19 infants admitted with a diagnosis of infantile apnea who were found to have periodic breathing were given oral theophylline to determine its effect. They were studied at a mean age of 7.1 weeks (1-16.4 week). Each infant was studied during two naps, immediately before and 7 days following the institution of theophylline therapy, which averaged 2.8 h in duration during which electro-oculograms, end-tidal CO2, heart rate, impedance respirations, and transcutaneous pO2 (tcpO2) were continuously monitored. Theophylline therapy (mean dose 2.3 mg/kg q. 6 h) was associated with a significant reduc

PURPOSE OF REVIEW: Today, numerous studies show that orexin peptides act as regulators of many functions including the control of sleep-wake states, breathing, and central chemosensitivity. However, little is known on neuronal mechanisms by which orexin regulates breathing in a state-dependent manner. This review summarizes recent data on the control of neuronal circuits by orexin, with a special emphasis on breathing, central chemosensitivity, and obstructive sleep apneas.
RECENT FINDINGS: Activity of hypothalamic orexinergic neurons is subjected to maturation and is mandatory to maintain lo

Sleep in adequate amount and quality is essential for normal child development. Sleep disorders may affect the childs daytime functions resulting in behavioral problems such as attention deficit, aggressiveness and hyperactivity. Recent reports have suggested that chronic sleep disturbance can cause neurocognitive deficits and impaired learning abilities. Obstructive sleep apnea syndrome, asthma, atopic dermatitis and juvenile rheumatoid arthritis--are common childhood diseases that can impair normal sleep. This article reviews the sleep characteristics in these diseases and the relation betwe

INTRODUCTION: While several studies have indicated that central sleep apnea (CSA) is associated with atrial fibrillation and atrial flutter (AF) in older populations, few studies have focused on older Asian populations.
METHODS: We conducted a cross- sectional analysis using data from the 1999-2000, 7th exam cycle of the Kuakini Honolulu-Asia Aging Study. Participants were 718 Japanese-American men between 79 and 97 years old, who had overnight polysomnography. Obstructive Apnea-Hypopnea Index (OAHI) was the measure of the number of obstructive apneas and hypopneas with >4% oxygen desaturatio

Parasympathetic nervous system activity dominates during non-rapid eye movement (REM) sleep, whereas the sympathetic nervous system is dominant during REM sleep. The balance between the sympathetic and parasympathetic nervous systems reduces the heart rate and blood pressure during non-REM sleep and increases these parameters during REM sleep and wakefulness. Heart rate variability analysis, a non-invasive method of autonomic nervous system analysis, may be useful in screening for obstructive sleep apnea and also to predict orthostatic hypotension secondary to autonomic disturbances in patient

Disorders of sleep in children are distinctly different from sleep disorders in adults. Since children are forced to constantly learn and adapt, the effects of disordered sleep may be more profound. This paper reviews the disorders of sleep that are most significant in each of the stages of development from infancy to adulthood. Recent literature pertaining to advances in the diagnosis and treatment of sudden infant death syndrome and obstructive sleep apnea syndrome in children are highlighted. In addition, recent literature on the relationship of behavior and learning problems to sleep disor

BACKGROUND: Impaired sleep efficiency is independently associated with worse prognosis in patients with chronic heart failure (CHF). Therefore, a test was conducted on whether auto-servo ventilation (ASV, biphasic positive airway pressure [BiPAP]-ASV, Philips Respironics) reduces sleep fragmentation and improves sleep efficiency in CHF patients with central sleep apnea (CSA) or obstructive sleep apnea (OSA).
METHODS: In this multicenter, randomized, parallel group trial, a study was conducted on 63 CHF patients (age 64 ± 10 years; left ventricular ejection fraction 29 ± 7%) with CSA or OSA (a

PURPOSE: An earlier study found that heated humidification reduced the side effects of positive airway pressure (PAP) in patients with obstructive sleep apnoea (OSA). However, other studies disagreed with this finding. Therefore, we evaluated the relationship between heated humidification and the side effects of PAP in patients with OSA.
METHODS: According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, six databases were searched for relevant randomised controlled trials (RCTs) performed from January 1999 to June 2021.

There are limited data on the longitudinal frequency and severity of the symptoms and complications of achondroplasia. We undertook a retrospective electronic chart review of 114 patients to develop a more thorough understanding of the lifetime impact of achondroplasia. Craniocervical stenosis (involving the foramen magnum with or without cervical vertebrae C1 and/or C2) was noted in nearly 50% of patients with craniovertebral junction imaging; however, corrective decompression surgery was only needed in 6% of patients. No children in our cohort died at 4 years of age or under. Kyphosis was pr

Obstructive sleep apnea (OSA) is a highly prevalent disorder that has profound implications on the outcomes of patients with chronic lung disease. The hallmark of OSA is a collapse of the oropharynx resulting in a transient reduction in airflow, large intrathoracic pressure swings, and intermittent hypoxia and hypercapnia. The subsequent cytokine-mediated inflammatory cascade, coupled with tractional lung injury, damages the lungs and may worsen several conditions, including chronic obstructive pulmonary disease, asthma, interstitial lung disease, and pulmonary hypertension. Further complicati

Oscillometry or Forced Oscillation Technique, traditionally used in intermittent clinical measurements, has recently gained substantial attention from its application as a continuous monitoring tool for large and small airways. However, low frequency (<8 Hz) continuous oscillometry faces high breathing noise, and hence requires high oscillation amplitudes to maintain an acceptable signal-to-noise ratio. Therefore, PAP machines that utilize low frequency oscillometry do so intermittently to distinguish airway patency several seconds after a breathing pause has occurred. We hypothesized that hig

Hypoglossal nerve stimulation therapy via the Inspire® implant is a common alternative to positive airway pressure (PAP) treatment for obstructive sleep apnea (OSA). While hypoglossal nerve stimulation (HGNS) therapy offers a high rate of successful treatment outcomes, the post-activation care pathway, which involves gradual titration of the amplitude to achieve both subjective and objective improvements, can be lengthy, ranging from 3 to 12 months post-activation. Here, we report a case of a 55-year-old man with severe obstructive sleep apnea and a history of hypertension who underwent succes

Floppy eyelid syndrome (FES) is defined as eyelid hyperlaxity with reactive palpebral conjunctivitis. It is a common condition that can be associated with significant ocular irritation. FES presents with easily everted eyelids and chronic papillary conjunctivitis in the upper eyelids. It is frequently associated with ocular and systemic diseases, notably keratoconus and obstructive sleep apnea, respectively. This comprehensive review describes the epidemiology, pathological changes, proposed pathogenesis, clinical manifestations, and a variety of treatment options for this condition. Conservat

BACKGROUND: Sleep apnea leads to cognitive impairment in older patients, but its association with neurodegeneration remains controversial, and most studies do not distinguish between the more common obstructive form (OSAS) and the rarer central form (CSAS).
OBJECTIVE: The purpose of this study was to assess the prevalence of the different forms of sleep apnea in a cohort of cognitively impaired elderly patients (>70 years) and to investigate their associations with cognitive deficit, weighted against known risk factors for neurodegeneration.
METHODS: Overnight polygraphy was performed for 76

Despite advancements in diabetes management technology, many patients with type 2 diabetes (T2D) struggle to achieve optimal glycemic control. Sleep disorders such as obstructive sleep apnea (OSA) and insomnia are common in T2D and linked to poor glycemic control. Insomnia, particularly with short sleep duration, may worsen glycemic control by increasing diabetes-related distress. To improve glucose management, the association among sleep duration, insomnia severity, diabetes-related distress, and glycemic control needs to be further evaluated. A path analysis was conducted using data from 237
