BACKGROUND: Orthopnoea and paroxysmal nocturnal dyspnoea are common entities regularly confronting thoracic physicians, particularly those with an interest in sleep medicine or non-invasive ventilatory support. One major cause is heart failure (HF), usually associated with abnormal lung function tests, and either obstructive or central sleep apnoea with Cheyne-Stokes respiration (CSA-CSR). PURPOSE: Traumatic diaphragmatic rupture is a diagnostic challenge for both surgeons and radiologists and generally occurs secondary to blunt and penetrating trauma of thoracoabdominal region.
MATERIAL AND METHODS: 56 patients who underwent surgical procedure due to blunt or penetrating trauma were included to the study.

We report a 52-year-old woman presenting with stab wounds on her back and upper extremities. A knife retained in her chest wall was not discovered in the emergency department. The rare occurrence of delayed hemorrhagic pericardial effusion requiring treatment is reported following significant blunt chest trauma (AIS greater than or equal to 3) in three patients from 12 to 15 days after injury. The author considers these wounds to be the most severe damage and treatment of patients of the kind should be performed with participation of the related specialists, provided with the urgent complex, instrumental examination and with due regard for the degree of criticism of wounded person. Methods and terms of treatment are strictly individual with regard to the degree of criticism of spine and associated injury.
