Use of latissimus dorsi pedicled myocutaneous flap for reconstruction in the chest area of an 8-month-old female infant with ectopia cordis.

Ectopia cordis (EC) is characterized by a complete or partial malposition of the heart outside the thorax. Despite the interdisciplinary treatment, the repair of EC is still very difficult and offers new surgical challenges because of its complexity and various combinations with other anomalies. We report the successful outcome after using a pedicled latissimus dorsi flap in reconstructive surgery in the setting of chronic wound dehiscence in an 8-month-old female infant born with a thoracic EC and omphalocele.

[Improvement of ultrasound study in the diagnosis of pleurisies of various etiologies].

The study was undertaken to improve ultrasonography using needle biopsy under ultrasound control in the diagnosis of pleurisies of various etiologies. The patients of the Department of Thoracic Surgery, who had been divided by the nosological principle (nonspecific pleurisy, tuberculosis effusions, and tumorous and posttraumatic pleurisies), were followed up. The study and statistical processing yielded major ultrasound criteria for pleural effusions and a final diagnosis was made on the basis of needle biopsy under ultrasound control with subsequent morphological verification.

[Congenital aplasia of left-sided pericardium].

Congenital defects of the pericardium are very rare and are often not recognized before surgery. We report a case study of a 16-year-old girl with a four-year history of dyspnoea on exertion, palpitations, left sided thoracic pain and near-syncope. Explorative thoracotomy revealed total absence of the left sided pericardium. This allows the heart to become displaced, thus giving the patient her uncharacteristic symptoms. The large defect was left open, to avoid compromising the venous circulation.

The microbiology, chemotherapy, and surgical treatment of tuberculosis.

Tuberculosis is a disease that has plagued humankind for centuries. The "white plague" is not only treatable and curable but also preventable. Initially, tuberculosis fell in the province of the general physician. With the advent of technologic advances in thoracic surgery, surgical management of tuberculosis was brought to the forefront. Effective bactericidal drug therapy became available by 1954 after the development of streptomycin in 1945 and isoniazid in 1952. Additional effective antituberculous drugs have relegated surgical therapy for tuberculosis to a relatively minor role.

Robotic resection of parathyroid adenoma in the middle mediastinum.

Ectopic parathyroid adenomas present a diagnostic and surgical challenge, particularly when located in anatomically complex regions such as the middle mediastinum. This report describes the case of a 76-year-old male with primary hyperparathyroidism caused by a parathyroid adenoma situated anterior to the oesophagus and beneath the aortic arch. Robotic-assisted thoracoscopic resection enabled precise, minimally invasive excision with successful post-operative biochemical resolution. This case highlights the evolving role of robotic techniques in endocrine and thoracic surgery.

Giant Aneurysm of Ascending Aorta and Aortic Arch: A Report of a Rare Case.

A thoracic aortic aneurysm is considered giant when its diameter exceeds 10 cm. We report a rare case of a giant aneurysm involving the ascending aorta and aortic arch in a 40-year-old man, initially diagnosed as an acute aortic dissection. The patient underwent emergency surgery, during which the ascending aorta and aortic arch were replaced under deep hypothermia and circulatory arrest with selective antegrade cerebral perfusion. Strong teamwork resulted in a favorable postoperative course for the patient.

Approach for oligometastasis in non-small cell lung cancer.

Non-small cell lung cancer (NSCLC) harboring a limited number of distant metastases, referred to as the oligometastatic state, has been indicated for surgery for the past several decades. However, whether the strategy of surgical treatment results in a survival benefit for such patients remains controversial. Experientially, however, thoracic surgeons often encounter long-term survivors among surgically resected oligometastatic NSCLC patients. In this article, the current situation of surgical approach and potential future perspective for oligometastatic NSCLC are reviewed.

Postoperative nodal irradiation in breast cancer patients with 1 to 3 axillary lymph nodes involved: the debate continues….

Regional lymph node irradiation is usually indicated in patients with positive node breast cancer. However, there are some controversies regarding the clinical benefits of adding regional nodal irradiation to whole-breast or thoracic-wall irradiation after breast surgery especially for patients with 1 to 3 positive axillary lymph nodes. More recently, two important studies (NCIC MA.20 and EORTC 22,922-10925) were published to address this significant issue and some further points need to be discussed.

Left ventricular pseudoaneurysm after valve replacement.

We present a case of left ventricular pseudoaneurysm, which is a very rare and fatal complication of cardiac procedures such as mitral valve replacement. A 55-year-old woman presented to the Department of Thoracic and Cardiovascular Surgery at Hanyang University Seoul Hospital with chest pain. Ten years prior, the patient had undergone double valve replacement due to aortic regurgitation and mitral steno-insufficiency. Surgical repair was successfully performed using a prosthetic pericardial patch via a left lateral thoracotomy.

Fasciocutaneous flap in esophageal stricture with ventriculoperitoneal shunt.

Abdominal surgery in a patient with ventriculoperitoneal shunt may increase the risk of shunt malfunction and infection. We present a successful case of resection and reconstruction of the cervical esophagus by rolled lateral thoracic artery fasciocutaneous flap in a patient with corrosive esophageal stricture and preexisting ventriculoperitoneal shunt. Follow-up esophagogastroscopy after 3 months revealed wide patent graft. Rolled fasciocutaneous flap may be a safe alternative treatment without risk of shunt-associated complications in a patient with ventriculoperitoneal shunt.

VACTERL Association Complicated with Right-sided Congenital Diaphragmatic Hernia.

We describe a neonate with VACTERL association and right-sided congenital diaphragmatic hernia (CDH). Such coexistence is rare. The lack of symptoms during the early neonatal period, the absence of bowel loops herniated into the right thoracic cavity, and an unfinished surgery led to clinical and radiological diagnostic difficulties. Respiratory distress occurred when the patient was 2 months old. Chest radiology plain film revealed typical findings of right-sided CDH. The diagnosis was confirmed after surgical exploration.

Migration of a Kirschner wire to the heart in a Yorkshire terrier.

A 12-year-old, male Yorkshire terrier was presented for acute pulmonary oedema. Thoracic radiographs showed a linear metallic foreign body within the cardiac silhouette. Echocardiogram showed a hyperechoic line extending through the left ventricle, the mitral valve, leading into the left atrium. A 4 cm long Kirschner wire was surgically removed by left fourth thoracotomy. The dog died two days after surgery for acute pulmonary oedema. Necropsy showed thrombi on the mitral leaflets that impeded their movement.

Coronary artery bypass surgery in undiagnosed traumatic pericardial rupture--25 years later.

A 55-year-old man was electively admitted for coronary artery bypass surgery. His admission chest X-ray showed an abnormal cardiac silhouette with complete leftward displacement. He had a past history of blunt thoracic trauma due to a motor vehicle accident treated conservatively. We present our findings and surgical difficulty during an operation on a patient with a previously undiagnosed pericardial rupture. This is the first reported case of its kind in the cardiac surgical literature.

Videothoracoscopic resection of intrathoracic neurogenic tumors: report of two cases.

Two cases of intrathoracic neurogenic tumors are discussed. The benign neoplasms were located in the posterior mediastinum and caused no clinical symptoms. In both cases, complete resection of the lesion was achieved by video-assisted thoracic surgery (VATS). There were no intraoperative complications. Postoperatively, one patient presented with Claude Bernard-Horner syndrome, which resolved spontaneously after 1 week. Although the great majority of mediastinal neurogenic neoplasms are benign, resection is necessary to prevent malignant transformation and intraspinal extension.

Minimally invasive diaphragm plication in an infant.

We report the use of video-assisted thoracic surgery to plicate the diaphragm after phrenic nerve injury associated with an operation for congenital heart disease. Right diaphragm paresis developed in a cyanotic newborn girl with pulmonary atresia and intact ventricular septum after a right modified Blalock-Taussig shunt. Diaphragm plication was performed endoscopically and the patient recovered. Refinement of technique and instrumentation may allow wider application of video-assisted thoracoscopic plication of the diaphragm in neonatal and pediatric patients.

The history of esophageal surgery.

BACKGROUND: Esophageal surgery has always been considered extremely demanding, given the surgical experience and skills required, due to the anatomic location of the esophagus as well as the impact of such extensive operations on patients' homeostasis. The intention of the present study is to review all historical time points of esophageal surgery, from its inception to the modern era of minimally invasive esophageal surgery. METHODS: The first clear description of esophageal surgery is dated back to 1738, in an attempt to remove a foreign body from the thoracic esophagus. However, the high m

Thoracic adolescent idiopathic scoliosis curves between 70 degrees and 100 degrees: is anterior release necessary?

STUDY DESIGN: A retrospective review of adolescents with main thoracic scoliotic curves surgically treated with either anterior release and posterior fusion or posterior fusion only. OBJECTIVES: To compare the radiographic and clinical outcomes of two surgical treatments: anterior-posterior spinal fusion (APSF) versus posterior spinal fusion (PSF) alone in patients with large 70 degrees to 100 degrees thoracic adolescent idiopathic scoliosis (AIS) curves. SUMMARY OF BACKGROUND DATA: Surgical treatment of thoracic AIS curves between 70 degrees and 100 degrees often consists of anterior and po

[Surgical Treatment of Pectus Excavatum in Patients with Marfan Syndrome Associated with Vertebral or Cardiovascular Disease].

Surgical treatment is often required in patients with Marfan syndrome presenting with scoliosis or cardioaortic disease. If these patients present with pectus excavatum, surgery for spinal deformity correction or chest wall closure during cardioaortic surgery can cause thoracic organ compression secondary to narrowing of the thorax resulting in hemodynamic and respiratory compromise. Cardiovascular complications serve as prognostic factors in patients with Marfan syndrome, and surgical treatment is often required in these cases. In our case series involving 4 patients, 2 patients with pectus e

Multimodality imaging of primary ureteral hemangiosarcoma with thoracic metastasis in an adult dog.

A 12-year-old spayed female standard Poodle was presented for investigation of severe hematuria. Abdominal ultrasound and thoracic and abdominal computed tomography identified severe hydronephrosis due to an obstructive ureteral mass with no evidence of metastasis. Histological examination after nephrectomy and ureterectomy confirmed an obstructive ureteral hemangiosarcoma. Forty days after surgery, the dog was presented with severe dyspnea. Survey radiographs of the thorax revealed a severe diffuse nodular interstitial pattern. Postmortem histological examination revealed pulmonary metastasis

Acute, unanticipated, and prolonged superior vena cava occlusion during pneumonectomy.

Acute, unanticipated superior vena cava (SVC) occlusion during thoracic surgery can have profound hemodynamic consequences and lead to devastating neurologic injury. We describe the successful anesthetic management of a pneumonectomy complicated by prolonged intraoperative SVC occlusion lasting a total of 290 minutes. To our knowledge, this represents the longest reported SVC occlusion time with no subsequent neurologic sequelae. Based on our favorable outcome and a review of the relevant literature, we offer a discussion of strategies for anesthetic management.
