UNLABELLED: An adult female presents for chronic eye complaints refractory to outpatient ophthalmology management, as well as purpuric rash, Reynaud's symptoms, and reported nodules. She received ophthalmology and rheumatology consultations while in the emergency department (ED) and was subsequently admitted. Expedited rheumatologic workup revealed evidence of granulomatosis with polyangiitis, a rare and potentially debilitating autoimmune vasculitis. She was ultimately discharged after an eight-day hospital course on a long prednisone taper with plan for outpatient management and transition t

Granulomatosis with polyangiitis (GPA), formerly known as Wegener's granulomatosis, is a rare form of pauci-immune vasculitis that primarily affects the respiratory tract and kidneys, though it can involve virtually any organ system. As a systemic vasculitis, it targets small- and medium-sized blood vessels and is associated with anti-neutrophil cytoplasmic antibodies (ANCA), particularly those directed against proteinase 3 (PR3). Due to its nonspecific symptoms and variable clinical presentation, GPA requires a high index of suspicion for timely diagnosis. Early treatment is essential, as the

INTRODUCTION: Granulomatosis with polyangiitis (GPA) is a rare systemic vasculitis, rarely affecting the genitourinary tract. Vulvar involvement is extremely uncommon and often misdiagnosed.
CASE PRESENTATION: A 21-year-old female patient presented with a 1-month history of necrotic vulvar lesions and skin ulcerations. The markedly elevated PR3-ANCA and histopathological findings supported the diagnosis of GPA. Imaging methods revealed multiple spleen infarctions and pseudo-masses in the kidneys; endocranial MRI showed pansinusitis and otomastoiditis. Initial treatment with systemic corticost

RATIONALE: Associations of autoimmune diseases are rare but interesting and challenging situations from a diagnostic, pathophysiological, and therapeutic point of view. This article studies a rare association of autoimmune diseases by discussing the pathophysiological hypotheses and an original therapeutic management. The coexistence of antineutrophil cytoplasmic antibody-associated vasculitis and spondyloarthritis has rarely been described.
PATIENT CONCERNS: We present a patient with inflammatory back pain, stiffness, and enthesopathies followed by pulmonary and ear, nose and throat granulom

This report presents a rare case of scleritis with peripheral ulcerative keratitis secondary to granulomatosis with polyangiitis (GPA). A 65-year-old Caucasian male presented to a regional ophthalmology service with an atypical red eye. His immune work-up demonstrated positive anti-neutrophil cytoplasmic IgG autoantibodies (ANCA) with anti-proteinase 3 antibody (PR3) elevation. Multi-system vasculitis was discovered including lung, liver, bladder, prostate, nasal and paranasal sinuses involvement. His ocular sequelae included significant peripheral corneal thinning requiring cyanoacrylate glui

OBJECTIVE: This article reviews autoimmune axonal neuropathies, their characteristic clinical features, disease and antibody associations, appropriate ancillary testing, treatment, and prognosis.
LATEST DEVELOPMENTS: In 2021, the American College of Rheumatology and the Vasculitis Foundation released new summary guidelines for the treatment of antineutrophil cytoplasmic autoantibody-associated vasculitides. In addition, novel autoantibodies have been recently identified; they are often paraneoplastic and associated with axonal neuropathies.
ESSENTIAL POINTS: Recognition of autoimmune axonal

Eosinophilic gastroenteritis (EGE) is a disease involving the gastrointestinal tract that involves the invasion of eosinophils into the lumen of the tract. Invasion of eosinophils in the tract causes edema and inflammation of the wall, resulting in acute or chronic diarrhea and abdominal pain. EGE can cause chronic inflammation in the small intestine, thereby mimicking chronic vasculitis. We report the case of a 24-year-old male patient with the chief complaint of acute progressive abdominal pain. Endoscopic findings of the small intestine were chronic and similar to those of polyangiitis (eos

Malignant catarrhal fever (MCF) is a severe, systemic, lymphoproliferative disease affecting domestic ruminants, caused by a group of MCF viruses in the genus Macavirus. Infection of cattle and bison with ovine herpesvirus 2 (OvHV2) is economically significant in North America. Sheep are the reservoir host of the virus, and only rarely manifest disease. Cattle and bison, however, frequently have lymphoproliferation, mucosal ulceration, and systemic vasculitis. OvHV2-induced MCF in cattle and bison is often fatal, with clinical recovery reported only rarely. Chronic cases are uncommon, but vasc

Immunoglobulin A (IgA) nephropathy is the most common cause of primary glomerulonephritis worldwide. IgA vasculitis (formerly known as Henoch-Schonlein purpura) typically presents with IgA nephropathy on renal biopsy in addition to extrarenal symptoms like purpura, abdominal pain, and arthritis. Diffuse alveolar hemorrhage (DAH) is the most common pulmonary complication, but this is rarely seen. In this case report, we describe a 35-year-old male with chronic untreated hepatitis B infection who presented with pulmonary-renal syndrome. He was found to have clinical findings of DAH and concomita

BACKGROUND: Sarcoidosis, an idiopathic systemic disorder characterized by noncaseating granulomas, is often associated with granulomatous uveitis. Anterior segment involvement can present with mutton-fat keratic precipitates, anterior chamber cell, and iris nodules. Sarcoid associated posterior uveitis may present with vitritis, retinal vasculitis, and choroidal lesions.
CASE SUMMARY: Sarcoid-associated retinal vasculitis is classically thought of as predominantly involving veins, but in this case report we describe a 76-year-old Caucasian woman presenting with bilateral posterior uveitis, un

Eosinophilic disease with orbital involvement is rare. We present two patients with dacryoadenitis associated with local and systemic eosinophilia. A 32-year-old man presented with episodic dacryoadenitis, lower respiratory inflammation and peripheral eosinophilia. Lung and lacrimal gland biopsies demonstrated eosinophilic infiltrate without granuloma, necrosis, or vasculitis. He improved with oral corticosteroids and Mepolizumab, an IL-5 inhibitor. The second case involved a 33-year-old man who similarly presented with episodic dacryoadenitis, pulmonary inflammation and pain/swelling in the h

Granulomatosis with polyangiitis (GPA) is a small-vessel vasculitis that is highly associated with anti-neutrophil cytoplasmic antibodies. GPA carries an increased risk of organ infarction, but renal infarction is rare. We herein describe a case of multiple renal infarctions caused by GPA. A 66-year-old man presented with hearing loss, nasal discharge, fatigue, and weight loss for several months. Cross-sectional contrast-enhanced computed tomography images revealed multiple low-attenuation areas in both kidneys. He subsequently developed fever and impaired renal function. Blood serum was posit

When death is sudden and unexpected, autopsy may not provide sufficient information to be sure of the cause of death. Some causes can only be identified by microscopy. We report a case of sudden death in a woman who suffered from vertigo and hypertension. The autopsy only showed sclerosis of the thoracic aorta, abdominal aorta and coronary arteries, and the toxicological results were negative and did not explain the cause of death. A microscopic and immunohistochemical analysis of samples from various organs was carried out. The histopathological examination revealed eosinophilia in the aortic

A 21-year-old woman presented with renal dysfunction during macrohematuria. A kidney biopsy revealed IgA nephropathy with a small percentage of crescent formation and macrohematuria-associated tubular injury. Macrohematuria-associated acute kidney injury could explain her renal dysfunction. However, she was seropositive for myeloperoxidase (MPO)-anti-neutrophil cytoplasmic antibody (ANCA) and showed fibrin deposition around one arteriole. Corticosteroids and mycophenolate mofetil were administered as for ANCA vasculitis, and the serum creatinine, abnormal urinalysis and MPO-ANCA titer all grad
