirty years. The endothelin (ET) system was discovered in 1988, and in the following years, it was demonstrated that it participated in the elevation of blood pressure and vascular remodeling in experimental models of hypertension, particularly those with more severe forms of hypertension. Several selective and dual antagonists of ETA receptors (ETARs) and ETB receptors (ETBRs) were developed, but none reached marketing for human hypertension. Following a successful trial in resistant hypertension, a novel antihyperten

Mechanisms and Ways to Overcome Acquired Resistance of Cancer Cells to Mcl-1 Antagonists.

Acquired drug resistance reduces the effectiveness of anticancer therapy and leads to cancer progression. Selective inhibition of anti-apoptotic proteins of the Bcl-2 family using BH3-mimetics is a promising treatment strategy for cancer patients. Recently, antagonists of the anti-apoptotic protein Mcl-1 have been actively studied in clinical trials. However, like other BH3-mimetics, they can lose their effectiveness due to the development of acquired resistance. We have found that cancer cells develop resistance to Mcl-1 inhibition through increased gene expression of other anti-apoptotic pro

Vitiligo exacerbation during upadacitinib treatment for atopic dermatitis and improvement following a switch to abrocitinib: a case report.

AIM: Janus kinase (JAK) inhibitors have emerged as targeted therapies for atopic dermatitis (AD), and increasing evidence suggests their potential benefit in vitiligo. While both diseases are considered immunologically distinct, recent insights point to overlapping cytokine pathways that may be modulated by JAK1-selective inhibitors. MATERIALS AND METHODS: We present a case of a 37-year-old male with moderate-to-severe AD and stable vitiligo who developed worsening of vitiligo following treatment with upadacitinib. Although AD symptoms resolved, vitiligo lesions progressed despite phototherap
