Outpatient treatment of sleep apnea syndrome with CO 2 laser, LAUP: laser-assisted UPPP results on 46 patients.

The pharyngeal airway obstruction during sleep in the obstructive sleep apnea syndrome (OSAS) can be improved after treatment by laser-assisted uvulopalatoplasty (LAUP), a new technique performed under local anesthesia. It permits surgery to be performed without hospitalization or general anesthetic. LAUP has many advantages over the traditional uvulopalatopharyngoplasty (UPPP). It is simple, reliable, hemostatic, sterilizing, and relatively painless. Our experience with LAUP in sleep apnea syndrome is described in 46 patients from December 1988 to May 1993. Among 40 patients classified as suc

[Effects of nasal continuous positive airway pressure (NCPAP) on nocturnal renal function in obstructive sleep apnea syndrome (OSAS)].

Nocturnal renal function was examined in 8 patients with obstructive sleep apnea syndrome (OSAS) and the effects of nasal CPAP (NCPAP) on renal function were also studied. Nocturia was observed more than twice in all cases when no treatment was performed, but it disappeared after initiation of NCPAP. Fractional nocturnal urine volume and creatinine clearance decreased significantly from 1.36 +/- 0.15 ml/min to 0.75 +/- 0.20 ml/min (p less than 0.01) and from 116.8 +/- 46.5 ml/min to 101.1 +/- 33.0 ml/min (p less than 0.05), respectively, after initiation of NCPAP. Expansion hyoidplasty: a potential surgical procedure designed for selected patients with obstructive sleep apnea syndrome. Experimental canine results.

Some patients with "idiopathic" obstructive sleep apnea syndrome have an inspiratory, circumferential, passive collapse of the hypopharyngeal lumen as the mechanism of airway obstruction. Permanent tracheotomy has offered documented dramatic resolution of the airway obstruction with reversal of the associated cardiopulmonary sequelae. Tracheotomy is not, however, without psychological and physical complications detracting from its use. In hope of developing a surgical alternative, the expansion hyoidplasty was conceived. The hyoid bone is transected just medial to each lesser cornu. The trisec

Respiratory pauses and apnea during daytime sleep in normal infants during the first year of life: longitudinal observations.

Respiratory pauses (3-10 sec in duration), apnea (less than 10 sex in duration), and periodic respiration observed in thoracic respirograms were measured in 226 polysomnograms obtained on 17 normal infants during the first year of life. All subjects had one or more respiratory pauses in a majority of recordings; 35% had respiratory pauses in all recordings; 75% of respiratory pauses were associated with body movement. There is marked intersubject and even intrasubject variability in respiratory pause rates. The range of mean respiratory pause rates among subjects was 2.0 - 14.4/h, and for sing

Beyond a Single Hypoxic Burden Value: Hypoxic Burden Ratio Assessed by Peripheral Arterial Tonometry in Obstructive Sleep Apnea.

PURPOSE: Hypoxic burden (HB) integrates the depth and duration of nocturnal oxygen desaturation in obstructive sleep apnea (OSA), but a single HB value may conceal distinct hypoxemic profiles. We explored the relationship between two automated HB measures derived from peripheral arterial tonometry-based home sleep apnea testing (PAT-HSAT) data: desaturation area under the SpO2 baseline (HB_baseline) and area under 90% SpO2 (HB_<90%). METHODS: We performed a retrospective cross-sectional study of 2136 adults with OSA, diagnosed by a peripheral arterial tonometry-derived apnea-hypopnea index ba

Development of a Multimodal Obstructive Sleep Apnea Diagnostic Prediction Model Using Two-Dimensional Facial Images and Clinical Data.

Obstructive sleep apnea (OSA) is a prevalent disorder in middle-aged and obese men with increased risk of cardiovascular disease, metabolic dysfunction, and neurocognitive impairments. Delays in diagnosis and treatment of patients with OSA increase long-term all-cause mortality. In this study, we developed a multimodal artificial intelligence (AI) model that utilizes two-dimensional facial photograph, cephalometric radiograph, and clinical data to enhance OSA screening. Clinical parameters of sex, age, body mass index, neck circumference, and abdominal circumference and sleep questionnaires we

Antepartum obstructive sleep apnea screening and its association with postpartum respiratory depression: a retrospective cohort study (2020-2025).

BACKGROUND: Obstructive sleep apnea is not routinely screened for in the peripartum population, and its association with postpartum respiratory depression remains unclear. We evaluated positive antepartum obstructive sleep apnea screening and its association with postpartum respiratory outcomes. METHODS: We conducted a single-center retrospective cohort study of parturients undergoing routine antepartum obstructive sleep apnea STOP screening (loud snoring, daytime tiredness, observed apnea, high blood pressure) from July 2020 to June 2025. Delivery encounters with at least one STOP response w

Association of parasagittal dural space calcification with body mass index, non-rapid eye movement sleep, and sleep apnea.

STUDY OBJECTIVES: The glymphatic-meningeal lymphatic system plays a crucial role in brain waste clearance. Parasagittal dural (PSD) space contains meningeal lymphatic vessels. Calcification in the PSD space is an under-recognized sign. This study aims to explore the potential associations between PSD space calcification and demographic as well as sleep-related factors. METHODS: In this retrospective study, consecutive patients who visited a sleep clinic between September 2015 and December 2023 were identified. All included individuals underwent a standard polysomnographic examination and a he

Obstructive sleep apnea as a potential mechanistic link between second-generation antipsychotics and metabolic syndrome: a narrative review.

This narrative review examines why metabolic syndrome is highly prevalent among individuals treated with second-generation antipsychotics (SGAs), yet weight gain alone does not fully explain this elevated risk. Metabolic disturbances frequently emerge soon after antipsychotic initiation, even without clinically significant changes in body mass, suggesting weight-independent mechanisms. Emerging evidence indicates that SGAs may influence respiratory regulation and increase vulnerability to obstructive sleep apnea (OSA), a common but underrecognized comorbidity in psychiatric populations. Association between left ventricular end-diastolic volume and cardiovascular disease in systemic hypertensive patients with obstructive sleep apnea.

BACKGROUND AND PURPOSE: Systemic hypertension and obstructive sleep apnea (OSA) are closely linked conditions that substantially increase the risk of cardiovascular disease (CVD). Left ventricular end-diastolic volume (LVEDV) is a crucial echocardiographic marker of cardiac remodeling; however, its prognostic value in patients with concomitant systemic hypertension and OSA remains poorly defined. This study aimed to examine the association between LVEDV and the incidence of CVD in this high-risk population. METHODS: This retrospective cohort study included patients with confirmed systemic hyp

Primary care versus sleep specialist pathways for obstructive sleep apnea with home testing: access, adherence, and treatment personalization.

PURPOSE: Although expanding obstructive sleep apnea (OSA) management capabilities has been a longstanding goal, patients report higher positive airway pressure (PAP) therapy compliance when cared for by specialists at centers accredited by the American Academy of Sleep Medicine (AASM) [1]. At Mayo Clinic, primary care providers (PCPs) are now authorized to order home sleep apnea testing (HSAT) and subsequent therapy, eliminating the need for consultation with Sleep Medicine. This study evaluates whether patients diagnosed with OSA through primary and specialty care pathways experience comparab

Receipt of glucagon-like peptide-1 receptor agonists among rural patients with obstructive sleep apnea and excess weight.

RATIONALE: Successful weight loss is difficult for those with obstructive sleep apnea (OSA), and gaps in weight management care are more pronounced in rural areas. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are promising agents to improve the effectiveness of weight management care in patients with OSA and excess weight living in rural areas. OBJECTIVE: We sought to examine the association of rural residence (vs urban) with receipt of GLP-1RAs among patients with OSA and excess weight. METHODS: Using medical record and administrative data from the Veterans Health Administration, we

Characterizing inspiratory duty cycle compensation to flow-limited breathing during drug-induced sleep endoscopy in obstructive sleep apnea.

Inspiratory duty cycle (IDC), the fraction of inspiratory time relative to total breath duration, serves as an adaptive response to flow-limited ventilation in obstructive sleep apnea (OSA). IDC compensation remains incompletely characterized in the context of OSA pathophysiology. We studied the relationship of IDC and flow-limited breathing in OSA during drug-induced sleep endoscopy (DISE). Eighty-two adults with OSA underwent DISE with continuous positive airway pressure (CPAP) titration. Airflow (VI), tidal volume (TV), and IDC were measured across varying levels of flow-limited breathing.

Integrating phenotypic information of obstructive sleep apnea and deep representation of sleep-event sequences for cardiovascular risk prediction.

BACKGROUND: Advances in wearable and machine learning technologies have opened new possibilities for predicting cardiovascular (CV) risk. Moreover, the association between obstructive sleep apnea (OSA) and CV risk has been well recognized. However, how to integrate such information effectively to enhance risk prediction remains unclear. This study seeks to explore effective strategies for incorporating OSA phenotypic information and overnight physiological information for precise prediction of CV adverse outcome, as well as to identify the most significant features among a broad spectrum of ri

Association between Attention Deficit Hyperactivity Disorder and Obstructive Sleep Apnea in Children: A Systematic Review and Meta-Analysis.

BACKGROUND: Obstructive Sleep Apnea (OSA) and Attention Deficit Hyperactivity Disorder (ADHD) are two relatively common conditions in the pediatric population, with multiple studies demonstrating a strong association between the two. Hypoxia and sleep fragmentation caused by OSA has been shown to exacerbate ADHD symptoms, while treatment for OSA, such as with continuous positive airway pressure or adenotonsillectomy, may improve ADHD symptoms. We aim to provide an updated systematic review and meta-analysis on the complex relationship between OSA and ADHD in children. METHODS: PubMed, Embase,

Borophene/PVDF Composite Film With High Piezoelectric Coefficient for Closed-Loop Monitoring and Intervention of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA), with the most direct symptom of snoring, is a common disorder that leads to intermittent hypoxia and cardiovascular risk. Here, we present a wearable borophene/PVDF (polyvinylidene fluoride) composite film integrated into a closed-loop OSA monitoring and intervention system. The composite film exhibits a piezoelectric coefficient of 31.9 pC·N-1, achieving effective biomechanical signal transduction within ±5% performance degradation after 10 000 compression or bending cycles. When attached to the neck, the composite detects biomechanical signals with high fidelit
