[Aortic insufficiency in certain so-called systemic diseases].

The authors present a retrospective study of all the patient followed up for systemic disease in the rheumatology Department of Bichat hospital between 1975 and 1984 in whom aortic regurgitation developed. Only rare or previously undescribed associations were retained: two MacDuffie syndromes, one adult form of Still's disease, one Takayashu's disease, one association of rheumatoid arthritis and Takayashu's disease, one rheumatoid arthritis, one Cogan's syndrome and two cases of disseminated lupus erythematosis. The authors use these cases and a review of the literature to discuss the possible

Cerebral angiography in ischemic strokes.

Angiography remains a very important means of evaluation of ischemic strokes: it confirms the diagnosis showing the occluded artery. It eventually shows the anastomotic pathways for cerebral circulation and it often finds the causes of stroke, among which the most frequent are atherosclerosis (70%) and fibromuscular hyperplasia (10%). The safer way to perform angiography is the retrograde route. Indications for angiography must be discussed according to the type of stroke, its onset, and the arterial territory involved.

[CNS involvement in primary angiitis--personal clinicomorphologic studies of 6 cases].

We report on the clinical symptomatology, diagnostic difficulty and inadequate possibilities for treatment of 6 patients suffering from primary agiitis of intracranial vessels. They died at age 36 to 64 years. The duration of the disease varied between 6 weeks and 11 1/2 years and was diagnosed, autoptically in all cases, as giant cell angiitis (2 cases) and, trombangiitis obliterans (4 cases). A degenerative cerebrovascular process had already been assumed clinically in four of the cases, and intracranial veins were also involved in three (very distinctly in one). The vascular disease was gen

Occlusive thromboaortopathy (Takayasu's disease) and pregnancy. Clinical course and management of 33 pregnancies and deliveries.

Twenty-seven Japanese patients with occlusive thromboaortopathy (Takayasu's disease) associated with 33 pregnancies and deliveries were followed up prospectively, from the prepregnant period. Inflammatory activity of the disease was apparently not enhanced by the pregnancy, but various cardiovascular-related events occurred in the perinatal period. During the intrapartum uterine contractions, marked elevation of systolic blood pressure was associated with 10 pregnancies, including 1 case of subsequent cerebral hemorrhage; this did not occur in 13 normal control gravidas. There were no maternal

Bone changes during prednisone treatment.

To evaluate changes inbone composition during treatment with corticosteroids, the bone mineral content (BMC, measured by photon absorptiometry) and the degree of bone mineralization (measured as the bone phosphorus/hydroxyproline ratio) were determined in 18 patients during prednisone treatment for haematological and connective tissue diseases. The prednisone dose ranged from 12 to 51 mg/day (mean 27). The BMC decreased significantly (mean 2.5%) during the studied 12 weeks of treatment, but the change did not correlate significantly to the prednisone dose. The degree of bone mineralization rem
