Benign and malignant colorectal strictures in ulcerative colitis.

Colorectal strictures, either benign or malignant, are not uncommon in ulcerative colitis. Fifty nine of 1156 ulcerative colitis patients (5%) admitted to this hospital between 1959 and 1983 developed 70 separate colorectal strictures. Seventeen of the 70 strictures (24%) proved to be malignant and the other 53 benign. Nine patients developed more than one stricture. Three principal features distinguished the 17 malignant from the 53 benign strictures in this series: (1) appearance late in the course of ulcerative colitis (61% probability of malignancy in strictures that develop after 20 years

Advanced colorectal carcinoma--5 year survival of 76 cases treated at the Department of Therapeutic Radiology, Singapore General Hospital.

The results of 76 cases of advanced colorectal carcinoma treated at the Department of Therapeutic Radiology, Singapore General Hospital, from 1977 to 1978, are presented. All cases had "Curative" Surgery and were given adjuvant radiotherapy or chemotherapy or both. All were adenocarcinoma and had lymph node involvement as well as tumour infiltration through the bowel wall. 44 cases of rectal carcinoma were given radiotherapy to the pelvis followed by I/V 5-fluorouracil for at least one year. 32 cases of colon cancer were given I/V 5-fluorouracil for at least one year. 20 of these cases had can

Effect of warfarin on formation and growth of pre-neoplastic lesions in chemically induced colorectal cancer in the rat.

Anticoagulant drugs are known to have an effect on tumour growth. However, the mechanisms by which they act are poorly understood, and have therefore been investigated in this study. Wistar rats were given eight weekly subcutaneous injections of azoxymethane, at a dose of 10 mg kg-1 week-1. Following this they were randomized into two groups: a control group, which received no further treatment, and a warfarin treated group, which received warfarin at 'non-therapeutic' doses in their drinking water, for a further 8 weeks. Pairs of rats from each group were killed at 5-weekly intervals from 10

Colorectal carcinoma in patients with Crohn's disease.

Epidemiologic studies have shown a fourfold to 20-fold increased risk of colorectal carcinoma in patients with Crohn's disease as compared with the general population, but management for this risk is controversial. This paper presents a series of 10 patients with Crohn's disease and colorectal carcinoma from one institution and a review of the literature. Compared with 118 consecutive patients having colorectal carcinoma of the usual type at the same institution, the 10 patients were younger (mean age 55 vs. 65 yr, p less than 0.05) and had a higher prevalence of mucinous carcinoma (50% vs. 9%

Phase II study of doxifluridine in advanced colorectal adenocarcinoma.

Doxifluridine, a new fluoropyrimidine derivative, was tested in a cooperative phase II trial by the Swiss Group for Clinical Cancer Research in advanced measurable colorectal cancer. The drug was given in a five consecutive day schedule by a bolus intravenous injection at a dose of 4 g/m2 per day and repeated every three to four weeks. Of 42 eligible patients, 40 had no previous chemotherapy. Response was defined in 27 patients having received two or more courses of doxifluridine. Seven responses (26%) were observed. Responses were seen only in sigmoid and rectum primary tumors. Toxicity consi

[Sphincter function after rectal resection with peranal anastomosis (author's transl)].

To test sphincter function after rectal resection with colon-anal anastomosis performed with Park's peranal technique, 17 patients were examined using the Kelly code and manometric studies. Compliance and anal resting pressure were reduced. Sphincter internal relaxation after rectal distension was also less compared to the norm. Continence was normal for 10 patients and 4 had satisfactory function.

Prognosis and Risk Factors in Patients With Non-radical Circumferential Resection Margin After Rectal Cancer Surgery.

BACKGROUND/AIM: Circumferential resection margin (CRM) has been shown to be a strong predictor of risk of local recurrence (LR) and distant metastases (DM) in patients with rectal cancer treated with total mesorectal excision (TME). However, not all patients with a CRM of zero develop LR or DM. This study aimed to identify factors associated with disease-free survival (DFS) despite non-radical excision (CRM=0 mm). PATIENTS AND METHODS: Patients surgically treated for adenocarcinoma of the rectum between 2005 and 2013 were identified through the Swedish ColoRectal Cancer Registry. RESULTS: Of

A prediction model for disease-free survival following R0/R1 rectal cancer resection: A retrospective longitudinal study based on the Norwegian and Danish colorectal cancer quality registries.

BACKGROUND: Prediction models may assist clinicians in communicating prognostic outcomes to cancer survivors. The Norwegian and Danish colorectal cancer registers (NCCR and DCCG) are valuable data sources for developing such models. Although few models exist for rectal cancer, their development and validation remain limited. METHODS: We assessed data from 21,116 stage I-III rectal cancer patients who underwent R0/R1 surgery. The assessment included clinicopathological predictors of DFS, with the index date being the date of the pathological report. A multivariable Cox regression model with in

Risk Factors for Regrowth After Nonoperative Management for Rectal Cancer.

BACKGROUND: Among patients with rectal cancer who achieve a complete clinical response (cCR) after neoadjuvant therapy and undergo nonoperative management (NOM), a subset experience tumor regrowth and require salvage surgery. We sought to identify clinicopathologic factors associated with tumor regrowth to assist in patient selection for NOM. METHODS: Patients treated for rectal cancer at a single National Cancer Institute (NCI)-designated Comprehensive Cancer Center in whom NOM was pursued based on cCR or near-cCR were identified. Patients were stratified based on whether they developed tumo

Assigning cancer stage to patients using administrative claims data: an example for colon and rectal cancers.

OBJECTIVES: This study sought to design predictive machine learning models to assign stage at diagnosis to colorectal cancer patients using only administrative claims data. The models were subsequently applied to Medicare fee for service (FFS) and commercially insured populations to assess the relationship between healthcare costs and stage at diagnosis. METHODS: FFS patients diagnosed with colon or rectal cancer tumors in 2016 or 2017 were identified in the Surveillance, Epidemiology and End Results (SEER)-Medicare linked dataset. Patients were assigned a cancer stage via multinomial logisti

Colon and rectal peritoneal carcinomatosis: are we mixing apples with oranges? A propensity score-matched analysis.

Rectal cancer is universally considered a different disease entity as compared to colon cancer, except when dealing with colorectal peritoneal carcinomatosis (PC), in which the two cancers are deemed as the same one. The present study aims to investigate the influence of primary tumor location (colon vs. rectum) on oncologic outcomes in patients undergoing cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for colorectal peritoneal metastases. Data from three referral centers undergoing CRS plus HIPEC for PC of colorectal origin were prospectively collected. The

Correlation Between Grade of Clinical Response to Neoadjuvant Therapy for Rectal Cancer and Oncologic Outcomes in the Era of Watch-and-Wait.

BACKGROUND: The watch-and-wait strategy provides an opportunity to pursue nonoperative management in rectal cancer patients with clinical complete response after neoadjuvant therapy. The management of those with near-complete response remains controversial. OBJECTIVE: We assessed the oncologic outcomes of patients managed by watch-and-wait versus total mesorectal excision according to clinical response to neoadjuvant therapy. DESIGN: Retrospective cohort study. SETTINGS: Comprehensive cancer center in New York. PATIENTS: Patients with rectal adenocarcinoma diagnosed between January 2006 an

Transanal total mesorectal excision: short- and long-term results of four certified colorectal cancer centers in Germany.

BACKGROUND: Transanal total mesorectal excision (TaTME), a novel approach for treating low rectal cancer, holds promise. However, concerns exist in certain countries about their oncologic safety due to less-than-optimal outcomes on global studies. This research seeks to evaluate the long-term oncologic outcomes focusing on local recurrence rate and overall survival after TaTME surgery in Germany. PATIENTS AND METHODS: This study analyzed data from patients who underwent elective TaTME surgery between 2014 and 2021 in four certified colorectal cancer centers in Germany. Primary endpoints were

Resection of synchronous liver metastases between radiotherapy and definitive surgery for locally advanced rectal cancer: short-term surgical outcomes, overall survival and recurrence-free survival.

AIM: There is debate as to the correct treatment algorithm sequence for patients with locally advanced rectal cancer with liver metastases. The aim of the study was to assess safety, resectability and survival after a modified 'liver-first' approach. METHOD: This was a retrospective study of patients undergoing preoperative radiotherapy for the primary rectal tumour, followed by liver resection and, finally, resection of the primary tumour. Short-term surgical outcome, overall survival and recurrence-free survival are reported. RESULTS: Between 2009 and 2013, 45 patients underwent liver rese

[Adjuvant Systemic Chemotherapy with S-1/Oxaliplatin or mFOLFOX6 after Curative Resection of Distant Metastases in Patients with Colorectal Cancer].

This study was aimed to assess the feasibility and short-term outcomes of adjuvant systemic chemotherapy with either S-1/oxaliplatin (SOX) or mFOLFOX6 (FOLFOX)after curative resection of distant metastases from colorectal cancer. We retrospectively examined 16 patients who underwent R0 resection of colorectal metastases, including the liver (n=6), lung (n=5), lymph node (n=3), and peritoneum (n=2), followed by chemotherapy with SOX (n=7) or FOLFOX (n=9) until disease progression. The mean recurrence-free survival was 13.2 months in the SOX group and 16.9 months in the FOLFOX group. The mean ov
