AIMS/HYPOTHESIS: Vascular endothelial growth factor (VEGF) plays a pivotal role in the pathogenesis of diabetic retinopathy. We investigated whether transgenic mice with moderate VEGF expression in photoreceptors (trVEGF029) developed changes similar to diabetic retinopathy and whether retinopathy progressed with time.

A 60-year-old man with diabetes mellitus had a sudden decrease in vision in his right eye 3 weeks after confirmed West Nile virus infection. Visual acuity in the right eye was 20/400. PURPOSE: To describe the hemodynamic effect of oral acetazolamide administration on ocular perfusion in a patient with pseudotumor cerebri associated with Crohn disease.
DESIGN: Interventional case report.
METHODS: A 20-year-old woman with a 5-year history of Crohn disease presented with a 2-week history of headache and blurred vision in both eyes. Ophthalmologic examination was normal. Fluorescein angiography showed a profound delay in retinal and choroidal perfusion. Lumbar puncture showed an opening pressure of 320 mm water. Therapy was initiated with oral acetazolamide 750 mg per day.

Visual evoked potentials were recorded in six normal subjects and in three patients with primary open-angle glaucoma, while intraocular pressure was artificially elevated in stepwise increases. The resulting perfusion-pressure amplitude curves in our controls showed a kink, which can be interpreted as a sign of vascular autoregulation. No kink was observed in the curves of our glaucoma patients, which we interpret as indicating a lack of vascular autoregulation in primary open-angle glaucoma.

The significance of prostaglandins and other eicosanoids in normal ocular physiology and in ocular disease is not well understood. We have utilized cultured human trabecular cells and an in vivo model of laser-induced inflammation in the rabbit eye to gain information concerning eicosanoid biosynthesis and its regulation in the anterior segment. Further investigations using these experimental paradigms should help to elucidate the physiologic and pathophysiologic roles of eicosanoids in the eye.

Ischemic optic neuropathy developed in five eyes of four patients with optic nerve drusen. Two of the patients were in their 20s, a decade when idiopathic ischemic optic neuropathy is rare. This argues against a chance concurrence of drusen and idiopathic ischemic optic neuropathy. Infarction of the distal portion of the optic nerve in patients with drusen may result from mechanical distortion of blood vessels in the laminar and prelaminar regions.

Two patients with malignant histiocytosis were found to have capillary occlusion by aggregates of neoplastic histiocytes, in skeletal muscle in one, and in renal glomeruli in the other. One patient had clinical evidence of similar occlusions in the arterioles and capillaries of the ocular fundi. Occlusion of small vessels by tumour cells may explain the confusion of both patients.

PURPOSE: To investigate the one-year outcomes of pneumatic displacement for submacular hemorrhage (SMH) secondary to neovascular age-related macular degeneration (nAMD). We compared cases of polypoidal choroidal vasculopathy (PCV) and non-PCV.
