Mind the gap: What is the difference between alcohol treatment need and access for Aboriginal and Torres Strait Islander Australians?

BACKGROUND: Alcohol-related harms cause great concern to Aboriginal and Torres Strait Islander (Indigenous) communities in Australia as well as challenges to policy makers. Treatment of alcohol use disorders forms one component of an effective public health response. While alcohol dependence typically behaves as a chronic relapsing condition, treatment has been shown to be both effective and cost-effective in improving outcomes. Provision of alcohol treatment services should be based on accurate assessment of treatment need. AIMS: In this paper, we examine the likely extent of the gap between

Alcohol in fatal and nonfatal injuries: a comparison of coroner and emergency room data from the same county.

Although alcohol is thought to be associated with severity of injury, relatively little data are available that compares alcohol's involvement in injury cases treated in the emergency room (ER) with coroner cases of injury fatality, both coming from the same county. Data from a probability sample of casualty patients 18 years and older treated at the University of Mississippi Medical Center ER in Jackson during a 6-month period (n = 275) are compared with data from coroner reports of all fatalities from unnatural causes among those 18 and older in the same county during a 1-year period surroun

Cocoa and Cardiac Rhythm: an Arrhythmogenic Trigger in Dietetic Chocolate Abuse?

The link between arrhythmias and lifestyle factors, such as obesity and alcohol consumption, is well-established; while coffee, tea, chocolate, and excessive energy drink consumption appear to be potential harmful in patients with rhythm disorders. However most nutritional recommendations are based on observational studies, still representing a fertile area for further research. This article provides a review of the association between cardiac arrhythmias and cocoa dietary patterns and components, thus discussing potentially related mechanisms.

[Ethanol-induced injury and GI bleeding].

Ethanol-induced injury of digestive organs closely linked to the GI bleeding. The gastric or esophageal mucosal hemorrhage evoked by extra-amounts of alcohols is initiated by the microcirculatory damage of gastrointestinal mucosa. Many investigators focused on the ethanol-induced gastric ulcer formation as well as the ethanol-induced gastroesophageal reflux injury. Although the most important treatment for such a damage is of course to stop the drinking, it is also important to protect the gastrointestinal mucosa by potentiate the mucosal defense systems.

Using machine learning to identify unique predictors of alcohol and cannabis impaired driving.

BACKGROUND: Alcohol- and cannabis-impaired driving remain major public health concerns, particularly among young adults. Although prior studies have identified numerous risk factors, most have focused on limited subsets of predictors, restricting a broader understanding of impaired driving. This study applied machine learning to identify salient predictors of alcohol- and cannabis-impaired driving from a wide range of candidate variables. METHODS: Data came from annual cross-sectional surveys of 18- to 25-year-olds participating in the Washington Young Adult Health Survey (2015-2022). Analyse

C/EBPβ transcription factor promotes alcohol-induced liver fibrosis in males via HDL remodeling.

BACKGROUND: Alcohol-associated liver disease (ALD) is the main cause of alcohol-associated mortality. However, the mechanism of ALD development is poorly understood. Epigenetic changes are thought to play an important role in ALD. We aimed to define the epigenetic changes induced by alcohol and predict drivers of these changes. METHODS: Mice were fed high-fat diet with or without 20% of alcohol in the drinking water for 20 weeks (WDA model). scATAC-seq data set was analyzed using Signac R package. To test the role of C/EBPβ, Cebpb-floxed mice were treated with AAV8-TBG-Cre or AAV8-control. R

Effectiveness and cost-effectiveness of a transdiagnostic intervention targeting alcohol misuse and psychological distress for men in Ukraine: study protocol for a randomised controlled trial.

BACKGROUND: Ukraine has experienced armed conflict since 2014, with significant escalation in 2022. Since then, an estimated 3.7 million people have been internally displaced. Alcohol misuse remains a substantial public health challenge in Ukraine, with high levels of psychological distress among the displaced population. The current study aims to evaluate the effectiveness and cost-effectiveness of a transdiagnostic intervention (CHANGE) to address alcohol misuse and psychological distress through problem-solving therapy and selected behavioural strategies for managing alcohol misuse. We hypo

A pilot evaluation of managed alcohol programs operating in the context of the COVID-19 pandemic.

BACKGROUND: Managed Alcohol Programs (MAPs) are a harm reduction strategy designed for individuals with severe AUD, unstable housing, and previous unsuccessful treatment attempts. MAPs provide access to individualized doses of beverage alcohol alongside other social supports and are effective for stabilizing alcohol consumption and reducing alcohol-related harms. In Canada, MAP models (scattered site outreach or fixed site) were developed in response to the COVID-19 pandemic to reduce harms associated with severe AUD, high-risk drinking, and unstable housing as means of supporting physical iso

Unraveling time-dependent genetic components underlying alcohol response.

While numerous studies have examined the subjective response to alcohol as an intermediate phenotype to understand its variability, heritability, and predictive capacity for alcohol-related disorders, in-depth analyses linking alcohol reactivity indicators to genetic factors within a large cohort have been absent. Our study aimed to quantify the exact contribution of each genetic variant relevant to the alcohol metabolism to the variability in alcohol response. Specifically, we focused on two primary genes involved in alcohol metabolism (ALDH2 and ADH1B) and three additional loci (ALDH1B1, ALD

Incidence and prevalence of alcoholic fatty liver disease: a prospective, community-based study among adult Sri Lankans.

BACKGROUND: Data on the alcoholic fatty liver (AFL) is limited. Therefore, we investigated alcohol use and AFL patterns among urban, adult Sri Lankans. METHODS: The study population (selected by age-stratified random sampling) was screened in 2007 (35-64 years) and re-evaluated in 2014. They were assessed by structured interviews, anthropometric measurements, liver-ultrasound, and biochemical and serological tests. AFL was diagnosed on ultrasound criteria, 'unsafe' alcohol consumption (Asian standards: males>14units, females>7units per week) and absence of hepatitis B/C markers. Controls were

Alcohol intake from early adulthood to midlife and mammographic density.

PURPOSE: Moderate alcohol consumption (15 g/day) has been consistently associated with increased breast cancer risk; however, the association between alcohol and mammographic density, a strong marker of breast cancer risk, has been less consistent. Less is known about the effect of patterns of alcohol intake across the lifecourse. METHODS: Using the Early Determinants of Mammographic Density study, an adult follow-up of women born in two US birth cohorts (n = 697; Collaborative Perinatal Project in Boston and Providence sites and the Childhood Health and Development Studies in California), we

Alcohol as a risk factor for cancer: existing evidence in a global perspective.

The purpose of the present review is to give an overview of the association between alcohol intake and the risk of developing cancer. Two large-scale expert reports; the World Cancer Research Fund (WCRF)/American Institute of Cancer Research (AICR) report from 2007, including its continuous update project, and the International Agency for Research of Cancer (IARC) monograph from 2012 have extensively reviewed this association in the last decade. We summarize and compare their findings, as well as relate these to the public health impact, with a particular focus on region-specific drinking patt

The biometric measurement of alcohol consumption.

BACKGROUND: Proper ascertainment of the history of alcohol consumption by an individual is an important component of medical diagnosis of disease and influences the implementation of appropriate treatment strategies that include prescription of medication, as well as intervention for the negative physical and social consequences of hazardous/harmful levels of alcohol consumption. Biological (biometric) diagnostic tests that provide information on current and past quantity and frequency of alcohol consumption by an individual, prior to onset of organ damage, continue to be sought. METHODS: Pla

Changes in health professionals' knowledge, attitudes and practice following provision of educational resources about prevention of prenatal alcohol exposure and fetal alcohol spectrum disorder.

We provided health professionals in Western Australia (WA) with educational resources about prevention of prenatal alcohol exposure and fetal alcohol spectrum disorder and assessed changes in their knowledge, attitudes and practice concerning fetal alcohol syndrome (FAS) and alcohol consumption in pregnancy. Following our 2002 survey of health professionals in WA, we developed and distributed educational resources to 3348 health professionals in WA in 2007. Six months later we surveyed 1483 of these health professionals. Prevalence rate ratios [PRR] and 95% confidence intervals [CI] were calcu

[Alcohol consumption, pregnancy and fetal alcohol syndrome: implications in public health and preventive strategies].

Alcohol consumption during pregnancy is a significant public health problem and is an established cause of serious birth defects and developmental delay collectively described as fetal alcohol syndrome (FAS). FAS is caused by congenital alcohol-induced damages and is a cause of mental retardation. It is characterised by facial abnormalities and growth deficiency. Infants affected by the syndrome show intellectual impairment, and difficulties in learning, memory, problem-solving, and attention as well as experiencing additional problems with mental health and social interactions. However, an ab

Religiosity as a Substance Use Protective Factor Among Female College Students.

Research exists about religiosity as a substance use protective factor. However, there is little attention of this issue regarding Israeli female college students. Undergraduate religious and secular students were studied. Religious students reported lower last month tobacco, alcohol, cannabis and non-medical prescription drug use. Secular females who smoked, missed class because of party habits and reported easy access to cannabis had a higher probability of binge drinking. This study contributes to knowledge about religiosity and college student substance use, and it provides useful informat

Family violence and Asian drinkers.

This is a study of 70 consecutive referrals to the Department of Psychological Medicine, National University Hospital, for treatment of drinking problems. There were 31 Chinese and 39 Indian drinkers, with 63 males and 7 females. Family violence with physical abuse occurred in 21 cases (30%). One of three wives of drinkers had been physically abused, but no children were victims. Drinkers with family violence were generally younger and more severely dependent on alcohol than the non-violent drinkers. There was no relationship between family violence and the ethnicity or marital status of the d

Water intoxication and rhabdomyolysis.

A 44-year-old woman was admitted because of stupor. She had consumed 3 liters of water due to thirst after drinking alcohol. Laboratory findings on admission revealed marked hyponatremia (sodium: 115 mEq/l). She was diagnosed as having water intoxication. She recovered from her hyponatremia upon excretion of a large amount of hypotonic urine. Subsequently, however, her serum creatine phosphokinase was markedly elevated at 28,650 IU/l, and her serum myoglobin reached 2,760 ng/ml. The relationship between the occurrence of hyponatremia secondary to water intoxication and rhabdomyolysis was sugge

An experimental evaluation of assessment reactivity in ecological momentary assessment of alcoholics anonymous involvement and alcohol-related outcomes: One day at a time.

OBJECTIVE: Assessment reactivity refers to the extent to which repeated measurement of behavioral and psychological phenomena influences subsequent behavior change. The present study used ecological momentary assessment (EMA) to evaluate whether frequent self-report influences Alcoholics Anonymous (AA)-related processes and alcohol-related outcomes. METHOD: Participants (N = 192; 63% female, Mage = 42.95) were assigned to either a 6-month EMA condition (n = 133), completing twice-daily surveys in addition to baseline and 3- and 6-month follow-up assessments, or a minimal assessment condition
