More than a drink: A rare anaphylactic reaction to sparkling water.

Anaphylaxis is a potentially life threatening, type I hypersensitivity reaction which can occur within seconds to minutes after exposure to an allergen. Sulfites have been implicated in causing such reactions with symptoms ranging from mild to potentially life threatening. Here we present a patient who had an anaphylactic reaction secondary to exposure to sulfites found in sparkling water.

Cromolyn sodium in ocular allergic diseases.

Cromolyn sodium is a highly effective topical anti-allergenic solution for the treatment of vernal, giant papillary, ragweed, and atopic conjunctivitis. It acts by inhibiting the release of histamine and thereby prevents the inflammatory response by subacute type I hypersensitivity reactions. The following paper reviews the drug action of cromolyn sodium and discusses its value in the treatment of type I ocular allergic diseases.

Regulatory issues concerning AETT and 6-MC.

Acetylethyltetramethyltetralin, a widely used synthetic fragance material, was found to cause neurotoxic effects in animals. 6-Methylcoumarin, another fragance material, was determined to be a photocontact allergen. Insufficient information on their usage, consumer exposure and likely hazard to human health under conditions of use impair regulatory decisions. FDA's ability to protect consumers against a health hazard is greatly influenced by the availability of such data.

Hen's Egg Allergy: Immunological Mechanisms, Epidemiology, and Predictors of Persistence in Childhood.

Hen's egg allergy is one of the most common food allergies worldwide and has a significant impact on children's health. Hen's egg allergy is primarily immunoglobulin E (IgE)-mediated reaction, and Gal d 1-5 are the most common allergens. Filaggrin mutations impair skin barrier function, promoting sensitization and persistent allergy. Family history, early atopic dermatitis, and limited microbial exposure can increase the risk of egg allergy. Additionally, there is a difference in the prevalence of hen's egg allergy between Eastern and Western countries. Egg allergy is closely linked to moderat…

The Basophil Activation Test in Allergy Diagnostics: Clinical Applications, Methodological Challenges, and Proposed Clinical Frameworks.

The basophil activation test (BAT) is a modern in vitro functional assay used for the diagnosis of immunoglobulin E-mediated allergic reactions. The method is based on flow cytometric assessment of activation marker expression on peripheral blood basophils after stimulation with specific allergens. BAT has gained increasing clinical relevance in allergology, particularly in patients with anaphylaxis risk, polysensitization, drug hypersensitivity, and inconclusive skin or serological test results. This narrative review summarizes current evidence on the diagnostic performance and clinical appli…

Eosinophil-epithelial interactions mediate protective intestinal remodeling during food allergy.

Food allergies are associated with progressive gastrointestinal symptoms driven by exacerbated mucosal type-2 immunity. Here, we investigated whether cellular interactions between the gut epithelium and innate immune cells regulate the severity of allergic symptoms in mice. Using the BALB/c OVA-alum food allergy model, we observed that repeated oral allergen challenges remodel the gut epithelium, expanding tuft cells, while shifting the intestinal stem cell niche toward a fetal-like repair state. Using uLIPSTIC, we systematically characterized in vivo immune-epithelial interactions and found t

Can Micro- and Nanoplastics Modify Food-Allergy-Relevant Pathways?-A Comprehensive Narrative Review.

Micro- and nanoplastics (MNPs) are widely detected in food, drinking water, food-contact materials, and human biological samples. This narrative review evaluates whether experimental evidence supports a role for MNPs as modifiers of food-allergen digestion, intestinal-barrier function, microbiota composition, and immune tolerance. In vitro studies indicate that protein-corona formation can alter allergen conformation, epitope accessibility, and proteolysis, although effects vary by polymer, particle size, dose, and digestive model. Rodent studies provide evidence that MNP exposure can disrupt

International perspective on obtaining disease modification with food allergy therapies.

BACKGROUND: A disease-modifying therapy aims to alter natural history by delaying or reversing disease progression. OBJECTIVE: We examined expert opinion regarding the concept of disease modification in food allergy to assess consensus, definitions, and identification of relevant biomarkers. METHODS: We contextualized relevant definitions to terms such as desensitization, sustained unresponsiveness, tolerance, and remission; and evaluated evidence-based disease-modifying effects of therapeutic strategies. RESULTS: Evidence from oral, epicutaneous and sublingual immunotherapy demonstrates th

Challenges in diagnosing occupational allergic contact dermatitis: a case report.

BACKGROUND: In Malaysia, occupational allergic contact dermatitis (ACD) is often under-reported. This case report describes a chemical engineer who developed possible ACD, likely due to workplace allergen exposure. CASE PRESENTATION: He presented with a 4-month history of intensely itchy rashes on both hands, which improved during work breaks. A dermatological examination revealed lichenified, pruritic papules with well-defined borders on the palmar surfaces of both hands. A skin patch test identified reactions to five allergens, including 'fragrance mix,' 'methyldibromo glutaronitrile,' 'cli…

Colophony in Dentistry: Routes of Exposure and Reported Reactions.

Colophony is a solid form of resin derived from coniferous trees that has both adhesive and water-resistant properties. For these reasons, this allergen is incorporated into many personal care products, medications, and occupational materials, and is thus commonly implicated in allergic contact dermatitis. Dedicated "dental" allergen series often include colophony, but dermatologists are likely not well-versed on its use in a dental setting. There are several routes of exposure to colophony via dental materials, including periodontal dressings, cements, and varnishes. Contact with these produc

ILC2-derived LIF licences progress from tissue to systemic immunity.

Migration and homing of immune cells are critical for immune surveillance. Trafficking is mediated by combinations of adhesion and chemokine receptors that guide immune cells, in response to chemokine signals, to specific locations within tissues and the lymphatic system to support tissue-localized immune reactions and systemic immunity1,2. Here we show that disruption of leukaemia inhibitory factor (LIF) production from group 2 innate lymphoid cells (ILC2s) prevents immune cells leaving the lungs to migrate to the lymph nodes (LNs). In the absence of LIF, viral infection leads to plasmacytoid

Intra-Airway Treatment with Synthetic Lipoxin A4 and Resolvin E2 Mitigates Neonatal Asthma Triggered by Maternal Exposure to Environmental Particles.

Particulate matter in the air exacerbates airway inflammation (AI) in asthma; moreover, prenatal exposure to concentrated urban air particles (CAPs) and diesel exhaust particles (DEPs) predisposes the offspring to asthma and worsens the resolution of AI in response to allergens. We previously tested the hypothesis that such exposure impairs the pathways of specialized proresolving mediators that are critical for resolution and found declined Lipoxin A4 (LxA4) and Resolvin E2 (RvE2) levels in the "at-risk" pups of exposed mothers. Here, we hypothesized that supplementation with synthetic LxA4 o…

Stabilization of activated mast cells by ORAI1 inhibitor suppresses peanut-induced anaphylaxis and acute diarrhea.

Mast cell (MC) activation triggered by immunoglobulin E (IgE)-antigen crosslinking involves intracellular Ca2+ influx through the ORAI1 channel, which precedes granule exteriorization and de novo synthesis of mediators. Pharmacologically suppressing MCs via the inhibition of the ORAI1 Ca2+ channel may represent a potential strategy for preventing anaphylaxis. This study demonstrated that peanut-induced anaphylaxis in sensitized mice resulted in significant hypothermia and acute diarrhea. Utilizing the Mcpt5cre-DTA mouse model, we demonstrated that this anaphylactic response was mediated by IgE

Rush immunotherapy in two cats with atopic skin syndrome.

CASE SERIES SUMMARY: Two cats with feline atopic skin syndrome (FASS) were included in this case series. They were diagnosed with FASS by a combination of history, physical examination and exclusion of other pruritic diseases. They underwent rush immunotherapy (RIT) after determination of offending environmental allergens by either serum IgE or intradermal testing. Cats were premedicated with an antihistamine and hospitalized for the day to undergo the procedure and to ensure adequate observation. Allergen extracts were administered subcutaneously at increasing concentrations every 30 mins unt…

Functional role of kallikrein 5 and proteinase-activated receptor 2 in eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, food antigen-driven, inflammatory disease of the esophagus and is associated with impaired barrier function. Evidence is emerging that loss of esophageal expression of the serine peptidase inhibitor, kazal type 7 (SPINK7), is an upstream event in EoE pathogenesis. Here, we provide evidence that loss of SPINK7 mediates its pro-EoE effects via kallikrein 5 (KLK5) and its substrate, protease-activated receptor 2 (PAR2). Overexpression of KLK5 in differentiated esophageal epithelial cells recapitulated the effect of SPINK7 gene silencing, including barr
