The free scapular flap.

We present our early experience with a flap that should become important to the microsurgeon. The scapular flap is based on the circumflex scapular branch of the subscapular artery and is a versatile, hardy, easily dissected flap. We have used it in 14 cases where a fairly thin flap was indicated. Four of the 14 patients developed complications. Two developed hematomas after removal of the drains. These were evacuated without any loss of the flap. One patient had to be returned to the operating room because of thrombosis of the venous anastomoses, but the flap eventually survived in its entire

The upper-extremity amputee. Early and immediate post-surgical prosthetic fitting.

The results of immediate and early post-surgical prosthetic fitting in eighty-seven upper-extremity amputees as well as the results in nine patients with shoulder dislocation who were fitted with temporary devices were reviewed. No local wound complications occurred and the rate of prosthetic acceptance was high. A practice prosthesis, with a filler insert formed from liquid Silastic foam allowed to set between the walls of the practice prosthesis and the amputation stump, was used extensively in this series. With the Silastic insert and practice prosthesis, prosthetic training could be instit

Recurrent anterior dislocation of the shoulder. Long-term follow-up of the Putti-Platt and Bankart procedures.

Follow-up of 176 patients among 223 who underwent 232 repairs for recurrent shoulder dislocation (average follow-up, 10.2 years) revealed a recurrence rate of 11%. Among the twenty with recurrence, important factors were youth, athletic activity, inadequate immobilization, a history of contralateral dislocation, and a family history of shoulder dislocation. Redislocation occurred two years or more after operation in seven of the twenty patients with recurrence. Surgical repair permitted good function without significant pain in most cases.

Comparison of Maternal Labor-Related Complications and Neonatal Outcomes Following Elective Induction of Labor at 39 Weeks of Gestation vs Expectant Management: A Systematic Review and Meta-analysis.

IMPORTANCE: Elective induction of labor at 39 weeks of gestation is common. Thus, there is a need to assess maternal labor-related complications and neonatal outcomes associated with elective induction of labor. OBJECTIVE: To examine maternal labor-related complications and neonatal outcomes following elective induction of labor at 39 weeks compared with expectant management. DATA SOURCES: A systematic review of the literature was conducted using the MEDLINE (Ovid), Embase (Ovid), Cochrane Central Library, World Health Organization, and ClinicalTrials.gov databases and registries to search f

The role of patients' overall expectations of health on outcomes following proximal humerus fracture repair.

INTRODUCTION: The purpose of this study is to evaluate the relationship between patients' own health expectations and treatment outcomes following surgical repair of proximal humerus fractures. HYPOTHESIS: Patients' health expectations will correlate with treatment outcomes following surgical repair of proximal humerus fractures. MATERIAL AND METHODS: Over a 14-year period, 247 patients with a displaced proximal humerus fracture who underwent ORIF with locking compression plates were prospectively followed at one academic institution. Minimum follow-up period was 12 months. Patient-reported

A 4-Year Analysis of the Incidence of Injuries Among CrossFit-Trained Participants.

BACKGROUND: High-intensity functional training (HIFT) is a new training modality that merges high-intensity exercise with functional (multijoint) movements. Even though others exist, CrossFit training has emerged as the most common form of HIFT. Recently, several reports have linked CrossFit training to severe injuries and/or life-threatening conditions, such as rhabdomyolysis. Empirical evidence regarding the safety of this training modality is currently limited. PURPOSE: To examine the incidence of injuries related to CrossFit participation and to estimate the rate of injuries in a large cr

Wrestling and sports medicine.

The coverage of wrestling events from the perspective of medical personnel is reviewed here. Considerations are made regarding the role of medical personnel, the supplies that are important for wrestling event coverage, and the injuries that are frequently encountered in wrestling. Attention is given to treatment of injuries and conditions that are largely specific to wrestling.

Clinical and structural results of arthroscopic repair of isolated subscapularis tear.

BACKGROUND: Even though the frequency of arthroscopic repair of isolated lesions of the subscapularis tendon has increased, few studies have presented clinical and anatomical outcomes of this treatment. We hypothesized that, after an arthroscopic repair, structural outcomes in the muscle have an influence on functional results. METHODS: A retrospective study was performed on twenty-two patients who had undergone arthroscopic repair of an isolated tear of the subscapularis tendon and had a mean follow-up of thirty-six months. Patients were evaluated preoperatively and postoperatively with use

Relationship between humeral head translation, spur formations, and the locations of tendon tears in massive rotator cuff tears.

OBJECTIVE: This study aimed to clarify the relationship between the directions of humeral head translation, the presence of acromial or coracoid spurs, and the locations of tendon tears in massive rotator cuff tears. Thirty shoulders from thirty patients with massive rotator cuff tears who underwent reverse shoulder arthroplasty were included. Preoperative 3DCT classified humeral head translation into three groups: minimal type, posterosuperior type, and anterosuperior type. The presence of acromial or coracoid spurs was also assessed. Preoperative MRI and intraoperative findings determined th

A unique case of Lemierre's syndrome status post blunt cervical trauma.

BACKGROUND: Lemierre's syndrome is a rare but potentially fatal condition. The course is characterized by acute tonsillopharyngitis, bacteremia, internal jugular vein thrombosis, and septic embolization. There have been some cases secondary to penetrating trauma to the neck. Literature review has yielded no cases secondary to blunt neck trauma in the absence of oropharyngeal injury. We aim to shed light on this unique cause of Lemierre's syndrome, so as to raise the index of suspicion for clinicians working up patients with blunt cervical trauma. METHODS: We present a case of a 25-year-old ma

Long Thoracic Nerve Injury Caused by Overhead Weight Lifting Leading to Scapular Dyskinesis and Medial Scapular Winging.

Scapular winging represents a rare phenomenon that most commonly results from nerve damage to either the long thoracic nerve, spinal accessory nerve, or, less commonly, the dorsal scapular nerve. This injury results in an abnormal scapulohumeral interaction during kinetic motion known as scapular dyskinesis. In this case report, the patient presented with scapular dyskinesis and medial scapular winging caused by overhead weight-lifting exercises, and a long thoracic nerve injury was diagnosed. Physicians are encouraged to consider long thoracic nerve damage in a patient with a history of repet

Sonographic detection of tears of the anterior portion of the rotator cuff (subscapularis tendon tears).

We studied the value of ultrasonography in identifying tears of the most anterior portion of the rotator cuff (subscapularis tendon tears). Shoulder sonography was performed on 1640 patients with shoulder symptoms, 435 of whom underwent surgery. Among the patients who were operated on, 17 had predominantly or exclusively subscapularis tendon tears; 14 of these were full-thickness and three were partial-thickness tears. Ultrasonography demonstrated 82% (14 of 17) of the subscapularis tendon tears; 86% (12 of 14) of the full-thickness tears and 67% (2 of 3) of the partial-thickness tears were co
