PURPOSE: To describe a patient with severe ischemic retinopathy and maculopathy in the context of neurofibromatosis Type 1.
METHODS: Case report illustrated with multimodal clinical imaging. A 16-year-old female with neurofibromatosis Type 1 presented with visual deterioration over several weeks. Her right eye was amblyopic because of an optic nerve glioma treated in childhood with chemotherapy.
RESULTS: Corrected visual acuities were counting fingers in the right eye and 20/150 in the left eye. Examination revealed bilateral optic disk pallor. In the left eye, severe and extensive ischemic

Libman-Sacks endocarditis (LSE) is the most characteristic cardiac manifestation of systemic lupus erythematosus (SLE). It is usually clinically silent but heart failure due to valvular dysfunction, secondary infective endocarditis and embolic phenomena can complicate valvular abnormalities. We present a patient with SLE and blindness due to right central retinal artery occlusion. Echocardiographic examination revealed a verrucous vegetation on the posterior mitral valve leaflet consistent with LSE. Anticoagulation therapy was started. Echocardiographic regression of the vegetation was observe

Severe vaso-occlusive retinopathy as the initial manifestation of systemic lupus erythematosus (SLE) is rare. We report a 16-year-old female who developed bilateral visual impairment. Fundus examinations showed bilateral "cherry-red spot" appearance, multiple confluent cotton wool spots, and widespread arterial occlusion. Laboratory studies showed leukopenia, antinuclear antibody (+), and anti-double-stranded DNA antibody (+). Malar rashes, oral ulcers, and bilateral knee joint tenderness were noted during physical examination. SLE was diagnosed and pulse therapy started immediately. Best corr

The clinical features of a proliferative vitreoretinopathy (PVR) are systematically described. The patient's history reveals moderate lost of vision and metamorphopsia. Ophthalmoscopy, biomicroscopy of the fundus with the contact lens, and black-and-white photography usually lead to the correct diagnosis. Early stages of PVR are characterized by increased reflectance and a cellophane appearance of the inner retinal surface and tortuosity of both small and larger vessels. Advanced epiretinal tissue formation causes development of surface wrinkling and single or multifocal star-folds. In cases o

A 67-year-old man developed a central retinal artery occlusion and was treated with an oral carbonic anhydrase inhibitor, oral glycerin, carbachol rebreathing, and anterior chamber paracentesis. He subsequently developed an anterior segment endophthalmitis. Vigorous medical and surgical management cured the endophthalmitis, but his visual acuity remained at light perception. To the best of our knowledge, this patient represents the first reported case of a culture-positive, anterior segment endophthalmitis following anterior chamber paracentesis.

The Authors studied the checkerboard pattern electroretinogram (PERG) behaviour in normal subjects and in patients suffering from traumatic lesion of optic nerve, demyelinating optic neuritis, axonal optic neuritis, retinal vascular disorders and retinitis pigmentosa in order to verify the PERG origin. According to our data the PERG appears to originate from ganglion cells since the signal diminished or disappeared in axonal lesion of optic nerve, as a result of retrograde ganglion cell degeneration. Furthermore, the demyelinating lesions of optic nerve were not able to modify the PERG wave. I

Two distantly related, two- to three-year-old male Akita dogs developed partial or complete night blindness which progressed to day blindness. Clinical features were attenuated retinal blood vessels, diffuse exaggerated tapetal reflex, and hyperreflective horizontal lines in the tapetal fundus. The principal histologic change was regional photoreceptor cell degeneration. Areas of moderately atrophic sensory retina alternated, concentrically to the papilla, with areas of severely atrophic retina. Ultrastructurally, there was disorganization of photoreceptor outer segments, retinal gliosis, foca

Patients with carotid artery disease may present with monocular, ipsilateral ocular symptoms, and signs that can herald a devastating stroke. Asymptomatic retinal emboli, transient monocular visual loss, and central retinal artery occlusion are the most common. Venous stasis retinopathy and ocular ischemic syndrome are associated with severe hypoperfusion of the eye and usually reflect severe carotid occlusive disease. Embolic ischemic optic neuropathies and compression of the intracranial optic nerve by supraclinoid carotid arteries remain debated. Third-order Horner's syndrome and eye pain a

Ninety-four cases of a hereditary retinal degeneration in household Abyssinian cats were found in Sweden, mainly during a 3-year period. The disease was investigated by ophthalmoscopy, fluorescein angiography, electroretinography, and light microscopy. A bilateral retinopathy was usually first seen in affected cats at the age of 1.5-2 years. Fluorescein angiography did not demonstrate abnormalities of etiological significance to the disease process. A reduction mainly of a- and b-wave amplitudes in the ERG indicated a generalized photoreceptor disease. Light microscopy showed that the photorec

Optic atrophy can often be a result of arterial blood flow insufficiency associated with systemic vascular disease (cardiovascular disease, hypertension, or diabetes mellitus). The lack of adequate blood perfusion pressure can create conditions leading to anoxia and death of the nerve fiber layer with a resultant visual field defect. A case of a 63-year-old white male is presented with optic atrophy resulting from anterior ischemic optic neuropathy 5 years earlier. A review of the literature concerning the more common causes of ocular vascular insufficiency (i.e., anterior ischemic optic neuro

The authors describes a case of Menkes' disease, sex-linked recessive hereditary disease characterized by early progressive psychomotor deterioration, failure to gain weight, seizures, hypothermie and characteristic Kinky hair (Pilitorti). This boy is a blind child, unable to fix and follow light. Fundus examination shows optic nerve atrophy with tortuosity of the retinal blood vessels. The case is discussed in the light of similar and others reports in the literature. The disease is believed to be cause by a generalized copper deficiency in the body by defect in copper intestinal absorbtion.
