A non-smoker adult male presented with haemoptysis of short duration. Chest radiograph (postero-anterior view) suggested an opaque left hemithorax. Further evaluation of lung lesion pointed towards a left lung hypoplasia with absent left pulmonary artery and a right-sided aortic arch (RAA). Both kidneys were enlarged with multiple cysts and thinning of parenchyma. This case describes a unique coexistence RAA and probable autosomal dominant polycystic kidney disease.

PATIENT PRESENTATION AND CLINICAL HISTORY: The patient is a 43-year-old male non-smoker who works as a farmer and cheese-maker. He complained of a dry cough for 6-12 months without fever or other clinical signs. His medical history was unremarkable with no use of drugs or alcohol and no signs of Raynaud's syndrome. In his family history he had two first-degree relatives with pulmonary fibrosis, one of whom also had rheumatoid arthritis.

PATIENT PRESENTATION AND DIAGNOSIS: The patient is a 73-year-old male retired house painter and ex-smoker who was admitted to the respiratory medicine department of a district hospital in Greece due to the presence of streaks of blood in his sputum. His past medical history included coronary artery disease, arterial hypertension and supraventricular tachycardia (SVT). A high resolution computed tomography (HRCT) was performed which showed enlarged mediastinal lymph nodes with evidence of fibrosis.

Mediastinal tuberculosis without lung involvement is an uncommon finding in an immunocompetent adult. We report the case of an 80-year-old male smoker who presented with dyspnea and stridor. He was found to have a mass in the middle mediastinum which also involved the anterior wall and 50% lumen if of the lower trachea. Histology and mycobacterial cultures of the lesion led to the diagnosis of mediastinal tuberculosis. Antituberculous treatment led to complete resolution of airway symptoms and reduction in the size of the mediastinal mass.

Spontaneous regression of pure seminoma metastases is a rare phenomenon, with only a few cases reported to date. To the best of our knowledge, this is the first report of regression of anaplastic pure seminoma metastases located in the retroperitoneum. We present a 27-year-old man, a marihuana smoker, with metastatic pure anaplastic seminoma in the high retroperitoneal lymph nodes. After orchiectomy, his metastases regressed with no medication. Several mechanisms are suggested to explain this phenomenon, which still remains elusive.

Making the correct diagnosis for a lingual ulceration can be difficult. We present the case of a 55-year-old man, alcoholic and smoker who presented with a single lingual ulceration. The initial diagnosis suggested a cell-squamed carcinoma but histological examination revealed a Wegener Granulomatosis. Wegener Granulomatosis in the oral cavity is found in 6-13% of cases but rarely primary sign of the disease. The authors review differential diagnosis that should be taken into consideration when presented with similar lesion.

Clinical description of a case of FMH in a 77 years-old man, heavy smoker and diabetic. At the beginning its appearance mimic a small polyp of the middle third of the right vocal cord. The patient refused any treatment at first, but one year later, when seen again in the office, a mass covering-up the right hemilarynx was detected and a total laryngectomy was advised and done. In spite of the correctness of the follow-up in the next six months, the unhappy commit suicide.

The authors carried out a study of thyroid function determining the levels of thyroxine (T4) and triiodothyronine (T3) in the serum of 60 patients (33 men, 27 women) between 31-79 years with Reinke's edema who had been treated microsurgically and phoniatrically. Hypothyroidism was found in one patient (a teacher, 47 years old, smoker). The study suggests that hypothyroidism is not an aetiological factor in the development Reinke's edema, which is in accordance with what other authors report.
