Transdermal nitroglycerine enhances spinal neostigmine postoperative analgesia following gynecological surgery.

BACKGROUND: Intrathecal neostigmine causes analgesia by inhibiting the breakdown of acetylcholine. Experimental data suggest that the production of endogenous nitric oxide is necessary for tonic cholinergic inhibition of spinal pain transmission. The purpose of this study was to determine whether association of transdermal nitroglycerine would enhance analgesia from a low dose of intrathecal neostigmine in patients undergoing gynecologic surgery during spinal anesthesia. METHODS: Forty-eight patients were randomized to one of four groups. Patients were premedicated with use of 0.05-0.1 mg/kg

Reduction by fentanyl of the Cp50 values of propofol and hemodynamic responses to various noxious stimuli.

BACKGROUND: Propofol and fentanyl infusion rates should be varied according to the patient's responsiveness to stimulation to maintain satisfactory anesthetic and operative conditions. However, somatic and autonomic responses to various noxious stimuli have not been investigated systematically for intravenous propofol and fentanyl anesthesia. METHODS: Propofol and fentanyl were administered via computer-assisted continuous infusion to provide stable concentrations and to allow equilibration between plasma-blood and effect-site concentrations. The propofol concentrations needed to suppress eye

[Radiofrequency perforation in pulmonary valve atresia and intact ventricular septum].

METHODS: Four neonates with Pulmonary Atresia and intact Ventricular Septum underwent Radiofrequency Valvotomy in our institution from October 1994 to June 1995. All patients had "membranous" atresia with confluent and normal size pulmonary branches; one of them had abnormal tricuspid tensor apparatus and severe right ventricular outflow tract hypertrophy. The right ventricle was considered of sufficient size to support biventricular circulation in all cases. In three patients the pulmonary arteries were supplied by the arterial duct maintained open with intravenous infusion of Prostaglandins.

[Alterations in number of rabbit myocardial beta-adrenergic receptors in endotoxic shock: down regulation in hyperdynamic sepsis model and effects of cytokines administration].

We investigated alterations in myocardial beta- and beta 1-adrenergic receptor (BAR and B1AR) number during hyperdynamic state induced by endotoxin or cytokines. [METHODS] Twenty-nine Japanese White rabbits were divided into 2 groups. Hearts were removed 18 h after intraperitoneal administration of sterile saline (SAL) or E. coli endotoxin (LPS; 50 micrograms/kg) (Group E, n = 12), or 3 h after intravenous injection of SAL or cytokines (interleukin 1-beta; 5 micrograms/kg followed by 25 ng/kg/min for 2 h, or tumor necrosis factor; 5 micrograms/kg) (Group C, n = 17). BAR and B1AR numbers were d

Electrical fascicle identification as an adjunct to nerve repair.

In view of the tragic consequences of median nerve and/or ulnar nerve paralysis and the biologic problems inherent in nerve healing, it seems logical that the surgeon should make a serious effort to control those problems that lie within his or her power: to identify and align those principal sensory and motor pathways in the severed nerve ends. I find that this is feasible. For over 15 years, I have used the electrical fascicle identification method, repairing 49 major upper extremity nerves. (A single radial nerve case was lost to follow-up.) As with any new technique, problems have been ide

A national survey of American Pediatric Anesthesiologists: patient-controlled analgesia and other intravenous opioid therapies in pediatric acute pain management.

BACKGROUND: The influence of patient characteristics, institutional demographics, and published practice guidelines on the provision of IV opioid analgesia, particularly as delivered through a patient-controlled analgesia (PCA) delivery device, to pediatric patients is unknown. METHODS: We sent a national, web-based, descriptive survey of pediatric pain management practice to select members of the Society for Pediatric Anesthesia to assess institutional demographics, availability and implementation of IVPCA and PCA by proxy, and recalled occurrence of serious and life-threatening opioid-relat

Intraoperative breast cancer margin evaluation using fluorescence imaging with intravenous injection of indocyanine green (BREASTIFLU-1): study protocol for a randomized diagnostic accuracy trial of quantitative dose-timing assessment.

BACKGROUND: Current intraoperative margin assessment techniques lack the accuracy needed for clinical practice. Indocyanine green fluorescence imaging (ICG-FI) offers a useful technique for detection of tumoral tissue, including breast cancer (BC). There is a great inconsistency in the literature regarding the use of ICG for BC fluorescence imaging mainly concerning ICG dosing and the timing of administration. This study aims to determine the dose and timing of intravenous (IV) ICG administration that provides the optimal diagnostic accuracy for ICG-FI margin assessment during breast-conservin

Safety and effectiveness of intravenous tranexamic acid for posterior lumbar interbody fusion in patients on preoperative anticoagulants: a retrospective case-control study.

BACKGROUND: Tranexamic acid (TXA) is a hemostatic agent used in posterior lumbar interbody fusion (PLIF). However, reports on its use in patients on preoperative anticoagulants are scarce. This study evaluates the safety and efficacy of preoperative 1 g TXA in such patients. METHODS: This study retrospectively analyzed the medical records of patients who underwent PLIF and had received anticoagulants within 1 week before surgery between January 2021 and June 2023. After general anesthesia, at 15 min before skin incision, patients who received intravenous TXA were recorded as the observation g

Preoperative Seizure Control and Perioperative Anesthetic Outcomes in Children Receiving Levetiracetam Versus Phenytoin: A Hospital-Based, Prospective, Comparative Cohort Study.

Background and objective Epilepsy is a common neurological disorder in children, and the perioperative period is associated with an increased risk of seizures and anesthetic complications. Intravenous phenytoin loading is associated with hemodynamic instability, whereas chronic oral therapy has minimal intraoperative cardiovascular effects. In contrast, levetiracetam has more favorable pharmacokinetics and fewer drug interactions. However, evidence comparing perioperative outcomes between levetiracetam and phenytoin in elective pediatric surgical settings remains limited. This study aimed to c

The Clean Pass: Upper Limb Impalement with Complete Neurovascular Preservation.

INTRODUCTION: Impalement injuries of the upper limb represent a rare and striking form of penetrating trauma, often associated with significant risk of neurovascular and tendinous damage due to the compact anatomical arrangement of the forearm. Despite their alarming external appearance, actual morbidity is determined by the trajectory relative to critical structures. Effective management requires careful pre-hospital stabilization, thorough pre-operative assessment, appropriate imaging, and meticulous surgical extraction under direct visualization to minimize complications. CASE REPORT: A 29

Development of new albumin-binding radiotracers for PET imaging of cerebrospinal fluid flow in the glymphatic system.

BACKGROUND: The glymphatic system is a recently discovered brain-wide clearance system that allows the cerebrospinal fluid (CSF) flow to clear metabolic waste, but the tools for the quantitative and non-invasive investigation of its function and activity especially in humans is lacking, hindering studies on glymphatic system physiology and therapeutic potential of glymphatic drug delivery and modulation. We postulated that albumin-binding radiotracers could be used to this end by binding to the endogenous protein in CSF, constituting a macromolecular, biological radiotracer, allowing for the v

Cannabinoid Hyperemesis Syndrome Presenting as Postoperative Nausea and Vomiting in a Chronic Cannabis User: A Case Report.

Cannabinoid hyperemesis syndrome (CHS) is a paradoxical condition seen in chronic cannabis users, marked by recurrent nausea, vomiting, and abdominal discomfort. Although more widely recognized in emergency medicine, CHS remains underdiagnosed in the perioperative setting, where its symptoms may be misattributed to common postoperative phenomena such as anesthetic effects, opioid-induced nausea, or surgical complications. This diagnostic gap can delay appropriate management and lead to unnecessary interventions. We report the case of a 40-year-old woman with a two-year history of daily cannabi

Environmental and patient safety outcomes of a health-system Green Anesthesia Initiative (GAIA): a retrospective observational cohort study.

BACKGROUND: Inhaled anaesthetics are greenhouse gases. However, changes in the delivery of inhaled anaesthetics can mitigate environmental impact. We hypothesised that system-wide changes to the delivery of anaesthesia care would reduce environmental harm without compromising patient outcomes. METHODS: We launched the Green Anesthesia Initiative (GAIA) in March, 2022, with the aims of reducing the use of nitrous oxide, using less environmentally harmful inhaled fluorinated ethers, and increasing intravenous anaesthetic use. In this retrospective cohort study, we used electronic health record

Donor Inhalation of Nebulized Dexmedetomidine Alleviates Ischemia-Reperfusion Injury in Rat Lung Transplantation.

INTRODUCTION: The occurrence of lung ischemia-reperfusion injury (LIRI) after lung transplantation results in primary graft dysfunction (PGD) in more than 50% of cases, which seriously affects the prognosis of recipients. Currently, donor lung protection is the focus of research on improving graft survival in lung transplant recipients. Dexmedetomidine (Dex) is a widely used general anesthesia adjuvant in clinical practice to alleviate ischemia-reperfusion injury in the lungs, liver, heart, kidneys, and brain. However, intravenous infusion of Dex can cause negative effects on the cardiovascula
