Laryngeal tuberculosis (LT), a rare but possible manifestation of extrapulmonary tuberculosis (TBC) and the most frequent granulomatous disease of the larynx, is slowly resurfacing due to the worldwide recrudescence of TBC. We present the case of a 59 y-o Caucasian woman, non-smoker, with no history of recent travels in endemic areas, affected by pulmonary sarcoidosis, that presented with a symptomatic vegetating lesion involving the left free margin of the epiglottic and a small, ulcerated lesion over the right arytenoid mucosa. While the patient's profile would not lead to a primary suspect

Tuberculosis (TB) is a highly infectious disease on the rise caused by the organism Mycobacterium tuberculosis and health care workers working in emergency departments, medical wards and autopsy rooms are in danger of contacting this disease. We present a case of a 42 year old man found dead under a pedestrian bridge with no medical history available. Post mortem computed tomography showed multiple cavities involving upper lobes of both lungs and areas of consolidation in both lung fields raising the suspicion of pulmonary tuberculosis. This was followed by a computed tomography guided lung bi

Isolated pancreatic tuberculosis (TB) is an exceedingly rare disease, even in countries with a high burden of TB. We report the case of a 40-year-old gentleman who presented with a two-week history of fever and abdominal pain. Computed tomography of the abdomen showed a large heterogeneous mass arising from the pancreas with peri-pancreatic lymphadenopathy, infiltration of the stomach, and encasement of the celiac vessels-highly suggestive of pancreatic malignancy. Oesophagogastroduodenoscopy with endoscopic ultrasonography was performed, which showed a necrotic area in the body of pancreas. F

INTRODUCTION: The Drug Reaction with Eosinophilia and Systemic Symptom (DRESS) is a severe drug-induced reaction.
CASE REPORT: We report the case of a 35-year-old man treated by RHEZ for a first episode of a smear positive pulmonary tuberculosis and who developed a DRESS syndrome due to pyrazinamide after twenty days of treatment, associated with a viral reactivation to Human Herpes Virus 6 (HHV6). He had a skin eruption, liver involvement and hypereosinophilia. He fully recovered after drug withdrawal, associated with local and general corticosteroids. He died two weeks after discharge.

Tuberculosis of the central nervous system (CNS-TB) is a critical disease with poor prognosis if not diagnosed and treated early. A regimen of drugs with good CNS penetration is required for treatment. Treatment options are limited in case of hypersensitivity reaction of the recommended regimen. We are reporting a rare instance where a patient developed hypersensitivity to both first line agents (isoniazid and rifampin) for CNS-TB and was successfully desensitized against both these medications to complete the treatment course using the rapid desensitization process. There have been reported c

Progressive liver failure is rarely seen in tuberculosis chemoprophylaxis with isoniazid. We present a case of a 32-year-old woman admitted to our clinic reporting abdominal pain, nausea and vomiting for 2 days. The initial diagnosis was fulminant toxic hepatitis due to isoniazid chemoprophylaxis, which was treated successfully with living donor transplantation. Tuberculosis continues to be a significant public health problem. Isoniazid-related hepatotoxicity is extremely rare in adults. The only treatment in cases of fulminant liver failure is orthotopic liver transplantation from a deceased

Tuberculosis is considered as ubiquitous disease as it involves any organ, but primary involvement of abdominal muscles is very rare. In most cases, the muscle involvement is secondary and is caused by either hematogenous route or direct inoculation from a tuberculous abdominal lymph node or extension from underlying tubercular synovitis and osteomyelitis. Autopsy studies have shown abdominal wall involvement in less than 1% of patients who died of tuberculosis. Antitubercular therapy is main form of management. Surgical intervention is always secondary in the form of either sonography or comp

OBJECTIVE: To study if different forms of clinical presentation of tuberculosis in children are associated with a different type of tuberculin reaction.
METHODS: This cross sectional study, describing Tuberculin skin testing (TST) responses in 268 children (134 cases and 134 controls) less than 12 yr of age was carried out over a period of 18 months at JIPMER, a tertiary care referral hospital in Pondicherry, India. The qualitative and quantitative TST responses in various clinical forms of Tuberculosis were analysed.
RESULTS: Koch's and Listeria variants were more common in children with TB

A case study of an acute tuberculous bronchiolitis (a rarely encountered form of generalized miliary tuberculosis), whose aggravated progressing resulted in fatal outcome 2 months after the onset of the initial disease signs, is described. The prevalence of non-specific infectious-and-allergic manifestations is a clinical-and-morphological peculiarity within the case study, which confirms a diversity of morphological changes occurring in the organism in tuberculosis infection. The etiological- and pathogenetic specific features of acute tuberculous bronchiolitis in children and in adults are u

In this study, a case has been reported involving a 66-year old male who was admitted for scrotal pain on the right side with possible testicular involvement, but with no associated urinary disorder. At physical examination, the right testicle was found to have increased in volume: this was further confirmed by ultrasonography, but the findings were insufficient to exclude the hypothesis of testicular cancer. An exploratory orchidectomy by upper inguinal route was therefore carried out, and histopathological examination showed the destruction of testicular tissue by several granulomas, and cas

We report a case of genital tuberculosis occurring in the tunica albuginea and the tunica vaginalis. A 29-year-old man came to our department with the complaint of a nodule in the right scrotum. The physical and radiological examinations revealed no signs of pulmonary or urogenital tuberculosis. The urine and the semen were clear and negative in culture of acid fast bacteria, but the tuberculin reaction was moderately positive. Surgical exploration revealed a white yellowish soybean sized nodule in the surface of the tunica albuginea of the right testicle apart from the epididymidis, and we th

1) Two students of different high-schools were found to have pulmonary tuberculosis by school medical examination. These two students learned in the same private extra-school tutoring institute when they were junior high-school students. Assuming this event as a group infection of tuberculosis in the extra-school tutoring institute, we performed an extraordinary examination on the teachers working there. 2) In the extraordinary examination, a patient who could be judged as the source of infection was found. 3) As a result of extraordinary examination on students learning in the institute, a bi

Theories of the pathogenesis of tuberculosis accepted by clinicians in this field, receive no mention or little emphasis in textbooks of pathology. These are: the systemic character of the disease from the initial infection; the role of tissue allergy; the subclinical, benign and lifelong character of most tuberculosis infections, yet liable to an endogenous activation by the influence of stress. These concepts go far back in history, and reference is made to Bayle (1816), Koch (1891), Calmette and Guérin (1906), Von Pirquet (1907) and Ghon (1912). The origin of the pulmonary apical lesion was

Tuberculosis is the leading cause of mortality by infectious agents worldwide. The necrotic debris, known as caseum, which accumulates in the center of pulmonary lesions and cavities is home to nonreplicating drug-tolerant Mycobacterium tuberculosis that presents a significant hurdle to achieving a fast and durable cure. Fluoroquinolones such as moxifloxacin are highly effective at killing this nonreplicating persistent bacterial population and boosting TB lesion sterilization. Fluoroquinolones target bacterial DNA gyrase, which catalyzes the negative supercoiling of DNA and relaxes supercoils

INTRODUCTION: The reduction of tuberculosis reported in admitted patients in a community hospital in La Habana (Cuba) was identified as a quality gap and priority for action. The objective was to increase by 50% the number of bacilloscopies and smear-positive tuberculosis confirmed by December 2017.
PATIENTS AND METHODS: A quality improvement initiative was conducted from January 2017 to December 2017 in a 300-bed secondary care teaching hospital. The improvement project was addressed to patients admitted with respiratory infections (upper or lower). The baseline was considered the period fro

Regulatory T cells (Tregs) play an essential role in immune homeostasis. In infectious diseases Tregs may inhibit protective responses facilitating pathogen multiplication and dissemination, but they may also limit the inflammatory response diminishing tissue damage. Although there is experimental and clinical evidence that Tregs are induced during Mycobacterium tuberculosis infection, their role in the immunopathogenesis of tuberculosis is still not completely understood. In this study, the phenotype, frequency and activity of circulating Tregs in active and latent tuberculosis were evaluated

The number of tuberculosis outbreak among adult groups has increased. In some of them, it is difficult to identify the route of infection and sometimes many people are involved in the outbreak. We experienced a suspected case of mass outbreak in a small company separated into 2 floors. The first patient, who lived in another city, was diagnosed as pulmonary tuberculosis in April with bilateral extensive cavitary lesions complicated with pleurisy on chest X-ray, and his sputum smear was Gaffky No. 8 and culture (+) (resistant to isoniazid). We conducted tuberculin skin test (TST) for those unde

Although the tuberculin test has aided in the diagnosis of tuberculosis for more than 85 years, its interpretation is difficult particularly because sensitization with non-tuberculous mycobacteria leads to false positive tests. Using the guinea pig model of tuberculosis, we have recently described a recombinant antigen (DPPD) that could circumvent this problem. The DPPD gene is unique to the M. tuberculosis complex organisms and is absent in the organisms representative of all other members of the Mycobacterium genus. Moreover, DPPD induced strong DTH in 100% of the guinea pigs infected with M

BACKGROUND: Because of the limitation of isolation spaces suitable for tuberculosis, proper use of these spaces is prudent. We examined the current approach to respiratory isolation to determine the compliance with Centers for Disease Control and Prevention (CDC) guidelines for identifying active tuberculosis.
METHODS: A retrospective review was performed of all patients placed in respiratory isolation and those with pulmonary tuberculosis.
RESULTS: Seventy-seven instances of isolation and three admissions without isolation were encountered. Isolated patients met CDC guidelines for infectiou
